📖 What is Microvascular Decompression for Trigeminal Neuralgia?
The trigeminal nerve carries feeling from the face – forehead, cheek and jaw – to the brain. In trigeminal neuralgia, this nerve sends sudden, severe, electric-shock-like pain into one side of the face. Attacks last seconds to minutes and can be set off by light touch, chewing, talking, brushing teeth or a breeze. In most patients the cause is a small blood vessel (usually the superior cerebellar artery) pressing on the nerve where it leaves the brainstem. Over time this pressure damages the nerve's insulation and makes it fire abnormally.
Microvascular decompression (MVD) is surgery that treats this cause. Through a small opening in the skull behind the ear (a keyhole retrosigmoid craniotomy), the neurosurgeon uses an operating microscope, and sometimes an endoscope, to find the vessel touching the nerve. The vessel is gently moved away and a small Teflon pad is placed between them. The nerve is not cut, so face feeling is usually kept. MVD is also called the Jannetta procedure and gives the longest-lasting pain relief of all trigeminal neuralgia treatments.

💡 Medicines are the first treatment. Carbamazepine or oxcarbazepine control pain in most patients at first. Surgery is advised when medicines fail or cause side effects.
