Spina Bifida & Tethered Cord Release Surgery in India

Spina bifida is a birth defect in which the spine and spinal cord do not close completely during early pregnancy. In its open form (myelomeningocele) the nerves are exposed and must be covered soon after birth. Many children – and some adults – also develop a tethered spinal cord, where the lower end of the cord is stuck to surrounding tissue and is stretched as the child grows. Tethered cord release (detethering) frees the cord to protect walking, bladder and bowel function. In India these operations are done by paediatric neurosurgeons with nerve monitoring, paediatric intensive care and urology support.

📖 What are Spina Bifida and Tethered Cord?

The spine forms from the neural tube in the first 4 weeks of pregnancy. If the tube does not close, the result is spina bifida. In the United States about 1 in every 2,800 babies is born with it (CDC), and rates are higher where folic acid supplementation and food fortification are limited. Taking folic acid before and in early pregnancy reduces neural tube defects by up to 70%. Myelomeningocele is closed surgically within 24–72 hours of birth to prevent infection; about 8 in 10 of these babies also develop hydrocephalus needing a shunt or endoscopic treatment, and many have a Chiari II malformation (see foramen magnum decompression (Chiari malformation)).

A tethered cord occurs when the base of the spinal cord is fixed by a thick filum terminale, a fatty lump (lipomyelomeningocele), a dermal sinus, or scar from the earlier closure (re-tethering). As the child grows, the cord is stretched, causing back and leg pain, weakness, foot deformities, worsening scoliosis and bladder or bowel problems. Untethering surgery aims to stop further damage; nerves that are already badly damaged may not fully recover, so timely surgery matters (StatPearls: Tethered Cord Syndrome; NINDS).

🖼️ Understand It in Pictures

💡 In simple words
The spinal cord is like a long cable running down the back. In spina bifida, the bones around it did not close before birth. Sometimes the bottom of the cable gets stuck, so it is pulled tight as the child grows taller. This can cause pain, weak legs and wetting. The surgeon cuts the cable free using a microscope and nerve monitoring, so it is no longer stretched.

From hidden to open spina bifida

Spina bifida occulta, meningocele, myelomeningocele and lipomyelomeningocele explained

🩺 Conditions Treated

👶
Myelomeningocele (open spina bifida)
Closure of the open defect in newborns; later care of shunts and tethering.
💧
Meningocele
Repair of a fluid sac on the back.
🧀
Lipomyelomeningocele / spinal lipoma
Fatty lump attached to the cord; untethering and partial or near-total lipoma removal.
🧵
Tight filum terminale
Simple sectioning of a thick filum through a small incision.
♻️
Re-tethering after earlier repair
Scar release in children who worsen years after closure.
🕳️
Dermal sinus and split cord malformation
Tract removal, or removal of the bony spur in diastematomyelia.

⚠️ Warning Signs of a Tethered Cord

  • ➤Back or leg pain that worsens with bending or growth
  • ➤New weakness, tripping or change in walking
  • ➤Foot deformity (high arch, club foot) getting worse
  • ➤Bladder problems: wetting, infections, poor stream
  • ➤Constipation or bowel accidents
  • ➤Rapidly progressing scoliosis
  • ➤Skin signs on the lower back: dimple, hair tuft, fatty lump
  • ➤Numbness in the legs or saddle area
💡 Not every child with spina bifida or a low-lying cord needs surgery. Spina bifida occulta alone usually needs no treatment, and some children with a tethered cord seen on MRI but no symptoms are followed with regular examinations and urodynamic tests. Surgery is advised when there are new or worsening symptoms, or for certain lesions (such as dermal sinus tracts) with a risk of infection. The decision is made by a paediatric neurosurgeon together with a urologist and orthopaedic surgeon.

🔬 Tests Before Treatment

TestWhy it is done
Whole-spine MRIShows the level of the cord, filum, lipoma, syrinx and Chiari malformation.
Brain MRI or CTChecks hydrocephalus and shunt function.
Urodynamic studyMeasures bladder pressure and function before and after surgery.
Spine and hip X-raysScoliosis, bony defects and hip position.
Ultrasound of kidneys and bladderLooks for kidney damage from high bladder pressure.
Neurological and orthopaedic examinationBaseline muscle power, sensation and foot shape.
Blood tests and anaesthesia reviewFitness for anaesthesia; latex allergy precautions.

🏥 The Procedure — Step by Step

Spina bifida and tethered cord release surgery steps
1
Consultation in India
The paediatric neurosurgeon examines the child and reviews MRI and bladder tests.
2
Team planning
Neurosurgery, urology, orthopaedics and anaesthesia agree the plan; latex-free setup.
3
Anaesthesia and positioning
General anaesthesia with the child lying face down.
4
Neuromonitoring
Electrodes record leg and anal sphincter muscle responses throughout (IONM).
5
Exposure
Skin incision over the lower spine and a small laminectomy.
6
Open the dura
The covering of the cord is opened under the operating microscope.
7
Release the tether
The filum is cut, or the lipoma and scar are separated from the nerves, guided by monitoring.
8
Watertight closure
The dura is closed (sometimes with a graft) to prevent spinal fluid leakage.
9
Lie flat
The child lies flat for 1–3 days to protect the wound and closure.
10
Mobilise and rehab
Walking, physiotherapy and bladder care resume before discharge.

Fetal surgery (selected centres)Repair before birthSelected pregnancies; not covered here
📚 Evidence note: The MOMS randomised trial (Adzick et al., NEJM 2011) showed that repairing myelomeningocele before birth reduced the need for a shunt from 82% to 40% and improved walking, compared with repair after birth, but increased premature birth risks; most babies worldwide are still repaired after birth. In tethered cord release, published series report improvement or stabilisation of pain in most patients and stabilisation of motor function in the majority, while bladder function improves less often – which is why early surgery, before damage is established, is recommended (StatPearls: Tethered Cord Syndrome).

🌱 Recovery After Treatment

🛏️
First days
Lying flat for 1–3 days reduces the risk of spinal fluid leak.
🩺
Wound care
Keep the wound clean and dry; watch for swelling or clear fluid.
🚶
Physiotherapy
Gentle walking and leg exercises; orthoses if needed.
💧
Bladder care
Clean intermittent catheterisation or medicines continue as advised.
🏫
School
Most children return to school after 3–4 weeks.
📊
Lifelong follow-up
Yearly neuro, urology and spine checks; growth can cause re-tethering.

🏆 Success Rate

OutcomeResult
Back and leg pain after untetheringImproves in most patients
Motor functionStabilises in most; improvement in some
Bladder functionImproves less often; stabilisation is the main goal
Re-tethering after lipoma surgeryOccurs in a minority over years; may need repeat release
Shunt need after prenatal vs postnatal repair (MOMS trial)40% vs 82%
💡 Outcomes depend on the type of lesion, the child’s age, how long symptoms have been present and how much nerve damage exists before surgery. Untethering mainly prevents further loss; some recovery is possible but not guaranteed.

🩹 Risks & Possible Complications

  • •Spinal fluid leak or pseudomeningocele (may need repair)
  • •Wound infection or wound breakdown
  • •New or worse leg weakness or numbness (uncommon with monitoring)
  • •Worsening bladder or bowel function
  • •Meningitis (rare)
  • •Re-tethering in the future as the child grows
