Know More About Procedure & Surgery
Thoracoscopy with Biopsy in India
Keyhole examination of the chest lining and lungs to find the cause of pleural fluid, lung spots or chest cancers, done by experienced pulmonologists and thoracic surgeons at accredited Indian hospitals – at a fraction of Western costs.
📖 What is Thoracoscopy with Biopsy?
The lungs are covered by a thin double layer called the pleura. Sometimes fluid collects between these layers (pleural effusion), or the pleura becomes thick or shows lumps. Common causes are tuberculosis (TB), lung cancer, spread of other cancers and mesothelioma. Fluid tests alone find the cause in only about half to two-thirds of cases, so a tissue sample (biopsy) is often needed.
Thoracoscopy, also called pleuroscopy or, when done by a surgeon under general anaesthesia, VATS (video-assisted thoracoscopic surgery), puts a thin camera into the chest through one to three small cuts of about 1–2 cm. The doctor looks at the pleura and lung surface, drains the fluid and takes several biopsies of abnormal areas. If needed, talc can be sprayed in the same sitting (pleurodesis) to stop the fluid from coming back.

🔬 Tests Before Treatment
| Test | Why it is done |
|---|---|
| Chest X-ray and CT chest with contrast | Shows the fluid, pleural thickening, lumps and the best place to enter. |
| Chest ultrasound | Marks the safe entry point and checks for loculated (pocketed) fluid. |
| Pleural fluid tests | Protein, LDH, ADA (for TB), cell count and cancer cells guide the plan. |
| Blood tests and clotting profile | Checks blood count, kidney function and bleeding risk. |
| ECG and echo (if needed) | Checks the heart before sedation or anaesthesia. |
| PET-CT (selected cases) | Helps stage cancer and find the most active areas for biopsy. |
🏥 The Procedure — Step by Step

⚖️ Types & Techniques
| Technique | What is done | Best for |
|---|---|---|
| Medical thoracoscopy (pleuroscopy) | Single port, local anaesthesia and sedation, by a pulmonologist | Unexplained pleural effusion, TB, talc pleurodesis |
| VATS pleural biopsy | Surgeon, general anaesthesia, 1–3 ports | Thick pleura, mesothelioma, pocketed fluid |
| VATS lung wedge biopsy | A small wedge of lung is removed | Lung nodules, interstitial lung disease |
| Thoracoscopy with talc poudrage | Talc sprayed after biopsy | Recurring fluid, especially in cancer |

🌱 Recovery After Treatment
🏆 Success Rate
| Outcome | Result |
|---|---|
| Diagnostic yield (finding the cause) | About 90–95% |
| Success of talc pleurodesis in stopping fluid | About 75–90% |
| Serious complication rate | About 1–2% |
| Procedure-related death | Very rare – about 0.1% or less |
Results depend on the cause of the fluid, how thick the pleura is, the lung's ability to expand fully and the experience of the team. A clear biopsy result means the right treatment can start quickly.
🩹 Risks & Possible Complications
Like any medical procedure, this treatment carries some risks. Your specialist will explain them in detail before treatment:
- •Chest pain at the drain site – common, usually mild and short-lived
- •Fever after talc pleurodesis – about 10–20%, usually settles in 1–2 days
- •Air leak lasting more than a few days – about 2–5%
- •Wound or chest infection (empyema) – about 1–3%
- •Bleeding – uncommon, usually minor
- •Irregular heartbeat or low oxygen during the procedure – rare
- •Tumour growth along the wound in mesothelioma – uncommon

Frequently Asked Questions
You will have stitches or staples in the incisions. Your doctor will take these out 1 to 2 weeks after your surgery. The amount of time you will need to recover depends on the surgery you had. But you probably will need to take it easy at home for at least 1 to 2 weeks.
Chest surgery (also known as thoracic surgery) involves the organs of the chest, but extends to the esophagus (tube between mouth and stomach), the trachea (airway) and the chest wall (rib cage and breastbone).
Aerobic activities like walking, running or jumping rope give your heart and lungs the kind of workout they need to function efficiently. Muscle-strengthening activities like weight-lifting or Pilates build core strength, improving your posture, and toning your breathing muscles.
Take a small, silent breath in and a small, silent breath out. Hold your nose with your fingers to prevent air from entering your lungs. Count how many seconds until you feel the first signs of air hunger. At the first sign of air hunger, you will also feel the first involuntary movements of your breathing muscles.
If you notice that you are expelling more mucus when you cough, or that it has changed, these could be symptoms of a lung infection. When a person has a lung infection, their mucus tends to change color, have a thicker and stickier consistency, and sometimes will have a foul odor.
Common signs are:
- Trouble breathing.
- Shortness of breath.
- Feeling like you're not getting enough air.
- Decreased ability to exercise.
- A cough that won't go away.
- Coughing up blood or mucus.
- Pain or discomfort when breathing in or out.
Thoracoscopy can be used to look at an abnormal area seen on an imaging test (such as a chest x-ray or CT scan). It also can be used to take biopsy samples of lymph nodes, abnormal lung tissue, the chest wall, or the lining of the lung (pleura). It is commonly used for people with mesothelioma and lung cancer.
Neuropathic pain after thoracic surgery, known as post-thoracotomy pain, is common and sometimes severe. The pain often becomes chronic and is associated with symptoms including burning, shooting, shocking, pressure-like, and aching sensations.
