🧠 What is Endoscopic Spine Surgery?

Endoscopic spine surgery is a keyhole operation on the spine. Instead of a long cut and stripping the back muscles away from the bone, the surgeon passes a thin tube (about 7–10 mm wide) through a small cut in the skin. A high-definition camera and light (the endoscope) inside the tube show the nerves on a screen, and tiny instruments are passed through the same tube to remove what is pressing on the nerve — usually a slipped (herniated) disc or thickened bone and ligament.

It belongs to the wider family of minimally invasive spine surgery (MISS), which also includes tubular microdiscectomy, unilateral biportal endoscopy (UBE) and MIS / robotic fusion with screws placed through small cuts. The goal is the same as open surgery — to free the trapped nerve — but with less muscle damage, less blood loss, less pain after surgery and a shorter hospital stay.

💡 In simple words
Between the bones of your spine are soft cushions (discs). When a disc bulges out or the canal becomes narrow, it pinches a nerve and causes pain that runs down the leg or arm. In endoscopic surgery the doctor goes in through a tiny cut with a camera and takes out only the part that is pressing on the nerve. Most patients walk the same day.

🩺 Conditions Treated with Endoscopic / MIS Spine Surgery

💿
Lumbar Disc Herniation (Sciatica)
A slipped disc in the lower back pressing on a nerve root, causing leg pain, numbness or weakness. The most common reason for endoscopic discectomy.
🦴
Lumbar Spinal Stenosis
Narrowing of the spinal canal or nerve exit holes by thick ligament, bone spurs and facet joints — leg pain or heaviness while walking.
🧷
Foraminal / Lateral Recess Stenosis
The nerve is trapped as it leaves the spine. Endoscopic foraminotomy can open the exit without removing stabilising bone.
🦒
Cervical Disc Prolapse (Selected)
A soft disc in the neck pressing on one nerve root (arm pain). Posterior endoscopic foraminotomy may avoid a fusion in suitable cases.
🔁
Recurrent Disc Herniation
A disc that slips again at the same level after earlier surgery can often be removed endoscopically.
🔩
Spondylolisthesis / Instability
When a vertebra slips forward, decompression is combined with MIS or robotic TLIF fusion (screws and a cage through small cuts) — quoted separately.

⚠️ Symptoms That May Need Surgery

  • ➤Pain shooting from the buttock down the leg (sciatica) or from the neck into the arm
  • ➤Numbness, tingling or pins-and-needles in the leg, foot, arm or hand
  • ➤Weakness — for example foot drop, difficulty walking on heels or toes, weak grip
  • ➤Leg pain or heaviness after walking a short distance that eases on sitting or bending forward
  • ➤Pain that has not improved after 6–12 weeks of medicines, physiotherapy or injections
  • ➤Pain so severe that sleep, work and daily life are affected
  • ➤MRI showing a disc or narrowing that matches the symptoms
  • ➤Emergency: loss of bladder or bowel control, or numbness around the private parts (cauda equina syndrome) — needs urgent surgery
💡 Not every slipped disc needs an operation. Most sciatica from a disc herniation settles within 6–12 weeks with pain relief, staying active, physiotherapy and sometimes a nerve-root injection. Surgery is advised when pain persists despite good non-surgical care, when there is weakness that is getting worse, or urgently in cauda equina syndrome. Endoscopic surgery is also not suitable for every spine problem — major instability or deformity usually needs fusion.

🔬 Tests Before Surgery

TestWhy it is done
MRI of the Lumbar or Cervical SpineThe key test — shows the disc herniation or stenosis, its exact level and side, and the nerve being compressed
X-rays with Flexion–Extension ViewsCheck alignment and whether the spine moves abnormally (instability), which may change the plan from decompression to fusion
CT Scan of the SpineShows bone spurs, calcified discs and the size of the nerve exit holes; helps plan the endoscopic approach
Nerve Conduction Study / EMGWhen it is unclear whether symptoms come from the spine or a nerve elsewhere (e.g. diabetes, carpal tunnel)
Neurological ExaminationStrength, sensation, reflexes and straight-leg-raise test to match the MRI findings with symptoms
Blood Tests, ECG, Chest X-rayPre-anaesthesia fitness: blood count, sugar, clotting, kidney and liver function, viral markers; DEXA bone scan in older patients if fusion is planned
Lumbar spine MRI used to plan endoscopic spine surgery

