📖 What is CABG (Heart Bypass) Surgery?

The heart muscle gets its blood from small arteries on its surface called coronary arteries. With age, diabetes, smoking, high cholesterol and high blood pressure, fatty plaque builds up inside these arteries and narrows them. This is called coronary artery disease. Less blood reaches the heart, which causes chest pain (angina), breathlessness and, when an artery closes suddenly, a heart attack.

Coronary artery bypass grafting (CABG, said 'cabbage') creates a new road for blood around the blocked part. The surgeon takes a healthy blood vessel from the chest wall (internal mammary artery), the arm (radial artery) or the leg (saphenous vein) and joins it above and below the blockage. It can be done with the heart-lung machine (on-pump) or on a beating heart (off-pump, OPCAB). Most patients get 3 or 4 grafts in one operation.

🖼️ Understand It in Pictures

💡 In simple words
The heart gets blood through small coronary arteries. Fatty plaque can narrow or block them, causing chest pain or heart attack. Bypass surgery uses a healthy blood vessel from your chest, arm or leg to make a new route around the block.

🩺 Conditions Treated

🫀
Triple vessel disease
Blockages in all three main heart arteries – CABG gives better long-term results than stents, especially in diabetics.
⚠️
Left main artery disease
A tight blockage of the main trunk artery that feeds most of the left side of the heart.
🩸
Diabetes with multi-vessel disease
Large trials show diabetics with several blocked arteries live longer after CABG than after stenting.
💔
Weak heart after heart attack
Reduced pumping (low ejection fraction) with living heart muscle that can recover once blood flow returns.
🔁
Failed or blocked stents
Repeated narrowing inside stents, or arteries that are too long, calcified or twisted for stents.
🔧
Bypass with valve repair
CABG combined with valve repair or replacement when a valve is also damaged.

⚠️ Symptoms That May Need Bypass Surgery

  • ➤Chest pain or pressure on walking or climbing stairs
  • ➤Pain spreading to the left arm, jaw, neck or back
  • ➤Breathlessness on mild effort
  • ➤Heavy tiredness and low exercise capacity
  • ➤Sweating, nausea or dizziness with chest discomfort
  • ➤Swelling of the feet (heart weakness)
  • ➤A previous heart attack
  • ➤Angiography showing several tight blockages
💡 Not every heart blockage needs bypass surgery. Single or simple blockages are often treated with medicines or an angioplasty with stent. Your heart team (cardiologist and cardiac surgeon) reviews your angiography and SYNTAX score to decide between medicines, stents or CABG.

🔬 Tests Before Treatment

TestWhy it is done
Coronary angiographyShows exactly where the arteries are blocked and how badly – the key test for planning grafts.
Echocardiography (2D Echo)Measures heart pumping (ejection fraction) and checks the heart valves.
ECG and stress test / viability scanShows heart strain and whether weak heart muscle is still alive and can recover.
Blood tests (CBC, kidney, liver, sugar, HbA1c, clotting)Checks fitness for surgery, diabetes control and bleeding risk.
Chest X-ray and lung function testChecks the lungs, important for breathing after surgery.
Carotid & leg vein DopplerChecks neck arteries for stroke risk and leg veins for use as grafts.

