A double-J stent is a ureteral stent with curving ends that prevent the stent slipping into the bladder or the kidney. Stents are used for various reasons in patients with kidney stones. They can be placed to help reduce sharp pain from a stone (renal colic) or to allow drainage when infection is present or when a stone prevents a kidney from working adequately. Stents are commonly placed after surgery for stones (for example, ureteroscopy) to allow healing and to ensure that swelling does not block the drainage of urine after the procedure.
The widespread use of ureteral stents has corresponded to the increase in possible complications, including stent migration, encrustation, stone formation, and fragmentation. Complications associated with the use of ureteral stents are primarily mechanical. Stent occlusion may be frequent and requires simple catheter exchange. Regardless of the initial indication for stent placement, transurethral cystoscopic exchange is usually a simple and effective therapy for occlusion.
Instructions may include:
Before surgery, someone from your health care team—usually the anesthesiologist—will assess which type of anesthesia is appropriate for you.
Depending on the country you live in and your hospital, types of anesthesia can include:
You may be asked to give a urine sample before the procedure to test for a urinary tract infection.
The procedure is usually performed with the patient asleep (under general anesthesia). Sometimes a local anesthetic, with or without sedation, is administered.
During this procedure, a tube with a tiny optic camera is inserted through the urethra into your bladder. The bladder is inspected, and the ureteric opening is located. The urologist may use x-ray images taken with a contrast agent (dye) in the ureters to assess the urinary tract and to locate the obstruction. The stent is placed during surgery by sliding it over a guide wire placed in the ureter (the tube that drains urine from the kidney into your bladder).
Frequently asked questions
What is DJ stenting procedure?
A thin, hollow tube placed inside the ureter during surgery to ensure drainage of urine from the kidney into the bladder. J shaped curls are present at both ends to hold the tube in place and prevent migration, hence the description "double J stent".
Why DJ stenting is done?
Ureteral double J (DJ) stents are frequently used to relieve ureteral obstruction and almost as a routine part of the ureteroscopic procedures by many surgeons. DJ stent placement has the potential side effects such as flank pain and urinary tract infection (UTI) due to retrograde urine flow.
Does stent removal require anesthesia?
Most patients tolerate having the stent removed using only a topical anesthetic placed in the urethra. However, sterile lubrication containing local anesthetic (lidocaine) is instilled into the urethra.
How long can you live with stents?
Stents were first used in the early 1980s, and some people with those original stents are still doing just fine nearly 30 years later. Stents can develop blockages too. In recent years, drug-eluting stents have been used in some patients.
Does stent damage kidney?
Occasionally a stent can become displaced and slip out of the water pipe (urethra). If this happens you should contact the hospital. Having a stent, along with your original kidney problem, can make it more likely that you will develop a urinary tract infection.
What happens if stent is not removed?
Forgotten ureteral stents can cause a spectrum of complications ranging from hematuria, stent occlusion, migration, fragmentation, encrustation, and stone formation to serious complications like recurrent urinary tract infection (UTI), urinary tract obstruction, and renal failure.
What can I expect after a ureteral stent?
You may have a small amount of blood in your urine for 1 to 3 days after the procedure. While the stent is in place, you may have to urinate more often, feel a sudden need to urinate, or feel like you cannot completely empty your bladder. You may feel some pain when you urinate or do strenuous activity.
How long do you stay in the hospital after having a stent put in?
Recovery from angioplasty and stenting is typically brief. Discharge from the hospital is usually 12 to 24 hours after the catheter is removed. Many patients are able to return to work within a few days to a week after a procedure.
