📖 What is Cardiac Catheterization?

The heart has four chambers and valves that keep blood flowing in one direction to the lungs and body. Holes in the heart, narrow or leaking valves, weak heart muscle or high blood pressure in the lungs (pulmonary hypertension) change the pressures and oxygen levels inside these chambers. Echo and CT give pictures, but sometimes doctors need exact measurements before deciding on surgery or medicines.

In cardiac catheterization, a thin, flexible tube (catheter) is passed through a blood vessel in the wrist, groin or neck into the heart under X-ray guidance. A right heart cath measures pressures in the right heart and lungs and the heart's output; a left heart cath measures pressures in the left heart and aorta. Blood samples measure oxygen levels to detect shunts (holes). Dye can be injected to see chambers and vessels. It is the gold-standard test for many heart conditions.

🖼️ Understand It in Pictures

💡 In simple words
A cardiac catheterization is a test where a thin flexible tube is passed through the wrist or groin up to the heart. Doctors measure pressures inside the heart chambers and lungs, check the valves, and may inject dye to see the heart arteries.

🩺 Conditions Diagnosed

👶
Congenital heart disease
ASD, VSD, PDA, tetralogy and complex defects – to measure shunts and plan surgery.
🫁
Pulmonary hypertension
Confirms the diagnosis and tests response to medicines (vasoreactivity).
🚪
Heart valve disease
Measures severity of narrowed or leaking valves when echo is unclear.
💔
Heart failure and cardiomyopathy
Measures filling pressures and heart output; part of transplant work-up.
🩺
Pericardial disease
Distinguishes constrictive pericarditis from restrictive cardiomyopathy.
🩸
Before heart surgery
Checks operability, lung pressures and coronary arteries before surgery.

⚠️ Symptoms That May Need a Heart Cath

  • ➤Breathlessness on exertion
  • ➤Blue lips or fingers (cyanosis)
  • ➤Poor growth or feeding in babies
  • ➤Chest pain
  • ➤Fainting or dizziness
  • ➤Swelling of legs or abdomen
  • ➤Heart murmur found by a doctor
  • ➤Unexplained tiredness
💡 Many heart problems can be diagnosed without a cath. Echocardiography, cardiac CT and MRI answer most questions. A cath is advised when exact pressures are needed, when tests disagree, before certain surgeries, or to diagnose pulmonary hypertension. If blocked coronary arteries are the main concern, a coronary angiography is done (available as a combined package).

🔬 Tests Before Treatment

TestWhy it is done
EchocardiogramShows heart structure and function and guides the cath plan.
ECGRecords heart rhythm.
Blood tests (kidney function, CBC, clotting)Ensure it is safe to use dye and puncture blood vessels.
Chest X-rayChecks heart size and lungs.
CT or MRI heart (selected cases)Gives detailed anatomy in complex cases.
Viral markersStandard screening before any invasive procedure.

🏥 The Procedure — Step by Step

Cardiac Catheterization procedure steps
1
Consultation
The cardiologist reviews your echo and reports and explains why the cath is needed.
2
Admission and fasting
Day-care admission after 4–6 hours fasting; an IV line is placed.
3
Local anaesthesia / sedation
Adults get local anaesthesia; children usually receive sedation or general anaesthesia.
4
Vessel access
A small sheath is placed in the wrist, groin or neck vessel.
5
Catheter to the heart
The catheter is guided into the heart chambers under X-ray.
6
Pressure measurements
Pressures in each chamber and the lung arteries are recorded.
7
Oxygen sampling
Blood samples from different chambers detect holes (shunts).
8
Special tests (if needed)
Vasoreactivity test with oxygen or nitric oxide, or dye injection.
9
Sheath removal and rest
The sheath is removed and pressure applied; rest for 2–6 hours.
10
Report and discharge
Same day or next day discharge with the report and treatment plan.

⚖️ Types of Heart Cath Study

StudyWhat is doneBest for
Right heart catheterizationPressures in right heart and lung arteries, cardiac outputPulmonary hypertension, heart failure, transplant work-up
Left heart catheterizationPressures in left ventricle and aortaValve disease, cardiomyopathy
Cath + coronary angiographyPressure study plus dye pictures of coronary arteriesAdults with suspected blockages or before valve surgery
Paediatric cathFull shunt and pressure study under sedation/GACongenital heart disease in children
📊 ESC/ERS 2022 pulmonary hypertension guidelines require right heart catheterization to confirm the diagnosis (mean pulmonary artery pressure above 20 mmHg), and large series report major complication rates below 1% in experienced centres.

Accuracy depends on careful measurement in an experienced lab and clear clinical questions. For children, sedation and experienced paediatric teams make the study safe and reliable.

🩹 Risks & Possible Complications

Cardiac catheterization is a low-risk test.

  • •Bruising or small haematoma at the puncture site – about 2–5%
  • •Temporary irregular heartbeat during the test – common, settles quickly
  • •Allergy to contrast dye – uncommon
  • •Kidney strain from dye – uncommon, higher in diabetic or kidney patients
  • •Bleeding or vessel injury needing repair – under 1%
  • •Stroke, heart attack or death – very rare, about 1 in 1,000 or less

This covers the cath lab procedure, cardiologist fees, consumables, local anaesthesia and day-care stay in a twin-sharing room. Combined angiography, paediatric sedation or general anaesthesia, and an overnight stay cost more.

❓ Frequently Asked Questions

➤ How much does cardiac catheterization cost in India?
A diagnostic right or left heart cath starts from about USD 500. With coronary angiography it is about USD 800, and a paediatric cath under sedation or GA is about USD 900.
➤ Do I need to stay in hospital?
Most adults go home the same day. Some patients and most children stay one night.
➤ How long should I stay in India?
About 3–5 days for consultation, the test and the report discussion. Longer if surgery or a device closure is planned.
➤ Is it painful?
Only the numbing injection stings briefly. You may feel some pressure, but the catheter in the heart is not painful.
➤ Will I be awake?
Adults are usually awake with mild sedation. Children are given sedation or general anaesthesia.
➤ Is it the same as angiography?
No. Angiography takes dye pictures of the coronary arteries; catheterization measures pressures and oxygen inside the heart. Both can be done together.
➤ Why do I need it if I already had an echo?
Echo estimates pressures, but a cath measures them directly and accurately, which is essential for some surgical and medicine decisions.
➤ Is it safe for children?
Yes. Paediatric cath labs routinely study newborns and children, with major complications under 1%.
➤ What are the risks?
Bruising at the puncture site is most common. Serious problems like bleeding, stroke or heart attack are very rare.
➤ When can I fly home?
Usually 1–2 days after the test.
➤ Can treatment be done in the same sitting?
Sometimes, for example device closure of a hole or balloon valvotomy, but this is planned and quoted separately.
➤ How many attendants can come?
One or two attendants can come; for children both parents can accompany. We provide medical visa letters.
➤ What follow-up is needed at home?
Your cardiologist sends a full report and plan. Follow-up depends on the diagnosis and can be done online.
➤ Is an interpreter available?
Yes, Satyug Healthcare provides free interpreters in Arabic, French and Portuguese.
