Blalock-Taussig (BT) Shunt Surgery in India

A Blalock-Taussig shunt is a small tube that connects an artery from the body to the lung artery, so more blood reaches the lungs to pick up oxygen. It is used in babies born with “blue” (cyanotic) heart defects where too little blood flows to the lungs, as a life-saving bridge until the full repair or the next stage of surgery.

👶 What is a BT Shunt?

In some heart defects, blood cannot flow easily from the heart to the lungs, so the baby’s oxygen level is low and the lips and skin look blue. A Blalock-Taussig shunt – today usually a modified Blalock-Thomas-Taussig shunt – is a small synthetic tube (usually 3–4 mm Gore-Tex) stitched between the subclavian or innominate artery and the pulmonary artery. Blood from the body’s circulation flows through the tube into the lungs, raising oxygen levels.

The shunt does not correct the heart defect. It is a palliative bridge: it keeps the baby well and lets the lung arteries grow until the next operation – complete repair (for example Tetralogy of Fallot repair) or the next stage of single-ventricle surgery (Glenn, then Fontan). A central shunt from the aorta, or a catheter stent in the ductus arteriosus, are alternatives in some babies. See our Tetralogy of Fallot, Bidirectional Glenn and Fontan packages.

🖼️ Understand It in Pictures

💡 In simple words
Think of the lungs as a garden that is not getting enough water because the main pipe is blocked. The surgeon adds a small bypass hose from another water line straight to the garden. The garden (lungs) now gets enough water (blood), so the baby turns pink instead of blue until the main pipe can be properly fixed.

Who may need a shunt

Conditions needing a Blalock-Taussig shunt

🩺 Conditions Treated

🔴
Pulmonary atresia
With or without a ventricular septal defect; no direct flow to the lungs.
🔵
Tetralogy of Fallot (selected)
Very small babies or tiny pulmonary arteries before full repair.
🫀
Tricuspid atresia
Single-ventricle heart; first stage before Glenn and Fontan.
🔄
Single-ventricle hearts
Including after the Norwood operation for hypoplastic left heart (some centres).
💨
Critical pulmonary stenosis
When balloon dilatation is not enough.
🦠
Ebstein anomaly and others
Complex defects with low lung blood flow.

⚠️ Symptoms of Low Lung Blood Flow

  • ➤Blue lips, tongue or nails (cyanosis)
  • ➤Low oxygen saturation on a finger probe
  • ➤Fast breathing
  • ➤Poor feeding and tiredness during feeds
  • ➤Slow weight gain
  • ➤Blue spells (sudden worsening of colour, crying, limpness)
  • ➤Squatting in older children
  • ➤Clubbing of fingers in older children
💡 Not every blue baby needs a shunt. Many babies with Tetralogy of Fallot can have complete repair directly, and some newborns are treated by a catheter stent in the ductus arteriosus instead of surgery. The paediatric cardiologist and surgeon choose the safest option after echo and CT. A newborn depending on the ductus arteriosus may need a prostaglandin drip and urgent treatment at the nearest capable centre.

🔬 Tests Before Surgery

TestWhy it is done
EchocardiogramMain test: defect type and size of the pulmonary arteries.
CT angiographyMeasures arteries and plans the best shunt route.
Oxygen saturation and blood gasSeverity of cyanosis.
Chest X-ray and ECGLungs, heart size and rhythm.
Cardiac catheterisation (selected)Pressures and detailed artery anatomy.
Blood testsBlood count (often high in cyanosis), clotting and kidney tests.

🏥 The Procedure — Step by Step

BT shunt surgery steps
1
Assessment
Paediatric cardiologist and surgeon review all images and agree the plan.
2
Preparation
Prostaglandin infusion if the baby depends on the ductus arteriosus.
3
Anaesthesia
General anaesthesia by a paediatric cardiac anaesthetist.
4
Incision
A side-chest incision (thoracotomy) or mid-chest incision (sternotomy).
5
Expose the arteries
The subclavian (or innominate) artery and pulmonary artery are carefully exposed.
6
Heparin
A blood thinner is given so the new tube does not clot.
7
Shunt placement
A small Gore-Tex tube is stitched between the two arteries, usually without the heart-lung machine.
8
Check flow
Oxygen levels and blood pressure are checked as blood flows to the lungs.
9
Close
A chest drain is placed and the wound is closed.
10
Recovery
Ventilator and ICU care for a few days, then ward; aspirin to keep the shunt open.

