📖 What is Balloon Atrial Septostomy?

The heart has two upper chambers (atria) separated by a wall called the atrial septum. Before birth, a small flap opening in this wall (the foramen ovale) lets blood pass between them. In some newborns with serious heart defects – most commonly transposition of the great arteries (TGA), where the two main arteries are swapped – oxygen-rich and oxygen-poor blood run in two separate circuits. The baby becomes very blue (cyanosis) and the life of the baby depends on blood mixing through small openings in the heart.

Balloon atrial septostomy, also called the Rashkind procedure, makes this opening bigger without open surgery. A paediatric cardiologist passes a thin tube (catheter) with a small balloon at its tip through a vein in the groin or the umbilical (belly-button) vein into the heart. The balloon is passed through the opening, inflated, and pulled back sharply to tear the thin wall and widen the hole. It is often done at the bedside in the NICU under ultrasound (echo) guidance. Oxygen levels usually improve within minutes. It is a bridge – most babies then need corrective heart surgery such as the arterial switch operation.

🖼️ Understand It in Pictures

💡 In simple words
Some newborns have a heart defect, such as transposition of the great arteries, where oxygen-rich and oxygen-poor blood cannot mix enough, so the baby turns blue. A balloon atrial septostomy makes the small natural opening between the top chambers bigger, so blood mixes and oxygen rises until surgery.

🩺 Conditions Treated

🔄
Transposition of the great arteries (TGA)
The most common reason – improves mixing of blood before arterial switch surgery.
🫀
Hypoplastic left heart syndrome
When the atrial opening is too small (restrictive) for blood to leave the left atrium.
⛔
Tricuspid atresia
Blood must cross the atrial septum; a small opening needs enlarging.
🚫
Mitral atresia
Left heart inflow blocked; septostomy decompresses the left atrium.
🫁
Total anomalous pulmonary venous return
Selected cases with a restrictive atrial opening, as a bridge to surgery.
⚙️
Pulmonary atresia with intact septum
Selected cases where right-to-left flow is restricted.

⚠️ Signs That May Need a Septostomy

  • ➤Blue lips, skin or tongue soon after birth
  • ➤Low oxygen levels despite oxygen
  • ➤Fast or difficult breathing
  • ➤Poor feeding and tiredness
  • ➤Diagnosis of TGA on echo
  • ➤Small (restrictive) atrial opening on echo
  • ➤Acidosis on blood tests
  • ➤Baby needing prostaglandin infusion
💡 Not every baby with TGA needs a septostomy. If the opening between the atria is already large and oxygen levels are good on prostaglandin (PGE1), the baby may go directly to arterial switch surgery. The decision is made by the paediatric cardiologist after an echo.

🔬 Tests Before Treatment

TestWhy it is done
Echocardiogram (heart ultrasound)Confirms the diagnosis and size of the atrial opening
Pulse oximetry & arterial blood gasMeasures oxygen and acid levels in the blood
Chest X-rayChecks heart size and lungs
ECGChecks heart rhythm
Blood tests (CBC, sugar, electrolytes, clotting)Baby's general condition before the procedure
Cranial ultrasound (if needed)Checks the brain in very sick or premature babies

🏥 The Procedure — Step by Step

Balloon Atrial Septostomy procedure steps
1
Urgent opinion
Our paediatric cardiologists review the baby's echo and oxygen levels the same day.
2
Travel planning
Air ambulance or medically escorted flight arranged if the baby is unstable.
3
NICU admission
Baby admitted directly to the cardiac NICU; prostaglandin infusion continued if needed.
4
Echo confirmation
Repeat echo confirms the defect and size of the atrial opening.
5
Sedation
Baby is sedated and kept comfortable; ventilation support if needed.
6
Catheter access
A thin balloon catheter is passed through the umbilical or groin vein.
7
Balloon septostomy
Balloon inflated in the left atrium and pulled back to widen the opening (2–3 times).
8
Check result
Echo shows a wider opening and oxygen levels rise.
9
NICU monitoring
3–7 days of NICU care; feeds started and prostaglandin often stopped.
10
Next step plan
Arterial switch or other corrective surgery planned, often during the same trip.

⚖️ Types & Techniques

TechniqueWhat is doneBest for

📊 Large registry data show balloon atrial septostomy is successful in about 95%+ of newborns with TGA, with serious complications in about 2–5%, which is why it remains a standard emergency treatment recommended by paediatric cardiology guidelines.

Rise in oxygen saturationUsually within minutes
Serious complicationsAbout 2–5%
Procedure-related deathUnder 1%
Arterial switch survival after BAS (TGA)About 95%+ at experienced centres

Results depend on the baby's condition on arrival, birth weight, prematurity, other defects and how quickly treatment is given. Early referral and safe transport are very important.

🩹 Risks & Possible Complications

This is an emergency procedure for very sick newborns; benefits usually far outweigh risks.

  • •Irregular heartbeat during the procedure – usually brief
  • •Bleeding or bruising at the vein site
  • •Blood clot in the leg vein
  • •Injury to heart valves or veins – rare
  • •Balloon rupture or embolism – rare
  • •Tear (perforation) of the heart – rare (under 1%)
  • •Stroke – reported in some studies, especially with very low oxygen

❓ Frequently Asked Questions

➤ What is the cost of balloon atrial septostomy in India?
It starts from about USD 3,500 including 3 days in the NICU. Longer stays, ventilation and corrective surgery add to the cost.
➤ Why does my baby need this procedure?
In some heart defects such as TGA, oxygen-rich blood cannot reach the body. Widening the opening between the upper chambers lets blood mix, which raises oxygen levels and keeps the baby stable until surgery.
➤ Is it an emergency?
Often yes. Many babies need it within hours or days of birth. Please contact us immediately with the echo report so we can plan safe transfer.
➤ Can a newborn travel to India safely?
Very sick babies may need an air ambulance or a medically escorted flight with oxygen and prostaglandin infusion. Our team helps arrange this.
➤ Is it open-heart surgery?
No. It is done through a thin catheter in a vein, without cutting the chest, often at the bedside in the NICU.
➤ How successful is it?
It successfully enlarges the opening in about 95% or more of babies, and oxygen levels usually rise within minutes.
➤ What are the risks?
Serious complications are uncommon (about 2–5%) and include heart rhythm problems, bleeding or rare injury to the heart. Your doctor will explain them before consent.
➤ Will my baby need more surgery?
Usually yes. Babies with TGA need an arterial switch operation, ideally within the first 2–3 weeks of life. Other defects need different repairs.
➤ How long will we stay in India?
About 10–14 days for septostomy alone. If the arterial switch is done in the same trip, plan for about 3–4 weeks.
➤ Can the mother stay with the baby?
Mothers can visit and feed the baby in the NICU. Hotel or guest house stay near the hospital is arranged by our team.
➤ How many attendants can come?
Usually two attendants (both parents) can travel on medical attendant visas. We issue the invitation letter quickly for emergencies.
➤ What follow-up is needed at home?
Regular checks with a paediatric cardiologist after corrective surgery, including echo, growth and development reviews. We arrange video follow-ups.
➤ Is an interpreter available?
Yes. Satyug provides interpreters for Arabic, French and Portuguese (plus English).
