Know More About Procedure & Surgery
MRgFUS Knifeless Brain Surgery for Parkinson's Disease in India
MR-guided Focused Ultrasound (MRgFUS) treats disabling Parkinson's tremor and essential tremor without any cut, skull opening or implant. It is done while you are awake, by a neurosurgeon, neurologist and neuroradiologist team at specialised Indian centres such as KIMS Hospitals, Hyderabad and Royal Care Super Speciality Hospital, Coimbatore.
📖 What is MRgFUS Knifeless Brain Surgery?
Parkinson's disease is a slowly progressive brain disease that affects movement. Many patients have a resting tremor – a 'pill-rolling' shaking of the hand or foot – that can make eating, writing, drinking or dressing very difficult. When medicines no longer control the tremor, or cause side effects, a procedure on the brain may be advised.
MRgFUS (Magnetic Resonance-guided Focused Ultrasound) focuses more than 1,000 ultrasound beams through the intact skull onto a tiny target deep in the brain – usually the thalamus (VIM nucleus) for tremor, or the globus pallidus for other motor symptoms. Where the beams meet, the tissue is heated to about 55 °C and a small lesion is made that blocks the faulty brain signals. MRI is used throughout to aim the beams and check the temperature in real time. There is no incision, no burr hole, no implant and no general anaesthesia, which is why it is often called 'knifeless' or 'incisionless' brain surgery.
MRgFUS is approved by the US FDA and by CDSCO (Government of India) for essential tremor and Parkinson's disease. More than 12,000 patients have been treated worldwide, and it has been available in India since August 2022.

🔬 Tests Before Treatment
| Test | Why it is done |
|---|---|
| Neurology examination & tremor rating scales | Confirms the diagnosis and measures how severe the tremor is (e.g. UPDRS / CRST scales), on and off medicines |
| MRI brain | Checks the brain anatomy and rules out other causes; used for planning the target |
| CT scan of the head | Measures the Skull Density Ratio (SDR) – the skull must let enough ultrasound energy through (usually SDR 0.40 or more) |
| Blood tests | Blood counts, clotting profile, kidney and liver function, blood sugar |
| ECG / heart check | Fitness to lie still for 2–4 hours and to receive sedation if needed |
| Memory & mood assessment | Patients with significant dementia or uncontrolled depression may not be suitable |
| Pre-procedure anaesthesia review | Planned, as per hospital protocol, before treatment day |
🏥 The Procedure — Step by Step

⚖️ MRgFUS vs Other Treatments for Parkinson's Tremor
| Option | What is done | Best for |
|---|---|---|
| Medicines (levodopa etc.) | Tablets taken daily | Early disease; first treatment for all patients |
| MRgFUS thalamotomy (this package) | Focused ultrasound lesion through the closed skull – no cut, no implant | Disabling tremor on one side; elderly or unfit for surgery; wants no implant |
| Deep Brain Stimulation (DBS) | Electrodes placed in the brain through small holes, connected to a battery in the chest | Tremor plus stiffness, slowness and fluctuations; both sides; adjustable |
| Gamma Knife thalamotomy | Radiation lesion – effect develops over months | Patients who cannot have MRgFUS or DBS |
📅 Your Day-by-Day Plan in India
| Day | What happens |

| Day 2–4 | OPD evaluation: neurology and neurosurgery consultations, CT head (skull density), MRI and blood tests |
| Day 5 | Admission, head shaving, frame fitting and MRgFUS procedure (2–4 hours) |
| Day 6–7 | Observation in hospital, review of tremor and discharge |
| Day 8–18 | Stay in hotel / guest house near the hospital, follow-up visits and fit-to-fly review |
| Day 19–21 | Fly home ✈️ |
🌱 Recovery After Treatment
🏆 Success Rate
| Outcome | Result |
|---|---|
| Immediate tremor reduction on the treatment table | Seen in most suitable patients |
| Average tremor improvement | About 70–75% (range from minimal to near 100%) |
| Parkinson's tremor improvement in trial (3 months) | About 62% vs 22% with sham |
| Essential tremor hand-tremor improvement (3 months) | About 47% vs almost 0% with sham |
| Long-term benefit | Improvement largely maintained at 3–5 years in essential tremor studies; some tremor may return |
Results are best for tremor of the hand and arm on one side. A denser skull lets ultrasound reach the target more easily, which is why the CT skull density check is important. Because Parkinson's disease is progressive, other symptoms will still need medicines and regular neurology follow-up.
🩹 Risks & Possible Complications
MRgFUS is generally considered safe and has no risk of wound infection, but like any brain procedure it can have side effects. Most are mild and temporary:
- •Headache, nausea or dizziness during the procedure – common and short-lived
- •Pain or swelling at the frame pin sites for a few days
- •Numbness or tingling of the lips, face or fingers – often temporary, sometimes long-lasting
- •Unsteady walking or imbalance – usually improves over weeks to 3–6 months
- •Weakness, slurred speech or involuntary movements on one side – uncommon, usually temporary
- •Very rarely, permanent neurological side effects or bleeding
- •Tremor may return partly over time and further treatment may be needed

Frequently Asked Questions
The procedure costs about USD 32,000 at our partner hospital, plus about USD 800–900 for the OPD evaluation. The package includes the room, procedure, consultations, routine investigations, pharmacy, consumables and food during the hospital stay. Hotel stay after discharge is about USD 35–50 per day.
Yes. There is no incision, no burr hole in the skull and no implant. Ultrasound waves pass through the closed skull and are focused on a tiny point in the brain under MRI guidance. Only the frame pins are placed under local anaesthesia.
Yes. You stay awake so the doctors can talk to you and test your tremor after each pulse, for example by asking you to draw a spiral. General anaesthesia is not needed.
The procedure itself takes about 2 to 4 hours, including frame fitting, MRI planning and the treatment pulses.
Plan for about 2 to 3 weeks: 2–3 days of OPD evaluation, 2–3 days in hospital and some days near the hospital for follow-up before you fly home.
Most suitable patients see the tremor reduce on the MRI table itself. Fine hand skills such as writing and eating keep improving over the next few weeks.
No. It treats tremor, usually on one side of the body. Parkinson's disease is progressive, so you will still need medicines and regular neurology follow-up for other symptoms.
Usually one side is treated first – normally the side that troubles you most. A second-side treatment may be considered after at least 9 months in selected patients, as advised by the specialist.
Patients with low skull density on CT, severe balance or walking problems, significant dementia, unstable heart disease, pacemakers or metal implants that are not MRI-safe, or who cannot lie still for a few hours, may not be suitable.
Common short-lived effects are headache, dizziness and nausea during the procedure. Some patients have numbness or tingling, or unsteady walking for a few weeks or months. Serious permanent side effects are rare.
MRgFUS has no cut or implant and a faster recovery, and suits patients with one-sided disabling tremor. DBS is adjustable, can treat both sides and also helps stiffness, slowness and fluctuations, but needs surgery and a battery. Your neurologist will advise which is right for you.
Yes. Age alone is not a barrier; it is often chosen for older patients who are not fit for open brain surgery, provided the evaluation is satisfactory.
Tremor is assessed at 1, 3, 6 and 12 months and then yearly, and an MRI brain is usually done at about 3 months. Most follow-up can be done online with reports from your home country.
The hospital issues a visa invitation letter for the patient and up to 2 attendants for the Indian e-Medical visa. We provide free airport pick-up, a coordinator, interpreter and help with hotel booking. Our medical opinion and estimate service is free.