🏥 The Procedure — Step by Step

Endoscopic spine surgery steps explained
1
Admission & preparation
Admission on the day of surgery or a day before, consent, anaesthesia check, and marking of the exact disc level.
2
Anaesthesia
General anaesthesia, or for some transforaminal procedures local anaesthesia with sedation so the patient can give feedback.
3
Positioning & level check
The patient lies face-down on a padded frame. Live X-ray (fluoroscopy) confirms the correct level before any cut is made.
4
Small incision
A cut of about 7–10 mm is made, usually slightly to the side of the midline (transforaminal) or over the space between the bones (interlaminar).
5
Dilators & working tube
Thin dilators gently push the muscle fibres apart (not cut), and a working tube is placed down to the spine.
6
Endoscope inserted
The camera is passed down the tube; a constant flow of saline keeps the view clear on the HD screen.
7
Nerve decompression
Using micro-instruments, burr and radio-frequency probe, the surgeon removes the herniated disc fragment and any thick ligament or bone spur pressing on the nerve.
8
Check the nerve is free
The nerve root is seen to be free and moving; bleeding points are sealed.
9
Closure
The tube is removed and the small cut is closed with one or two stitches or skin glue and a small dressing.
10
Walk & go home
Most patients walk within a few hours, start physiotherapy the next day and are discharged in 1–3 days; fusion cases stay 3–4 days.

⚖️ Types of Endoscopic & Minimally Invasive Spine Surgery

TechniqueHow it is doneBest for
Transforaminal endoscopic discectomy (TELD / PTED)Endoscope enters from the side through the natural nerve exit hole (foramen); often under local anaesthesiaForaminal and far-lateral disc herniations; many upper and mid lumbar levels
Interlaminar endoscopic discectomy (IELD)Endoscope enters from the back through the gap between two laminaeL5–S1 and central herniations; lateral recess stenosis
Unilateral biportal endoscopy (UBE)Two small portals — one for the camera, one for instrumentsLumbar canal stenosis and decompression of both sides through one side
Tubular microdiscectomy (MISS)Microscope through a 16–22 mm tube, muscle-splittingDisc herniations and stenosis where endoscopy is not suitable
MIS / robotic TLIFDecompression plus cage and screws placed through small cuts with navigation or robotSpondylolisthesis, instability, recurrent herniation with back pain
📊 A 2022 randomised trial in the BMJ (613 patients with sciatica) found percutaneous transforaminal endoscopic discectomy non-inferior to open microdiscectomy for leg pain at 12 months, with less blood loss and a shorter hospital stay. A 2022 meta-analysis (World Neurosurgery) found fewer overall complications with full-endoscopic discectomy (5.5% vs 10.4% in randomised trials). Your surgeon chooses the technique that suits your MRI.

📅 Your Day-by-Day Plan in India

DayWhat happens
Day 1Arrival in New Delhi — airport pick-up, hotel check-in, coordinator meeting
Day 2–3Spine surgeon consultation, MRI / CT review or repeat scans, blood tests, anaesthesia check
Day 3–4Admission and endoscopic spine surgery; walking the same evening
Day 4–6Ward stay, physiotherapy, pain control; discharge (1–3 days for discectomy, 3–4 days for MIS fusion)
Day 7–11Rest in a hotel / guest house near the hospital, daily walking, wound check
Day 12–14Stitch removal or wound review, surgeon follow-up and fit-to-fly letter; fly home ✈️ — follow-up by video consultation

🌱 Recovery After Surgery

🚶
Same Day
Walking with support within a few hours of surgery. Leg pain often eases straight away; some back soreness at the cut is normal.
🏠
Days 1–3
Discharge home or to the hotel. Short walks several times a day; avoid long sitting, bending and lifting.
🩹
Days 10–14
Wound check and stitch removal if used. Surgeon issues fit-to-fly clearance.
💼
Weeks 2–4
Many people return to desk work in this period; physiotherapy for core and back muscles begins.
🏋️
Weeks 6–8
Gradual return to heavier work. No heavy lifting before your surgeon allows it; after fusion, recovery is longer.
⚽
3 Months
Return to sports and full activity for most patients after discectomy, with surgeon approval.

