What is Pyeloplasty?

Pyeloplasty is surgery used to correct a condition called ureteropelvic junction (UPJ) obstruction. UPJ obstruction blocks the flow of urine out of the kidney into the ureter. The ureter is a tube attaching the kidney to the bladder. The blockage is at the point where the kidney (renal pelvis) attaches to the ureter (ureteropelvic junction).

Causes 

UPJ obstruction mostly happens in children. When the condition is present from birth, it is called a congenital condition. The blockage happens when the area between the ureter and part of the kidney becomes too narrow. This condition happens as the kidney is forming. It can result in a damaged kidney from urine buildup and increased pressure in the kidney. Usually, only one kidney is affected.

Other causes of blockage for adults and children include:

  • Abnormal blood vessel over the ureter
  • Scar tissue
  • Infection
  • Earlier treatments for a blockage
  • Kidney stones

Diagnosis

UPJ obstruction is typically found with an ultrasound before the baby is born. Sometimes the blockage will not show until after birth.

After birth, tests can be performed including:

  • CT scan
  • CT urogram (scan of both kidneys with contrast)
  • Ultrasound
  • Blood test
  • Urine and blood test
  • Electrolytes
  • Nuclear scan of kidneys
  • X-ray of kidneys, bladder, ureters
  • Voiding cystourethrogram (X-ray of bladder while it is emptying)

Symptoms

Symptoms of UPJ obstruction include:

  • Abdominal mass
  • Urinary tract infection
  • Back or flank pain
  • Bloody urine
  • Kidney infection
  • Poor growth in infants
  • Vomiting

Treatment

UPJ obstruction can be treated with surgery. If the condition is severe, this may need to be done early in life. Mostly, surgery can wait until later in life. Surgery is not required for all cases. After a successful surgery, the urine will flow normally. For children, open surgery is often done. For adults, less invasive surgery may be done. The procedure is known as pyeloplasty. The process takes a few hours.

During pyeloplasty, a 2-3 inch cut is made just below the ribs. The obstructed segment of ureter is removed. The normal caliber ureter is then reattached to the renal pelvis. A stent (a tiny wire-meshed tube) is placed to drain urine from the kidney. After the surgery heals, the stent will be removed. The more common approach currently in adults is robotic-assisted or laparoscopic pyeloplasty. The same type of repair is done, however, through multiple small (8 - 10mm) incisions. A ureteral stent is then placed.

After the Surgery/Recovery

  • Patients will stay in the hospital for a day or two after surgery. Recovery is usually quick.
  • The ureter may be swollen. The patient may have some pain for a few days after surgery.
  • Kidney drainage may be poor at first. This often gets better over time as the area heals.
  • A few weeks after surgery, the stent is removed, and the doctor may use an ultrasound to check for swollen kidneys.

Risks Involved in the Procedure

Although this procedure has proven to be very safe, as in any surgical procedure there are risks and potential complications.

  • Bleeding: Blood loss during this procedure is typically minor (less than 100 cc) and a blood transfusion is rarely required. If you are still interested in autologous blood transfusion (donating your own blood) prior to your surgery, you must make your surgeon aware.
  • Infection: All patients are treated with broad-spectrum intravenous antibiotics prior to starting the surgery to decrease the chance of infection from occurring after surgery.   • Hernia: Hernias at incision sites rarely occur since all keyhole incisions are closed carefully at the completion of your surgery.
  • Tissue / organ injury: Although uncommon, possible injury to surrounding tissue and organs including bowel, vascular structures, spleen, liver, pancreas and gallbladder could require further surgery. Injury could occur to nerves or muscles related to positioning.
  • Conversion to open surgery: this surgical procedure may require conversion to the standard open operation if extreme difficulty is encountered during the laparoscopic procedure. This could result in a larger standard open incision and possibly a longer recuperation period.
  • Failure to correct UPJ obstruction: Roughly 3% of patients undergoing this operation will have persistent blockage due to recurrent scarring. If this occurs additional surgery may be necessary.
