- Dr. J. B. Agrawal stood second in merit at the MS (General Surgery) examination of the University of Bombay in 1971. He introduced gastrointestinal staplers in India in 1982 and has conducted many workshops on gastrointestinal stapling. He was also one of the early enthusiasts of laparoscopic surgery and contributed the chapter on "Laparoscopic Cholecystectomy" in the textbook of surgery published by the Association of Surgeons of India.
- His name features in the 2015 edition of the Limca Book of Records for removing the largest (9 cm) gallstone laparoscopically. He has further acquired skills in bariatric surgery and robotic surgery and is one of the few senior surgeons to specialise in these fields. He has travelled to many countries as faculty, delivering lectures and demonstrating laparoscopic surgical techniques.
- Dr. Agrawal has been Past President of the Indian Medical Association (Bombay West Branch), the Association of Medical Consultants, the Maharashtra Chapter of the Association of Surgeons of India, the Indian Hernia Society (2014) and the Society of Endoscopic and Laparoscopic Surgeons of India (SELSI, 2017). He was the founder Secretary of the Indian Association of Gastrointestinal Endo Surgeons (IAGES).
- He is a Fellow of the International College of Surgeons (FICS) and the American College of Gastroenterology (FACG), and was conferred an Honorary Fellowship in Advanced Laparoscopic Surgery (FALS) by IAGES and the Fellowship of the International College of Laparoscopic Surgeons (FCLS).
- He received the Academic Excellence Award and delivered the Dr. G. M. Phadke oration in January 2015 at Nagpur during MASICON on the "Past, Present and Future of Groin Hernia Surgery", as well as the SELSI oration in 2017 at Jaipur on "T.A.R. for abdominal wall reconstruction".