🏥 The Procedure — Step by Step

CABG Surgery procedure steps
1
Admission & checks
You are admitted 1 day before surgery. Tests are repeated, and the surgeon and anaesthetist explain the plan.
2
Blood thinner plan
Some blood thinners (like clopidogrel) are stopped a few days before; aspirin is usually continued.
3
General anaesthesia
You are fully asleep. A breathing tube, IV lines and a urine catheter are placed.
4
Opening the chest
The breastbone (sternum) is opened through a cut of about 20–25 cm down the middle of the chest.
5
Harvesting the grafts
The internal mammary artery is freed from the chest wall; vein from the leg or artery from the arm is taken at the same time.
6
Heart-lung machine or off-pump
The heart is either stopped and supported by a heart-lung machine, or kept beating and steadied with a stabiliser (off-pump).
7
Joining the grafts
Each graft is stitched below the blockage and to the aorta above, using very fine stitches under magnification.
8
Closing the chest
Graft flow is checked, drain tubes are placed and the breastbone is closed with steel wires.
9
ICU care
You spend about 1–2 days in the cardiac ICU. The breathing tube is usually removed within 4–12 hours.
10
Ward recovery & discharge
About 5 days in the ward with walking, breathing exercises and physiotherapy, then discharge with medicines and a clear plan.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
On-pump CABGHeart is stopped; heart-lung machine pumps blood during graftingMost patients, complex or many grafts
Off-pump CABG (OPCAB)Grafts stitched on the beating heart without the heart-lung machinePatients with kidney disease, calcified aorta or stroke risk
Total arterial revascularisationUses mainly arteries (both mammary arteries, radial artery) instead of veinsYounger patients – arterial grafts stay open longer
MICS CABG (minimally invasive)1–2 grafts through a small cut between the ribs, breastbone not cutSelected patients with 1–2 blocked arteries
CABG with valve surgeryBypass plus valve repair or replacement in one operationPatients with both blocked arteries and a damaged valve
📊 In the FREEDOM trial (NEJM 2012), diabetic patients with multi-vessel disease had fewer deaths, heart attacks and strokes over 5 years after CABG than after stents (18.7% vs 26.6%). The internal mammary artery graft stays open in about 90% or more of patients at 10 years.

🏆 Success Rate

OutcomeResult
Survival after planned (elective) CABGAbout 97–98%
Relief of anginaAbout 90% of patients are free of chest pain
Mammary artery graft open at 10 yearsAbout 90% or more
Vein grafts open at 10 yearsAbout 50–60%
Survival at 10 yearsAbout 75–80% (depends on age, heart strength, diabetes)

Results are best when the heart pump is still fairly strong, the patient stops smoking, and diabetes, blood pressure and cholesterol are well controlled after surgery. Age, kidney disease and emergency surgery increase risk.

🩹 Risks & Possible Complications

CABG is a major operation, but in experienced centres serious problems are uncommon.

  • •Irregular heartbeat (atrial fibrillation): about 20–30%, usually temporary and treated with medicines
  • •Wound or leg-vein site infection: about 2–5%
  • •Bleeding needing re-opening of the chest: about 2–3%
  • •Short-term memory or concentration problems: common in the first weeks, usually improve
  • •Kidney strain: about 1–5%, higher in diabetics and those with kidney disease
  • •Stroke: about 1–2%
  • •Heart attack around surgery or graft blockage: about 1–3%

❓ Frequently Asked Questions

➤ What is the cost of CABG surgery in India?
CABG in India starts from about USD 5,000 for a standard bypass in a twin-sharing room. A private room, total arterial grafts or high-risk cases cost about USD 6,000–11,000. We send a written estimate after reviewing your angiography.
➤ How long do I need to stay in India?
Plan for about 2–3 weeks. This includes 1–2 days of tests, about 1–2 days in ICU and 5 days in the ward, and 7–10 days of rest near the hospital before you fly.
➤ Is CABG better than stents?
For blockages in all three arteries, left main disease or diabetes with several blocked arteries, studies show CABG gives longer life and fewer repeat procedures than stents. For one or two simple blockages, stents are usually enough.
➤ What is the success rate of bypass surgery in India?
Survival after planned CABG is about 97–98% in experienced Indian centres, similar to international results. About 90% of patients are free of chest pain afterwards.
➤ What is off-pump (beating-heart) CABG?
The surgeon stitches the grafts while the heart keeps beating, without the heart-lung machine. It may suit patients with kidney disease, a calcified aorta or stroke risk. Your surgeon chooses the safest method for you.
➤ Is the surgery painful?
You are fully asleep during surgery. Afterwards the chest is sore for 2–4 weeks, mainly when coughing or moving. Pain medicines keep this well controlled.
