Cytoreductive Surgery (CRS) + HIPEC in India

CRS + HIPEC removes all visible cancer from the abdomen and then washes it with heated chemotherapy, for selected patients with pseudomyxoma, peritoneal mesothelioma, and ovarian, colorectal, appendix or gastric cancer spread to the abdominal lining. It is performed by specialist teams at NABH and JCI accredited hospitals in India.

📖 What is CRS + HIPEC?

The peritoneum is the thin, smooth lining that covers the inside of the abdomen and the organs within it. Some cancers – especially of the appendix, colon, ovary and stomach – can spread onto this lining as many small deposits. This is called peritoneal metastasis or peritoneal carcinomatosis. Rarer cancers such as pseudomyxoma peritonei (jelly-like tumour from the appendix) and peritoneal mesothelioma begin in the abdomen itself. Standard chemotherapy given into a vein reaches the peritoneum poorly.

Cytoreductive surgery (CRS) is a long operation in which the surgeon removes all visible tumour from the abdomen. This may include stripping the affected lining and removing parts of organs such as the bowel, spleen, gallbladder, omentum, uterus and ovaries. It is immediately followed by HIPEC (hyperthermic intraperitoneal chemotherapy): chemotherapy heated to about 41–43°C is circulated inside the abdomen for 30–90 minutes to kill microscopic cancer cells that cannot be seen. The heat makes the chemotherapy more effective, and very little reaches the rest of the body.

🖼️ Understand It in Pictures

💡 In simple words
Some cancers spread onto the lining of the belly. In one operation, the surgeon first removes all visible tumour (CRS), then fills the belly with heated chemotherapy (HIPEC) for a short time to kill tiny cancer cells left behind.

🩺 Conditions Treated

🟡
Pseudomyxoma peritonei (PMP)
Jelly-like tumour usually from the appendix; CRS + HIPEC is the standard treatment.
🎗️
Peritoneal mesothelioma
Rare cancer of the abdominal lining; CRS + HIPEC offers the best long-term survival.
🌸
Ovarian cancer
Stage III ovarian cancer at interval surgery after chemotherapy, and selected recurrences.
🔵
Colorectal and appendix cancer
Limited peritoneal spread without widespread liver or lung disease.
🟤
Gastric (stomach) cancer
Selected patients with limited spread, in specialist centres.
➕
Other selected tumours
Primary peritoneal cancer and some sarcomas, after tumour board review.

⚠️ Signs That May Need Evaluation

  • ➤Increasing abdominal swelling or girth
  • ➤Fluid in the abdomen (ascites)
  • ➤Abdominal pain or heaviness
  • ➤Early fullness and loss of appetite
  • ➤Unexplained weight loss
  • ➤Change in bowel habits or blockage symptoms
  • ➤Raised tumour markers (CEA, CA-125, CA 19-9)
  • ➤Peritoneal deposits seen on CT, PET-CT or during previous surgery
💡 CRS + HIPEC is only for carefully selected patients. It is not suitable when the cancer has spread widely to the liver, lungs or bones, when the peritoneal disease is too extensive to remove completely (very high PCI score), or when the patient is too weak for a long operation. These patients are treated with systemic chemotherapy, targeted therapy, PIPAC or supportive care.

🔬 Tests Before Treatment

TestWhy it is done
CT scan of chest, abdomen and pelvis with contrastMaps the spread of disease and rules out distant spread.
PET-CT and / or MRI abdomenShows the extent of peritoneal disease more clearly.
Tumour markers (CEA, CA-125, CA 19-9)Help with diagnosis and follow-up.
Biopsy and histopathology reviewConfirms cancer type, which decides whether HIPEC will help.
Diagnostic laparoscopy (selected cases)Measures the Peritoneal Cancer Index (PCI) before deciding.
Heart, lung, kidney and nutrition assessmentConfirms the patient can safely undergo a long surgery.
Colonoscopy / upper GI endoscopyChecks the primary tumour in bowel or stomach cancers.

🏥 The Procedure — Step by Step

CRS + HIPEC procedure steps
1
Tumour board decision
Surgical oncologist, medical oncologist and radiologist confirm that complete removal is possible.
2
Pre-surgery optimisation
Nutrition support, breathing exercises and bowel preparation.
3
Admission and anaesthesia
General anaesthesia with epidural for pain control.
4
Abdomen opened and PCI scored
The surgeon measures the spread of disease in 13 regions (PCI score).
5
Cytoreductive surgery
All visible tumour is removed, including affected lining and organs.
6
HIPEC
Heated chemotherapy (41–43°C) is circulated in the abdomen for 30–90 minutes.
7
Reconstruction
Bowel is joined again; a temporary stoma (bag) is sometimes needed.
8
Closure with drains
Drains and sometimes a feeding line are placed.
9
ICU care
Usually 2–3 days in ICU for monitoring of blood pressure, kidneys and blood counts.
10
Ward recovery and discharge
Gradual diet, walking and drain removal; discharge after 10–14 days in total.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
CRS + HIPEC (open, closed method)Abdomen closed while heated chemo circulatesMost patients
CRS + HIPEC (open, coliseum method)Abdomen kept open and chemo stirred by handEven distribution in complex cases
Interval CRS + HIPEC for ovarian cancerAfter 3 cycles of chemotherapy, with cisplatin HIPECStage III ovarian cancer (OVHIPEC-1 trial)
Laparoscopic CRS + HIPECKeyhole surgery for limited diseaseLow-volume disease, selected PMP
PIPACPressurised chemotherapy spray by laparoscopyPatients not suitable for CRS + HIPEC
📊 In the OVHIPEC-1 randomised trial (NEJM 2018), adding HIPEC to interval surgery for stage III ovarian cancer improved median overall survival from about 34 to 46 months; in pseudomyxoma peritonei, complete CRS + HIPEC gives 10-year survival of about 60–70% in large series.
