📖 What is a Total Thyroidectomy?
The thyroid is a butterfly-shaped gland in the front of the neck. It makes thyroid hormone, which controls the body's energy, heart rate, weight and temperature. The thyroid can grow very big (goitre), develop lumps (nodules), become overactive (Graves' disease), or develop cancer. Most thyroid cancers – papillary and follicular – grow slowly and are curable with good surgery.
A total thyroidectomy removes the whole thyroid gland. It can be done through a 4–6 cm cut in a natural neck crease (open), or without any neck scar using an endoscopic or robotic approach through the mouth (TOETVA), armpit or breast area. After surgery you take one thyroid hormone tablet (levothyroxine) every day for life. For cancer, nearby lymph nodes may also be removed (central neck dissection), and radioactive iodine may follow.

💡 Many thyroid nodules never need surgery. Most nodules are harmless and are only watched with ultrasound. Admission
You are admitted on the day of or the day before surgery. Blood tests and voice check are done.

General anaesthesia
You are fully asleep. A special breathing tube with nerve monitoring sensors is placed.

Incision
Open: 4–6 cm cut in a neck skin crease. Endoscopic/robotic: small cuts inside the lower lip, armpit or around the breast – no neck scar.

Finding the voice nerves
The recurrent laryngeal nerves are found and checked with an electronic nerve monitor throughout surgery.

Saving parathyroid glands
The four tiny parathyroid glands that control calcium are carefully kept with their blood supply.

Removing the thyroid
Both lobes and the middle part (isthmus) are removed completely.

Lymph nodes if needed

Closing
Bleeding is controlled, the wound is closed with dissolving stitches or glue.
