📖 What is Endoscopic Adenoidectomy?

The adenoids are a small pad of lymph (gland) tissue high at the back of the nose, behind the palate. They help fight germs in babies and young children, and usually shrink after the age of 5–7. In some children they become very large (adenoid hypertrophy) because of repeated infections or allergy. Large adenoids block the back of the nose and the openings of the ear tubes, causing mouth breathing, snoring, disturbed sleep, and fluid or infection in the ears.

Adenoidectomy is the surgical removal of the adenoids through the mouth or nose – there is no cut on the face or neck. In the endoscopic method, the surgeon looks with a thin HD camera (endoscope) and removes the adenoid tissue completely and precisely using a microdebrider or coblation wand. This gives a more complete removal and less bleeding than the older 'blind' curette method. It is often combined with tonsillectomy or ear grommets.

🖼️ Understand It in Pictures

💡 In simple words
The adenoids are soft lumps of tissue at the back of the nose. In some children they grow too big and block the nose or the ear tubes. Endoscopic adenoidectomy removes them through the mouth or nose using a thin camera, with no cut on the outside.

🩺 Conditions Treated

😴
Snoring & sleep apnoea
Loud snoring, pauses in breathing and restless sleep due to a blocked airway.
👃
Chronic nasal blockage
Constant mouth breathing and blocked nose not helped by sprays.
👂
Glue ear (otitis media with effusion)
Fluid behind the eardrum causing hearing loss; adenoidectomy reduces recurrence.
🔁
Recurrent ear infections
Repeated middle-ear infections, often done with grommets.
🤧
Chronic adenoiditis & sinusitis
Long-standing infected adenoids causing a runny nose and sinus symptoms in children.
😮
Adenoid facies & dental changes
Long face, open mouth and bite problems from years of mouth breathing.

⚠️ Symptoms That May Need Treatment

  • ➤Breathing through the mouth
  • ➤Loud snoring every night
  • ➤Pauses in breathing during sleep
  • ➤Nasal (blocked) voice
  • ➤Constant runny or blocked nose
  • ➤Repeated ear infections
  • ➤Hearing difficulty
  • ➤Daytime tiredness or poor attention
💡 Not every child with big adenoids needs surgery. Mild symptoms are often treated first with steroid nasal sprays and allergy medicines for 6–12 weeks, and adenoids naturally shrink with age. Surgery is advised when symptoms are severe, persistent or affect sleep, hearing or growth.

🔬 Tests Before Treatment

TestWhy it is done
Nasal endoscopyA thin camera shows the size of the adenoids and how much they block the airway.
X-ray lateral neck (soft tissue)A simple X-ray showing adenoid size, used especially in young children.
Ear examination and audiometry/tympanometryChecks for fluid behind the eardrum and hearing loss.
Sleep study (selected)Confirms and measures obstructive sleep apnoea in severe or unclear cases.
Blood tests (CBC, clotting)Checks for anaemia and bleeding problems before surgery.
Anaesthesia assessmentConfirms the child is fit and free of active infection.

