📖 What is a Laparoscopic Appendectomy?

The appendix is a small, finger-shaped pouch about 5–10 cm long attached to the start of the large bowel on the lower right side of the abdomen. It has no essential function in adults. When it gets blocked and infected, it becomes swollen and painful – this is appendicitis. If not treated, the appendix can burst (perforate) and spread infection through the abdomen (peritonitis), which is dangerous.

Appendectomy (also called appendicectomy) is the surgical removal of the appendix. In the laparoscopic (keyhole) method, the surgeon makes 3 small cuts of 5–10 mm, inserts a thin camera and fine instruments, ties off the base of the appendix and removes it through one of the small cuts. Compared with open surgery, it causes less pain, fewer wound infections, smaller scars and faster return to normal life.

🖼️ Understand It in Pictures

💡 In simple words
The appendix is a small pouch on the large bowel. In appendicitis it becomes swollen and infected and can burst. An appendectomy removes it, usually by keyhole surgery and sometimes through one larger cut.

🩺 Conditions Treated

🔥
Acute appendicitis
Sudden infection and swelling of the appendix – the most common reason.
🔁
Recurrent (grumbling) appendicitis
Repeated mild attacks of right lower abdominal pain.
🧱
Appendix mass or abscess (interval appendectomy)
Removal 6–8 weeks after an infection treated with antibiotics or drainage.
💥
Perforated appendix
Burst appendix with local infection, cleaned out at surgery.
🎗️
Appendix tumours (small)
Small carcinoid/neuroendocrine or mucinous tumours found on scans.
🪨
Appendicolith
Hard stone inside the appendix causing repeated pain.

⚠️ Symptoms of Appendicitis

  • ➤Pain starting around the navel, moving to the lower right abdomen
  • ➤Pain worse on walking, coughing or pressing
  • ➤Loss of appetite
  • ➤Nausea or vomiting
  • ➤Low-grade fever
  • ➤Bloating or constipation, sometimes diarrhoea
  • ➤Tenderness in the lower right abdomen
  • ➤Repeated episodes of right-sided pain
💡 Some cases can be treated first without surgery. Mild, uncomplicated appendicitis without a stone can sometimes settle with antibiotics, but about 25–40% return within 5 years. An appendix abscess is first drained and treated with antibiotics, with surgery planned later. International patients usually travel for planned (interval or recurrent) appendectomy; acute appendicitis is an emergency and must be treated locally first.

🔬 Tests Before Treatment

TestWhy it is done
Blood tests (CBC, CRP)Show infection and inflammation.
Ultrasound abdomenShows a swollen appendix; first test in children and pregnancy.
CT scan abdomenMost accurate test; shows abscess, perforation or tumour.
Urine testRules out kidney stones or urinary infection.
Pregnancy test (women)Rules out pregnancy-related causes of pain.
ECG and chest X-ray (if needed)Fitness for general anaesthesia.

🏥 The Procedure — Step by Step

Laparoscopic Appendectomy procedure steps
1
Consultation
The surgeon examines you and reviews scans and blood tests.
2
Pre-operative tests
Blood tests and anaesthesia check; fasting for 6 hours.
3
Admission
You are admitted to a private room and given antibiotics through a drip.
4
General anaesthesia
You are fully asleep during surgery.

Port placement
Three small cuts are made, including one at the navel; the abdomen is gently inflated with gas.
6
Freeing the appendix
The appendix and its blood vessel are separated and sealed.
7
Removal
The base is closed with loops or a stapler, and the appendix is removed in a bag.
8
Wash-out and closure
Any pus is cleaned out; the small cuts are closed with dissolvable stitches or glue.
9
Recovery
You eat and walk the same evening or next morning.
10
Discharge

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Laparoscopic appendectomy (3-port)Keyhole removal through 3 small cutsMost adults and children
Single-incision laparoscopic (SILS)All instruments through one cut at the navelSelected patients wanting minimal scar
Open appendectomyOne 5–8 cm cut in the lower right abdomenWhen keyhole is not possible or very complicated cases
Interval appendectomyPlanned removal 6–8 weeks after an abscessAfter appendix mass or abscess
Antibiotics onlyNo surgery; antibiotics for uncomplicated casesSelected mild cases; higher chance of return
📊 A Cochrane review (2018) found laparoscopic appendectomy gives less wound infection, less pain and faster return to normal activity than open surgery. In the CODA trial (NEJM 2020), about 29% of patients treated with antibiotics alone needed appendectomy within 90 days.

🏆 Success Rate

OutcomeResult
Cure of appendix symptomsAbout 98–99%
Conversion to open surgeryAbout 2–10% (higher if perforated)
Wound infection (laparoscopic)About 1–3%
Return to normal activityAbout 1–2 weeks

Results are best when surgery is done before the appendix bursts or in a planned setting. Perforation, abscess, obesity, diabetes and older age increase recovery time and complication risk.

🩹 Risks & Possible Complications

  • •Shoulder-tip pain and bloating from gas – common, 1–2 days
  • •Wound infection – about 1–3%
  • •Abscess inside the abdomen – about 2–5% (more after perforation)
  • •Conversion to open surgery – about 2–10%
  • •Bleeding – under 1%
  • •Injury to bowel or bladder – rare (under 0.5%)
  • •Hernia at a port site – about 1%

❓ Frequently Asked Questions

➤ How much does laparoscopic appendectomy cost in India?
It starts from about USD 899 with a 1–2 day private-room stay. Complicated or perforated cases cost about USD 1,500–2,500.
➤ How long will I stay in hospital and in India?
The hospital stay is 1–2 days. Plan about 5–7 days in India so the surgeon can check the wounds and share the biopsy report.
➤ Can I travel to India with acute appendicitis?
No. Acute appendicitis is an emergency and should be treated at the nearest hospital. Patients travel to India for planned surgery, such as recurrent appendicitis or interval appendectomy after an abscess.
➤ Is the surgery painful?
Pain is mild and controlled with tablets. Many people feel shoulder-tip pain for a day due to the gas used during surgery.
➤ How big are the scars?
Three small cuts of 5–10 mm each, one at the navel. They heal into tiny, barely visible marks.
➤ When can I eat and walk?
Most patients walk and drink a few hours after surgery and eat light food the same evening or the next morning.
➤ When can I return to work?
Office work after about 1–2 weeks. Heavy lifting and sports after 4–6 weeks.
➤ What is the success rate?
Removing the appendix cures appendicitis in about 98–99% of patients. It cannot come back because the appendix is gone.
➤ What are the risks?
Risks are low: wound infection (1–3%), abscess (2–5%), bleeding (under 1%) and rare bowel injury. Complicated appendicitis carries higher risk.
➤ Can appendicitis be treated with antibiotics only?
Some mild cases can, but about 25–40% come back within a few years. Surgery is the permanent solution.
➤ Can I live normally without an appendix?
Yes. The appendix is not needed for digestion or immunity in adults, and life is completely normal without it.
➤ Is laparoscopic appendectomy safe for children and the elderly?
Yes. Paediatric surgeons operate on children, and elderly patients have a heart and lung check first. Keyhole surgery is preferred in both groups.
➤ Can a family member come with me?
Yes. We provide visa invitation letters for the patient and an attendant, plus airport pick-up and a coordinator.
➤ What follow-up is needed at home?
Usually none beyond a wound check. If the biopsy shows anything unusual, our team arranges a video consultation with your surgeon.