🏆 Success Rate

OutcomeResult
No leg pain 2 years after full-endoscopic discectomy82% of patients; a further 14% had only occasional pain (Ruetten et al., Spine 2008)
Leg pain at 12 months vs open microdiscectomyNon-inferior — median pain score 7 vs 16 out of 100 (Gadjradj et al., BMJ 2022)
Overall complications vs open discectomy5.5% vs 10.4% in randomised trials (meta-analysis, World Neurosurgery 2022)
Recurrent disc herniationAbout 6% at 2 years — the same as open surgery (Ruetten et al., 2008)
Repeat surgery within 1 yearAbout 5% (BMJ 2022 trial)

Results are best when the MRI finding clearly matches the leg or arm pain and surgery is done before nerve damage becomes long-standing. Back pain from worn discs alone responds less well to any decompression operation.

🩺 Risks & Possible Complications

Endoscopic spine surgery has a lower complication rate than open surgery in trials, but every operation carries some risk. Your surgeon will explain these in detail:

  • •Recurrence — the same disc can slip again (about 5–6% in trials), sometimes needing another operation
  • •Temporary burning, tingling or numbness in the leg (dysaesthesia), especially after the transforaminal approach — usually settles in weeks
  • •Tear of the nerve covering (dural tear) with CSF leak — less common than with open surgery
  • •Incomplete removal of the disc or stenosis, or conversion to a small open operation
  • •Infection, bleeding or blood clots — rare
  • •Rare: nerve root injury with weakness, anaesthesia risks

💰 Cost of Endoscopic Spine Surgery in India

The cost of endoscopic spine surgery in India starts from about USD 6,000 for a one-level endoscopic decompression, plus USD 500–800 for the initial evaluation (consultation, scans and blood tests). Minimally invasive fusion with screws (MIS / robotic TLIF) costs more because of implants. ❓ Frequently Asked Questions

➤ How much does endoscopic spine surgery cost in India?
A one-level endoscopic decompression or discectomy starts from about USD 6,000 (economy room) and is about USD 6,600–7,300 in twin or single rooms. MIS or robotic fusion (TLIF) costs about USD 6,800–8,250 or more because of implants. Evaluation costs USD 500–800 extra.
➤ Is endoscopic spine surgery better than open surgery?
For a slipped disc, trials show leg-pain relief is as good as open microdiscectomy, with less blood loss, less muscle damage, a shorter stay and fewer overall complications. It is not suitable for every case — your surgeon decides based on your MRI.
➤ How big is the cut?
Usually about 7–10 mm — often closed with one or two stitches or skin glue.
➤ Will I be awake during the surgery?
Most patients have general anaesthesia. Some transforaminal procedures are done under local anaesthesia with sedation, so you are relaxed and comfortable.
➤ How long will I stay in hospital and in India?
Usually 1–3 days in hospital for endoscopic discectomy and 3–4 days if fusion is done. The total stay in India is about 2 weeks.
➤ When can I walk after surgery?
Most patients walk with support within a few hours of surgery, the same day.
➤ When can I fly home?
Usually after the wound check at around 10–14 days, once the surgeon gives a fit-to-fly letter. Walk in the aisle every hour during the flight.
➤ When can I return to work?
Many people return to desk work in 2–4 weeks. Heavy manual work usually needs 6–8 weeks or longer, as advised by your surgeon.
➤ Can the disc slip again?
Yes. About 5–6% of patients have a recurrence at the same level within 2 years, similar to open surgery. Good posture, core exercises and avoiding heavy lifting early help.
➤ Can endoscopic surgery treat spinal stenosis?
Yes, many cases of lateral recess, foraminal and central lumbar stenosis can be decompressed endoscopically (including UBE). If there is instability, fusion may still be needed.
➤ Is it suitable for neck (cervical) problems?
Selected cases — for example a soft disc pressing on one nerve root — can be treated with posterior endoscopic foraminotomy. Many cervical problems still need ACDF or disc replacement.
➤ Who is not a good candidate?
Patients with major instability, severe deformity, infection or tumour, or back pain without nerve compression usually need a different treatment.
➤ What should I send for a free opinion?
A recent MRI (within 3–6 months) as images or a CD, the report, X-rays, a short history, age, weight and current medicines.