📚 Evidence note: In a large US database study of newborns having a modified BT shunt (Petrucci et al., Annals of Thoracic Surgery 2011), discharge mortality was 7.2% among 1,273 babies, with higher risk in very small babies and complex single-ventricle hearts. Shunt narrowing or clotting is the main early complication, which is why heparin and then aspirin are used and oxygen levels are monitored closely.

🌱 Recovery and Care at Home

💊
Aspirin daily
A small daily dose helps keep the shunt open – never miss doses.
💧
Avoid dehydration
Vomiting, diarrhoea or fever can cause shunt clotting – seek care early.
📈
Oxygen level
Oxygen stays lower than in a healthy child; your doctor gives your child’s target range.
🍼
Feeding and growth
Regular weight checks; extra calories if growth is slow.
📅
Next operation
Full repair or Glenn usually within months; follow-up echo is essential.
⚠️
Warning signs
Bluer colour, fast breathing, poor feeding or a sleepy baby need urgent review.

🏆 Results & Outlook

OutcomeResult
Survival to discharge after neonatal BT shunt (US STS database, 1,273 babies)About 93% (discharge mortality 7.2%)
Long-termDepends on the heart defect and on the next stage of surgery

🩹 Risks & Possible Complications

  • •Shunt narrowing or clotting – the main early risk; may need urgent catheter or surgical treatment
  • •Too much lung blood flow (over-circulation) causing breathlessness or low blood pressure – managed with medicines or shunt adjustment
  • •Chylothorax (lymph fluid around the lung) – occasional
  • •Nerve injury (phrenic or recurrent laryngeal) – uncommon
  • •Bleeding or infection – uncommon
  • •Distortion of the pulmonary artery at the shunt site – occasional, repaired at the next operation

❓ Frequently Asked Questions

➤ How much does BT shunt surgery cost in India?
About USD 6,000–7,300 at leading Delhi NCR hospitals (BLK-Max and Artemis quotes, Oct 2026), plus USD 500–800 for evaluation.
➤ Is a BT shunt a cure?
No. It is a bridge that improves oxygen levels until the full repair or the next stage of surgery.
➤ How long will my baby be in hospital?
Usually 5–7 days, including a few days in the ICU.
➤ How long do we need to stay in India?
About 3–4 weeks, including tests, surgery and follow-up.
➤ Is the heart-lung machine used?
Usually not. Most shunts are placed without bypass, though some babies need it.
➤ What oxygen level should my baby have after the shunt?
It stays lower than normal because the defect is not yet repaired. The doctor gives your baby’s target range.
➤ Why is aspirin needed?
Aspirin reduces the risk of the shunt clotting. It must be given every day.
➤ What happens next?
Your child will need a full repair or the next stage (for example Glenn) usually within months.
➤ What are the main risks?
Shunt clotting or narrowing, too much lung blood flow, chylothorax and bleeding. Overall risk is higher in very small or complex babies.
➤ Is there an alternative to surgery?
In some babies, a catheter stent in the ductus arteriosus can be used instead. The team advises which is safer.
➤ What should I do if my baby has diarrhoea or fever?
Seek medical care early – dehydration can cause the shunt to clot.
➤ Can one parent stay with the baby?
Yes, one parent can usually stay in the room; ICU visiting follows hospital rules.
➤ When can we fly home?
Usually 1–2 weeks after discharge, once the echo is good and the doctors agree.
➤ Do you provide an interpreter?
Yes. Free interpreters for Arabic, French and Russian (plus English), plus a coordinator for all hospital visits.
