Know More About Procedure & Surgery
Parathyroidectomy in India
Parathyroidectomy is the surgical removal of one or more overactive parathyroid glands, the four tiny glands in the neck that control the body's calcium level. When one gland becomes enlarged, usually due to a benign growth called an adenoma, it produces too much parathyroid hormone (PTH), leading to high calcium, kidney stones, bone thinning, fatigue and low mood. Surgery is the only permanent cure for primary hyperparathyroidism and has a very high success rate in experienced hands. Indian endocrine surgeons offer modern localisation scans and minimally invasive techniques at a much lower cost than in Western countries.
Cost of parathyroidectomy in India starts from USD 2500 at leading JCI / NABH accredited hospitals. 1-3 days in hospital; total stay in India 7-10 days.
| Package | Cost (USD) | Room | Hospital stay |
|---|---|---|---|
| Minimally Invasive (Focused) Parathyroidectomy | 2500 USD | Twin Sharing | 1-2 days hospital |
| Bilateral Neck Exploration Parathyroidectomy | 3200 USD | Twin Sharing | 2-3 days hospital |
| Subtotal / Total Parathyroidectomy with Autotransplant (secondary or tertiary hyperparathyroidism) | 4200 USD | Private Room | 3-4 days hospital |
Final cost depends on the hospital, room category, doctor, implants/medicines used and the patient's medical condition. Share your reports with Satyug Healthcare for a free written estimate.
About the Procedure
- Most cases of primary hyperparathyroidism (about 85%) are caused by a single benign adenoma, while the remainder involve enlargement of several glands (hyperplasia) or, rarely, parathyroid cancer.
- Minimally invasive (focused) parathyroidectomy removes only the abnormal gland through a small 2-3 cm incision when scans clearly show its location.
- Bilateral neck exploration allows the surgeon to examine all four glands and is used when scans are unclear or several glands are suspected to be diseased.
- Patients on dialysis or after kidney transplantation may develop secondary or tertiary hyperparathyroidism, which is treated by removing three and a half glands (subtotal) or all glands with a small piece re-implanted in the forearm or neck muscle.
- Intra-operative PTH monitoring measures the hormone level during surgery; a significant fall confirms that the overactive tissue has been removed.
- Leading Delhi/NCR hospitals have dedicated endocrine surgery units with access to Sestamibi scans, 4D-CT and PET-CT (Fluorocholine) for precise localisation.
Purpose of the Procedure
- To cure primary hyperparathyroidism by removing the source of excess parathyroid hormone.
- To bring blood calcium back to normal and relieve symptoms such as fatigue, bone pain, thirst, constipation, poor concentration and low mood.
- To prevent or reduce kidney stones and protect kidney function.
- To stop bone loss and reduce the risk of osteoporosis and fractures; bone density often improves after surgery.
- To control severe secondary hyperparathyroidism in kidney patients when medicines are no longer effective.
- To remove suspected parathyroid cancer completely when it is present.
Diagnosis and Tests Before the Procedure
- Blood tests for serum calcium (corrected or ionised), parathyroid hormone (PTH), phosphate, vitamin D and kidney function.
- 24-hour urine calcium to rule out familial hypocalciuric hypercalcaemia, a condition that does not need surgery.
- Neck ultrasound by an experienced radiologist to locate the enlarged gland and check the thyroid.
- Tc-99m Sestamibi scan with SPECT-CT to identify the overactive gland.
- 4D-CT neck or Fluorocholine PET-CT in cases where the gland cannot be found on standard scans or after previous surgery.
- Bone mineral density (DEXA) scan to assess osteoporosis, and kidney ultrasound to look for stones.
- Vocal cord assessment (laryngoscopy) before surgery, especially in patients who have had previous neck surgery.
- Fitness tests for anaesthesia including ECG, chest X-ray and routine blood tests.
Step-by-Step Procedure
- The patient is admitted on the day of surgery or the evening before, after review of all scan results by the endocrine surgeon.
- General anaesthesia is given; in selected cases a focused operation may be done under regional block.
- A small horizontal incision is made in a natural skin crease of the lower neck.
- The surgeon carefully identifies and protects the recurrent laryngeal nerve, which controls the voice box.
- The abnormal parathyroid gland is located and removed; in bilateral exploration all four glands are examined.
- Blood is taken for intra-operative PTH; a fall of more than 50% from the starting level confirms successful removal.
- The removed gland is sent for frozen section or histopathology examination.
- The wound is closed with dissolvable sutures or skin glue, leaving a small, neat scar.
- Calcium levels are checked after surgery, calcium and vitamin D supplements are started if needed, and the patient is discharged after 1-3 days.
Risks and Possible Complications
- Low calcium (hypocalcaemia) after surgery, causing tingling in the fingers or around the mouth; usually temporary and treated with calcium tablets.
- Hungry bone syndrome in patients with severe bone disease, where calcium drops sharply and needs intensive replacement.
- Temporary or, rarely, permanent hoarseness from injury to the recurrent laryngeal nerve.
- Bleeding or haematoma in the neck, which can rarely affect breathing and needs urgent attention.
- Persistent or recurrent hyperparathyroidism if additional abnormal glands are present.
- Wound infection or a thickened scar.
- Permanent hypoparathyroidism, mainly after subtotal or total parathyroidectomy.
Your specialist will explain the risks specific to your condition. Choosing an experienced, high-volume hospital keeps complication rates low.
Recovery and Aftercare
- Most patients are walking and eating the same day and go home within 1-3 days.
- Neck discomfort and mild difficulty swallowing settle within a few days.
- Calcium and vitamin D supplements may be needed for a few weeks; blood calcium and PTH are checked before departure.
- Many patients notice improvements in energy, mood and concentration within weeks.
- Light activities can be resumed after a few days and normal activities, including office work, after 1-2 weeks.
- Bone density usually improves over the following 1-2 years, and calcium should be checked periodically in the home country.
- Patients are generally fit to fly 5-7 days after surgery once calcium levels are stable.
