📖 What is PCNL Surgery?
Kidney stones are hard lumps formed when minerals such as calcium, oxalate or uric acid crystallise in the urine. Small stones often pass by themselves. Large stones – usually bigger than 2 cm – or branching staghorn stones that fill the kidney cannot pass, and can cause pain, repeated infection, bleeding and slow damage to the kidney.
Percutaneous nephrolithotomy (PCNL) is keyhole surgery. The surgeon makes a 1 cm cut in the back, creates a narrow tunnel straight into the kidney under X-ray or ultrasound guidance, breaks the stone with a laser or ultrasonic probe, and removes the pieces. It is the recommended first treatment for kidney stones above 2 cm by the European and American urology associations. Mini-PCNL uses an even thinner tunnel.

💡 Small stones usually do not need PCNL. Stones under 5–6 mm often pass with water and medicine, and stones under 2 cm are usually treated with shock wave lithotripsy (ESWL) or flexible ureteroscopy (RIRS). Your urologist chooses the method based on stone size, position and hardness.

Admission and pre-op checks
You are admitted on the day of surgery after tests and anaesthesia review; urine must be infection-free.

Anaesthesia
General or spinal anaesthesia is given, along with a dose of antibiotics.

Ureteric catheter
A thin tube is placed into the ureter through the urethra to outline the kidney with dye.

Kidney puncture
Lying on the tummy (or back), a needle is guided into the kidney under X-ray or ultrasound.

Tract dilatation
The puncture is widened to a 1 cm (or smaller, for mini-PCNL) working tunnel.

Stone fragmentation
A nephroscope is passed in and the stone is broken with laser, ultrasonic or pneumatic energy.

Stone removal
Fragments are flushed or grasped out; an X-ray checks that the kidney is clear.

Drainage
A DJ stent and/or a small nephrostomy tube may be left for a day or two (tubeless PCNL avoids the tube).

Recovery on the ward
You walk the same or next day; the tube and catheter are removed and a check X-ray or ultrasound is done.

Discharge on day 3 with painkillers, plenty of fluids and a plan for stent removal.

Results depend on stone size, hardness and spread, kidney anatomy and the surgeon's experience. Very large staghorn stones sometimes need a second sitting to clear completely.
