📖 What is Tetralogy of Fallot (TOF)?

Tetralogy of Fallot (TOF) is a heart defect present from birth. It is the most common 'blue' (cyanotic) heart disease, seen in about 3–5 of every 10,000 babies. It has four parts: (1) a hole between the two pumping chambers (ventricular septal defect, VSD); (2) narrowing of the path to the lungs (pulmonary stenosis); (3) the main artery (aorta) sitting over the hole; and (4) a thickened right pumping chamber. Because of this, less blood reaches the lungs and oxygen-poor blood goes to the body, making the lips and nails look blue.

TOF repair (total correction) is open-heart surgery done using the heart-lung machine. The surgeon closes the VSD with a patch and widens the narrowed path to the lungs by removing extra muscle and, if needed, enlarging the valve or artery with a patch. The ideal age for repair is usually 3–12 months, but older children and even adults can be successfully repaired. After surgery, most children grow and play normally.

🩺 Conditions Treated

🫀
Classic Tetralogy of Fallot
VSD with narrowing below and at the lung valve.
🔵
TOF with pulmonary atresia
The lung valve is completely closed; may need a conduit (tube).
🔄
TOF after a previous shunt
Children who had a BT shunt as a baby and now need complete repair.
🧒
Late-presenting TOF
Older children and adults never operated before.
➕
TOF with additional defects
TOF with ASD, abnormal coronary artery or right-sided aortic arch.
💧
TOF with absent pulmonary valve
Rare form with very large lung arteries pressing on the airways.

⚠️ Signs That May Mean TOF

  • ➤Bluish lips, tongue or nails (cyanosis)
  • ➤'Tet spells' – sudden blue episodes when crying or feeding
  • ➤Squatting after playing or walking (older children)
  • ➤Fast breathing or breathlessness
  • ➤Poor feeding and slow weight gain
  • ➤Tiring quickly with play
  • ➤Heart murmur heard by the doctor
  • ➤Clubbing (rounded fingertips) in older children
💡 There is no medicine that cures TOF – surgery is needed in almost all children. Medicines (such as propranolol) can reduce tet spells while waiting. Very small or sick babies may first need a temporary BT shunt or a stent in the heart, followed by full repair later.

🔬 Tests Before Treatment

TestWhy it is done
Echocardiography (heart ultrasound)Confirms TOF and measures the VSD, valve and lung arteries.
Oxygen saturation (pulse oximetry)Shows how blue the child is.
Cardiac CT angiographyShows the lung arteries and coronary arteries in detail.
Cardiac catheterisation (selected cases)Measures pressures and shows small lung arteries.
ECG and chest X-rayChecks rhythm and the 'boot-shaped' heart.
Blood tests (CBC, haematocrit, clotting, blood group)Checks for thick blood and readiness for surgery.
Genetic test (22q11 deletion, if features)Associated with some cases; guides care.

🏥 The Procedure — Step by Step

TOF Repair Surgery procedure steps
1
Paediatric cardiology review
Echo and CT reviewed and surgery planned.
2
Admission
Admission 1 day before surgery for tests and blood arrangement.
3
Anaesthesia
General anaesthesia by a paediatric cardiac anaesthetist.
4
Heart-lung machine
The chest is opened through the breastbone and the heart-lung machine takes over.
5
VSD closed
The hole between the ventricles is closed with a patch.
6
Outflow widened
Extra muscle is removed and the lung valve or artery is widened with a patch (valve-sparing when possible).
7
Heart restarted and checked
Repair is checked with echo in the operating room (TEE).
8
Paediatric cardiac ICU
Ventilator for hours to a day, then close monitoring for 2–3 days.
9
Ward care
Feeding, drain removal, medicines and parents' teaching.
10
Discharge
Discharge after about 7–8 days in hospital, after a final echo.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Complete repair – valve-sparingVSD closed; outflow widened while keeping the lung valveGood-sized lung valve
Complete repair – transannular patchPatch across the lung valve to widen itSmall lung valve
Repair with RV–PA conduitTube placed from right ventricle to lung arteryPulmonary atresia or abnormal coronary artery
BT shunt / PDA or RVOT stent (staged)Temporary extra blood flow to the lungsVery small or unwell newborns
Pulmonary valve replacement (later)Valve replaced surgically or by catheterLeaking lung valve years after repair
📊 Large international registries (STS Congenital Heart Surgery Database) report early mortality of about 1–2% for elective TOF repair, and long-term studies show over 90% of patients are alive 30 years after repair.

🌱 Recovery After Treatment

🏥
Days 1–3
ICU care with breathing support and heart monitoring.
🍼
Week 1
Feeding resumes, child becomes pink; discharge after 7–8 days.
🩹
Weeks 2–4
Wound heals. Avoid lifting the child under the arms for 4–6 weeks.
🎒
Weeks 4–6
Older children can return to school.
⚽
2–3 months
Normal play and activity, as advised by the cardiologist.
📆
Lifelong
Regular heart check-ups with echo; some patients need a valve procedure years later.

🏆 Success Rate

OutcomeResult
Early survival after elective TOF repairAbout 97–99%
Long-term survival at 30 yearsOver 90%
Children with normal or near-normal activityAbout 90%
Need for pulmonary valve replacement by adulthoodAbout 10–30%
Hospital stay including ICU7–8 days

Results are best when repair is done in infancy at an experienced paediatric cardiac centre. Very low oxygen, older age at repair, small lung arteries or other defects increase the risk.

🩹 Risks & Possible Complications

TOF repair is a major but well-established operation. Possible risks include:

  • •Irregular heart rhythm soon after surgery (junctional ectopic tachycardia) – about 5–10%, usually temporary
  • •Leaking lung valve (pulmonary regurgitation) over the years – common, may need valve replacement later
  • •Small leftover hole or narrowing needing follow-up – about 5%
  • •Fluid around the lungs or heart (effusions, chylothorax) – about 5%
  • •Infection or wound problems – about 2–3%
  • •Complete heart block needing a pacemaker – about 1%
  • •Death – about 1–3% (higher in complex or late cases)

💰 Cost of TOF Repair Surgery in India

The cost of TOF repair surgery in India starts from about USD 5,500. This includes open-heart surgery, anaesthesia, perfusion, paediatric cardiac ICU and ward stay in a sharing room for 7–8 days, and routine medicines. Complex repairs with conduits, longer ICU stay or older patients cost more.
