📖 What is CAR T-Cell Therapy?

Our immune system has white blood cells called T-cells that attack germs and abnormal cells. Some blood cancers hide from these T-cells. In CAR T-cell therapy, the patient's own T-cells are collected from the blood, sent to a special laboratory and changed so they carry a new receptor called a chimeric antigen receptor (CAR). This receptor works like a homing device that finds a marker on cancer cells – for example CD19 on B-cell leukaemia and lymphoma, or BCMA on myeloma.

The changed cells are multiplied into millions and given back as a single drip (infusion), after a few days of mild chemotherapy that makes space for them. The CAR T-cells then grow inside the body and kill the cancer cells. It is a one-time treatment, used mainly when cancer has come back or has not responded to at least two earlier treatments. India now has its own approved CAR-T therapy (NexCAR19, approved by CDSCO in 2023), which costs much less than imported products.

🩺 Conditions Treated

🩸
B-cell acute lymphoblastic leukaemia (ALL)
Relapsed or resistant ALL in children and young adults.
🎗️
Diffuse large B-cell lymphoma
Lymphoma that came back or did not respond after 1–2 lines of treatment.
🔵
Follicular & mantle cell lymphoma
Relapsed low-grade or mantle cell lymphoma after several treatments.
🦴
Multiple myeloma
Relapsed myeloma using BCMA-targeted CAR-T cells.
🔁
Relapse after transplant
Blood cancers returning after a bone marrow transplant (selected cases).
🧪
Clinical trials
New CAR-T treatments under study in Indian centres for other cancers.

⚠️ Signs That CAR-T May Be Considered

  • ➤Leukaemia or lymphoma that came back
  • ➤No response to 2 or more chemotherapy lines
  • ➤Relapse after bone marrow transplant
  • ➤Not fit for or no donor for transplant
  • ➤Myeloma relapsing after several drugs
  • ➤Swollen lymph nodes returning after treatment
  • ➤Rising abnormal cells in blood or marrow
  • ➤Positive PET scan after chemotherapy
💡 CAR-T is not a first treatment. Most blood cancers are first treated with chemotherapy, immunotherapy and sometimes a bone marrow transplant, which cure many patients. CAR-T is considered when these fail. Patients must be fit enough, have suitable T-cells and controlled infections; very weak patients or those with uncontrolled disease in the brain may not be suitable.

🔬 Tests Before Treatment

TestWhy it is done
PET-CT scanShows how much lymphoma or myeloma is active and where.
Bone marrow biopsy and flow cytometryMeasures leukaemia or myeloma cells and confirms the CD19 or BCMA marker.
Complete blood count and lymphocyte countChecks if enough T-cells can be collected.
Heart (echo, ECG) and lung testsEnsure the body can handle side effects like cytokine release.
Liver and kidney functionCheck organ fitness for chemotherapy and cell infusion.
Infection screening (HIV, hepatitis B/C, others)Required before cell collection and treatment.
Brain MRI / lumbar puncture (selected)Checks for cancer in the brain or spinal fluid.

🏥 The Procedure — Step by Step

CAR T-Cell Therapy procedure steps
1
Evaluation (this package)
Haematologist consultation, review of reports, key tests and counselling on suitability, cost and timeline.
2
Approval and planning
Product chosen (Indian or imported), approvals and financial clearance completed.
3
T-cell collection (apheresis)
Blood is passed through a machine for 3–5 hours to collect T-cells; the rest is returned.
4
Cell manufacturing
Cells are engineered and grown in the lab – about 2–3 weeks for Indian products, up to 6–8 weeks for some imported ones.
5
Bridging therapy
Chemotherapy or radiation keeps the cancer under control while cells are made.
6
Lymphodepletion
3 days of mild chemotherapy (fludarabine and cyclophosphamide) make space for the new cells.
7
CAR-T infusion
The cells are given as a 15–30 minute drip, like a blood transfusion.
8
Close monitoring
Hospital or daily monitoring for 1–2 weeks for fever (CRS) and nerve side effects.
9
Staying near the centre
Patients stay within easy reach of the hospital for about 4 weeks after infusion.
10
Response check
PET-CT or bone marrow test at about 1 month and 3 months to check the response.

❓ Frequently Asked Questions

➤ What is the cost of CAR T-cell therapy in India?
The evaluation package starts from USD 1,500. The full treatment, including bridging therapy, the CAR-T product and post-infusion care, usually ranges from about USD 45,000 with a made-in-India product to about USD 140,000 with an imported product. This is far less than in the USA or Europe.
➤ What does the USD 1,500 package include?
It covers the specialist consultation, review of all your reports, key evaluation and counselling on whether CAR-T suits you, which product, the timeline and total cost. It does not include the CAR-T product itself.
➤ How long do I need to stay in India?
Usually about 2–3 months: evaluation and cell collection, 2–8 weeks of manufacturing with bridging therapy, the infusion, and about 4 weeks of close follow-up near the hospital.
➤ Is CAR-T available in India?
Yes. India approved its own CAR-T therapy (NexCAR19) in 2023 for B-cell leukaemia and lymphoma, and other products and clinical trials are available at selected centres. Some products may need special regulatory approval for each patient.
➤ How successful is CAR-T therapy?
About 80–90% of patients with relapsed B-cell ALL go into remission, and about 30–40% of patients with resistant large B-cell lymphoma stay in long-term remission. Results vary by cancer and patient.
➤ What are the main side effects?
Cytokine release syndrome (high fever, low blood pressure) and nerve side effects (confusion, speech problems). They are usually temporary and are treated with medicines like tocilizumab and steroids in hospital.
➤ Is CAR-T painful?
The infusion itself is like a blood transfusion and is not painful. Cell collection is similar to giving blood but takes a few hours.
➤ Who is suitable for CAR-T?
Patients with B-cell ALL, B-cell lymphoma or myeloma that came back or did not respond to earlier treatments, who are fit enough and have no uncontrolled infection. The specialist decides after evaluation.
➤ Can children have CAR-T?
Yes. Children and young adults with relapsed B-cell ALL are among the patients who benefit most. Paediatric haematology teams manage their care.
➤ Is CAR-T a cure?
For some patients it gives long-lasting remission that may be a cure, especially in lymphoma and ALL. Others may relapse and need further treatment such as a transplant.
➤ Why does the bridge treatment take so long?
The cancer must be kept under control while the cells are being made, which takes 2–8 weeks depending on the product. Bridging chemotherapy or radiation does this.
➤ Can a family member come with me?
Yes, and a caregiver must be with you at all times for at least 4 weeks after infusion. Up to two attendants can come on a medical attendant visa; we send the invitation letter.
➤ What follow-up is needed after going home?
Blood counts, immunoglobulin levels and scans at regular intervals, and vaccinations later. Your Indian doctors guide your local team and follow you up by video.
➤ Should I have a tele-consultation before travelling?
Yes. We strongly advise a video consultation with the CAR-T specialist before you travel, to confirm suitability, product choice, timeline and total cost.
