📖 What is a Laryngectomy?

The larynx or voice box sits in the front of the neck above the windpipe. It holds the vocal cords that make the voice, and it protects the airway when we swallow. Laryngeal cancer is closely linked to smoking and alcohol and often starts with a hoarse voice. Early cancers can often be cured with radiotherapy or small laser surgery, but advanced cancers or cancers that come back after radiotherapy usually need the voice box removed.

A laryngectomy is an operation to remove part (partial laryngectomy) or all (total laryngectomy) of the voice box. After a total laryngectomy, the windpipe is brought out to a permanent opening in the front of the neck called a stoma, and the patient breathes through it. Eating by mouth continues. Speech can be restored, most often with a small one-way valve called a voice prosthesis (tracheoesophageal puncture, TEP), or by oesophageal speech or an electrolarynx.

🩺 Conditions Treated

🎗️
Advanced laryngeal cancer
Stage III–IV cancers, especially when cartilage is destroyed (T4a).
🔁
Recurrent cancer after radiotherapy
Cancer that returns after radiation or chemoradiation (salvage laryngectomy).
🗣️
Hypopharyngeal cancer
Cancer of the lower throat around the voice box, often needing larynx removal.
⚪
Selected early cancers
Partial laryngectomy for early cancers to save the voice.
💥
Severe laryngeal trauma
Crushed voice box after injury, rarely.
🚫
Non-functioning larynx
Severe radiation damage or constant aspiration (food going into lungs) not treatable otherwise.

⚠️ Symptoms That Need Evaluation

  • ➤Hoarse voice for more than 3 weeks
  • ➤Pain or difficulty in swallowing
  • ➤Lump or swelling in the neck
  • ➤Noisy breathing or breathlessness
  • ➤Persistent cough or coughing blood
  • ➤Ear pain on one side
  • ➤Feeling of something stuck in the throat
  • ➤Unexplained weight loss
💡 Many laryngeal cancers can be treated without removing the voice box. Early (stage I–II) cancers are often cured by radiotherapy or transoral laser surgery with voice preserved. Some advanced cancers can be treated with chemoradiation (organ preservation). The tumour board recommends the best option for each patient.

🔬 Tests Before Treatment

TestWhy it is done
Laryngoscopy and biopsyConfirms cancer, its type and spread inside the voice box.
CT or MRI neck with contrastShows tumour size, cartilage invasion and neck lymph nodes.
PET-CTChecks spread to distant organs and confirms recurrence.
Chest CTLooks for spread to the lungs or a second cancer.
Blood tests, ECG and lung functionFitness for major surgery.
Thyroid function and nutrition checkThyroid may be removed partly; nutrition affects healing.
Speech and swallowing assessmentPlans voice restoration and feeding after surgery.

🏥 The Procedure — Step by Step

Laryngectomy procedure steps
1
Tumour board review
Head and neck surgeon, radiation and medical oncologists plan treatment.
2
Counselling
Speech therapist explains stoma care and voice options before surgery.
3
General anaesthesia
A tracheostomy may be done first if breathing is narrowed.
4
Neck incision
A U-shaped cut is made across the neck.
5
Neck dissection
Lymph nodes on one or both sides are removed where needed.
6
Removing the larynx
The voice box with the tumour is removed with safe margins.
7
Closing the throat
The throat (pharynx) is sewn closed; a flap is used if extra tissue is needed.
8
Creating the stoma
The windpipe is stitched to the skin to form a permanent breathing opening.
9
Voice puncture (TEP)
A small passage for a voice prosthesis is often made at the same time.
10
Recovery and discharge
Feeding tube, stoma care training and gradual return to swallowing.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Transoral laser surgeryTumour removed through the mouth with laserEarly glottic cancers; voice kept
Partial (vertical / supraglottic) laryngectomyPart of the voice box removedSelected early or intermediate cancers
Supracricoid partial laryngectomyMost of larynx removed, function keptSelected T2–T3 cancers in fit patients
Total laryngectomyWhole voice box removed, permanent stomaAdvanced (T4a) and recurrent cancers
Laryngopharyngectomy with flapLarynx and part of throat removed, rebuilt with flapHypopharyngeal cancer
📊 The VA Laryngeal Cancer Study (NEJM, 1991) showed that induction chemotherapy plus radiotherapy preserved the larynx in about 64% of advanced cases with similar survival to surgery, while later data show total laryngectomy gives better survival for T4a tumours with cartilage destruction (NCCN Head and Neck Guidelines).

🩹 Risks & Possible Complications

Laryngectomy is major surgery. Possible risks include:

  • •Leak from the throat to the neck skin (fistula), about 10–20%
  • •Wound infection or breakdown, higher after radiotherapy
  • •Narrowing of the stoma or throat
  • •Low thyroid or calcium levels needing tablets
  • •Problems with the voice prosthesis (leaking, needing change)
  • •Loss of normal sense of smell and change in taste
  • •Bleeding, chest infection or blood clots

💰 Cost of Laryngectomy in India

The cost of laryngectomy surgery in India starts from about USD 4,000 in an economy room, with 4–5 days in hospital as per the package. Neck dissection, flap reconstruction, a voice prosthesis, a longer hospital stay and radiotherapy can change the final cost.

❓ Frequently Asked Questions

➤ How much does laryngectomy cost in India?
The package starts from about USD 4,000 with 4–5 days in hospital. A total laryngectomy with neck dissection and a full 10–14 day stay usually costs about USD 6,000–9,000.
➤ How long is the hospital stay?
This package covers 4–5 days. Many total laryngectomy patients need 10–14 days until they can swallow safely; extra days are charged separately.
➤ How long should I stay in India?
Plan for about 3–4 weeks for surgery and recovery. If radiotherapy is advised, plan for about 9–10 weeks or arrange it at home.
➤ Will I be able to speak after total laryngectomy?
Yes, most patients learn to speak again. A voice prosthesis gives useful speech in about 80–90% of patients. Other options are oesophageal speech and an electrolarynx.
➤ Will I be able to eat normally?
Yes. After the throat heals, over 90% of patients eat by mouth. A feeding tube is used for about 1–2 weeks.
➤ Will I breathe through my neck for life?
After a total laryngectomy, yes – breathing is through the stoma permanently. After a partial laryngectomy, normal breathing usually returns.
➤ Can my voice box be saved?
In early cancers and some advanced cancers, radiotherapy, chemoradiation or laser surgery can save the voice box. The tumour board will advise if this is safe for you.
➤ What is the success rate?
5-year survival is about 60–70% for stage III and about 40–50% for stage IV laryngeal cancer, depending on spread and treatment.
➤ What are the main risks?
The most common problem is a leak from the throat (fistula) in about 10–20%, which usually heals with care. Others include infection, low thyroid levels and stoma narrowing.
➤ Will I need radiotherapy after surgery?
Often yes, for advanced cancers or if lymph nodes are involved. It usually starts 4–6 weeks after surgery.
➤ Can I go back to work?
Many patients return to light work in 2–3 months. Swimming is not possible because water can enter the stoma.
➤ What follow-up is needed?
Check-ups every 1–3 months for 2 years, scans as advised, and voice prosthesis changes every few months. We can arrange video follow-up.
➤ Can family members come?
Yes, one or two attendants are recommended. Satyug provides visa invitation letters.
➤ Is an interpreter available?
Yes. Our coordinators help in Arabic, French, Russian and other languages, and can help with communication boards after surgery.
