Know More About Procedure & Surgery
Robotic Esophagectomy (Esophageal Cancer Surgery) in India
Robotic esophagectomy removes cancer of the food pipe (oesophagus) and nearby lymph nodes through small cuts, with fewer lung complications than open surgery. It is done by senior GI cancer surgeons at NABH and JCI accredited hospitals in India, at a fraction of Western costs.
📖 What is Robotic Esophagectomy?
The oesophagus (esophagus) is the muscular food pipe that carries food from the throat to the stomach. Oesophageal cancer usually begins in its inner lining. The two main types are squamous cell carcinoma (common in Asia, Africa and the Middle East, linked to tobacco, hot drinks and alcohol) and adenocarcinoma (linked to long-term acid reflux and Barrett's oesophagus). The main symptom is difficulty swallowing that gets worse over time, often with weight loss.
An esophagectomy removes the diseased part of the food pipe and nearby lymph nodes. The stomach is then shaped into a tube and pulled up into the chest or neck to make a new food pipe. In robotic esophagectomy (robot-assisted minimally invasive esophagectomy, RAMIE), the surgeon controls fine robotic instruments and a 3D camera through small cuts in the chest and abdomen instead of opening the chest. For most patients, surgery is combined with chemotherapy or chemoradiation before the operation.

🩺 Conditions Treated
⚠️ Symptoms That May Need Evaluation
- ➤Difficulty swallowing, first solids then liquids
- ➤Food sticking in the chest
- ➤Unintended weight loss
- ➤Pain or burning behind the breastbone
- ➤Vomiting or bringing food back up
- ➤Hoarse voice or chronic cough
- ➤Long-standing acid reflux
- ➤Vomiting blood or black stools
🔬 Tests Before Treatment
| Test | Why it is done |
|---|---|
| Upper GI endoscopy with biopsy | Confirms cancer, type and exact location. |
| CT scan of chest and abdomen | Shows tumour size and spread to nodes or organs. |
| PET-CT scan | Detects hidden spread before major surgery. |
| Endoscopic ultrasound (EUS) | Shows how deep the tumour has grown and nearby nodes. |
| Bronchoscopy (upper / middle tumours) | Checks the windpipe is not involved. |
| Lung function test, ECG and echo | Confirms fitness for major chest surgery. |
| Nutrition assessment | Many patients need feeding support before surgery. |
🏥 The Procedure — Step by Step

⚖️ Types & Techniques
| Technique | What is done | Best for |
|---|---|---|
| Robotic esophagectomy (RAMIE) | Robot-assisted chest and abdominal surgery through small cuts | Most resectable cancers; precise node removal |
| Laparoscopic / thoracoscopic (MIE) | Keyhole surgery without a robot | Suitable patients; lower cost option |
| Ivor Lewis esophagectomy | Join made in the chest | Lower oesophagus and GE junction cancers |
| McKeown (three-stage) esophagectomy | Join made in the neck | Middle and upper oesophagus cancers |
| Open / transhiatal esophagectomy | Abdomen and neck cuts, or open chest | Patients unfit for chest keyhole surgery, complex cases |

🌱 Recovery After Treatment
🏆 Success Rate
| Outcome | Result |
|---|---|
| Complete tumour removal (R0) at expert centres | About 90–95% |
| 5-year survival after chemoradiation + surgery (CROSS trial) | About 47% |
| 5-year survival – early stage (stage I) | About 70–80% |
| Lung complications – robotic vs open (ROBOT trial) | 32% vs 58% |
| Operative mortality at high-volume centres | About 2–4% |
*Survival depends mainly on cancer stage, response to chemotherapy or chemoradiation, complete tumour removal, nutrition and overall fitness. Outcomes are best at high-volume centres.
🩹 Risks & Possible Complications
Esophagectomy is one of the biggest cancer operations. Possible risks include:
- •Pneumonia or other lung complications – about 15–30% (lower with robotic surgery)
- •Leak from the new join (anastomotic leak) – about 10–20%, most heal with drainage and feeding
- •Irregular heartbeat (atrial fibrillation) – about 15–20%
- •Hoarse voice from nerve injury – about 5–15%, often temporary
- •Narrowing of the join needing endoscopic stretching – about 10–30%
- •Chyle (lymph fluid) leak in the chest – about 2–4%
- •Death within 90 days – about 2–5%

Laparoscopic (non-robotic) esophagectomy costs about USD 6,500–7,000. Pre-surgery evaluation and chemotherapy or radiation are charged separately.