🏥 The Procedure — Step by Step

Endoscopic Adenoidectomy procedure steps
1
Consultation
The ENT surgeon examines the nose, throat and ears and confirms adenoidectomy is needed.
2
Pre-op checks
Blood tests and anaesthesia check; fasting for about 6 hours before surgery.
3
Admission
Morning admission; a parent stays with the child until anaesthesia.
4
General anaesthesia
The child is fully asleep; a breathing tube protects the airway.
5
Endoscopic view
An HD endoscope is passed through the nose to see the adenoids clearly.
6
Adenoid removal
Adenoid tissue is removed with a microdebrider or coblation wand under direct vision.
7
Bleeding control
Any bleeding is stopped with suction cautery or coblation.
8
Added procedures
Grommets (ear tubes) or tonsillectomy are done at the same sitting if planned.
9
Recovery
The child wakes in recovery, then rests on the ward with pain relief and fluids.
10
Discharge
Discharge after an overnight stay once eating and drinking well.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Endoscopic microdebrider adenoidectomyRotating shaver removes tissue under camera viewComplete removal, including tissue near ear tubes
Endoscopic coblation adenoidectomyLow-temperature plasma removes and seals tissueLess bleeding, faster recovery
Conventional curettageAdenoids scraped with a curette without direct visionSimple cases; lower cost but less precise
AdenotonsillectomyAdenoids and tonsils removed togetherChildren with sleep apnoea and large tonsils
Adenoidectomy + grommetsAdenoids removed and ear tubes placedGlue ear and recurrent ear infections
📊 Studies comparing techniques show endoscope-guided adenoidectomy leaves less residual tissue than conventional curettage, and guidance from NICE and the AAO-HNS supports adenoidectomy with grommets to reduce repeat glue ear and ear infections in children over about 4 years.

🌱 Recovery After Treatment

🛏️
Day 1–2
Mild throat or ear pain, blocked nose and bad breath are normal; paracetamol helps.
🍦
Week 1
Soft, cool foods and plenty of fluids; normal diet as tolerated.
🏠
Day 3–7
Rest at home or hotel; avoid crowds to prevent infection.
🎒
Week 1–2
Most children return to school after about 1 week.
🌬️
Week 2–4
Snoring and mouth breathing improve as swelling settles.
👂
6–8 weeks
Hearing check if surgery was done for glue ear.

🏆 Success Rate

OutcomeResult
Improvement in nasal blockage and snoringAbout 85–90%
Cure of sleep apnoea with adenotonsillectomy (otherwise healthy children)About 70–80%
Reduction in repeat grommet surgery (with grommets)About 50%
Adenoid regrowth needing repeat surgeryAbout 1–5% (lower with endoscopic removal)

Results are best when allergies are also treated and when large tonsils are removed at the same time in children with sleep apnoea. Endoscopic removal lowers the chance of leftover tissue.

🩹 Risks & Possible Complications

❓ Frequently Asked Questions

➤ What is the cost of endoscopic adenoidectomy in India?
It starts from about USD 2,000 with a 1-day stay in a private room. Adding tonsillectomy or grommets increases the cost; we share an exact estimate after reviewing reports.
➤ At what age can adenoids be removed?
It can be done at any age when needed, most often between 2 and 10 years. Very young children are monitored a little more closely after surgery.
➤ How long does the surgery take?
About 30–45 minutes. There are no cuts on the face or neck; everything is done through the mouth and nose.
➤ How long should we stay in India?
About 5–7 days: consultation and tests, surgery with one night in hospital, and a follow-up check before flying home.
➤ Is it painful for my child?
Pain is usually mild – a sore throat or ear ache for a few days – and is well controlled with paracetamol. Most children are eating normally within a day or two.
➤ Why choose endoscopic over normal adenoidectomy?
The surgeon can see the adenoids directly and remove them completely, including tissue near the ear tubes. This reduces leftover tissue and regrowth.
➤ Can adults have adenoidectomy?
Yes. Adults with enlarged adenoids are usually checked for other causes first (including a biopsy if needed), then treated endoscopically.
➤ Will snoring stop after surgery?
Most children snore much less within 2–4 weeks. If large tonsils are also present, removing both gives the best result.
➤ Does removing adenoids weaken immunity?
No. Many other lymph tissues in the throat and body continue to fight infection. Studies show no long-term harm to immunity.
➤ When can my child return to school?
Usually after about 1 week.
➤ Can adenoids grow back?
Rarely – about 1–5% – and less often after complete endoscopic removal.
➤ Can both parents travel with the child?
Yes. Both parents can travel on medical attendant visas. We send the visa invitation letters.
➤ What follow-up is needed at home?
A check-up with your local doctor after 2–4 weeks, and a hearing test at 6–8 weeks if the surgery was for ear problems. Our team stays in touch.
