📖 What is Robotic Total Thyroidectomy?

The thyroid is a butterfly-shaped gland at the front of the neck. It makes hormones that control energy and metabolism. It can become enlarged (goitre), develop lumps (nodules), become over-active (Graves' disease) or develop cancer. When the whole gland needs to be removed, the operation is called a total thyroidectomy. After surgery you take a daily thyroid hormone tablet (levothyroxine) for life.

Traditional surgery uses a 5–8 cm cut across the front of the neck. In robotic thyroidectomy, the surgeon controls robotic arms with a magnified 3D view through small cuts placed away from the neck – in the armpit and/or around the breast (BABA or trans-axillary approach), behind the ear (retro-auricular) or inside the lower lip (TORT/TOETVA). This leaves no visible neck scar. The robot gives fine, steady movements to protect the voice nerves and the tiny parathyroid glands that control calcium.

🖼️ Understand It in Pictures

💡 In simple words
The thyroid is a small gland in the neck that controls how fast the body works. A lump, cancer or large goitre may mean the whole gland must be removed. In robotic surgery the gland is taken out through small cuts away from the front of the neck, such as the armpit or behind the ear.

🩺 Conditions Treated

🎗️
Papillary / follicular thyroid cancer
Selected cancers usually up to about 4 cm without major spread.
🦋
Multinodular goitre
Enlarged gland causing pressure, cosmetic concern or growth.
🔥
Graves' disease
Over-active thyroid not controlled by medicines or radioiodine.
❓
Suspicious thyroid nodules
Bethesda III–V nodules on both sides needing full removal.
⚡
Toxic multinodular goitre
Nodules making excess hormone.
🧬
Hereditary thyroid cancer risk
Selected patients, such as RET gene carriers.

⚠️ Symptoms That May Need Thyroid Surgery

  • ➤Swelling or lump in the front of the neck
  • ➤Difficulty swallowing
  • ➤Pressure or choking feeling
  • ➤Hoarse voice
  • ➤Breathlessness when lying flat
  • ➤Fast heartbeat, weight loss, tremor (over-active)
  • ➤Bulging eyes (Graves')
  • ➤Suspicious nodule on ultrasound or biopsy
💡 Many thyroid nodules are harmless and only need monitoring. Over-active thyroid can often be controlled with medicines or radioiodine. Robotic surgery is not suitable for very large goitres, cancers invading nearby tissues, or extensive lymph node spread – open surgery is safer in these cases.

🔬 Tests Before Treatment

TestWhy it is done
Thyroid function tests (TSH, T3, T4)Shows if the gland is under- or over-active
Ultrasound neckSize of the gland, nodules and lymph nodes
FNAC (needle biopsy)Checks if a nodule is cancer (Bethesda category)
Vocal cord check (laryngoscopy)Records voice box function before surgery
Calcium, PTH and vitamin DBaseline for parathyroid function
CT neck (if needed)For large goitre or cancer staging
Blood tests, ECG, chest X-rayFitness for general anaesthesia

🏥 The Procedure — Step by Step

Robotic Total Thyroidectomy procedure steps
1
Admission and assessment
Admission on the day of or before surgery; final checks and consent.
2
General anaesthesia
You are fully asleep; a special breathing tube monitors the voice nerves.
3
Hidden incisions
Small cuts are made in the armpit/breast area, behind the ear or inside the lip.
4
Creating the working space
A tunnel is made under the skin to reach the thyroid.
5
Robot docking
The da Vinci robotic arms and 3D camera are attached.
6
Thyroid removal
Both lobes are removed while identifying the voice nerves and parathyroid glands.
7
Lymph node clearance (if needed)
Central neck nodes removed for cancer when indicated.
8
Closure
A small drain may be placed; cuts are closed with dissolving stitches.
9
Recovery
Calcium levels checked, voice assessed and drain removed.
10
Discharge
Discharge after 2–3 days on thyroid hormone and calcium tablets if needed.

⚖️ Robotic and Other Approaches

ApproachWhat is doneBest for
Trans-axillary (armpit)Single cut in the armpitMainly one-sided disease; total possible in selected cases
BABA (bilateral axillo-breast)Small cuts in both armpits and breast areasTotal thyroidectomy with good view of both sides
Retro-auricular (facelift)Cut behind the ear and along hairlineSelected patients, also neck node surgery
Transoral (TORT / TOETVA)Cuts inside the lower lip, no skin scarSmaller glands and small cancers
Open thyroidectomy5–8 cm cut at the front of the neckLarge goitres, advanced cancer
📊 Meta-analyses of robotic versus open thyroidectomy (including large Korean series of over 5,000 patients) show similar rates of permanent nerve injury (about 1%) and permanent low calcium (about 1–2%), with better cosmetic satisfaction but longer operating time.

🏆 Success Rate

OutcomeResult
Temporary voice changeAbout 3–8%
Permanent voice nerve injuryAbout 1%
Temporary low calciumAbout 15–30%
Permanent low calcium (hypoparathyroidism)About 1–2%

Results depend on the surgeon's robotic experience, the size of the gland, the type and stage of cancer and careful follow-up of thyroid hormone and calcium levels.

🩹 Risks & Possible Complications

Robotic thyroidectomy is safe in experienced hands, with risks similar to open surgery.

  • •Temporary low calcium causing tingling – about 15–30%, treated with tablets

  • •Permanent voice nerve injury – about 1%
  • •Permanent low calcium – about 1–2%
  • •Conversion to open surgery – uncommon (under 2–3%)
