🌷 GYNAECOLOGIC ONCOLOGY PACKAGE — INDIA

Uterine (Endometrial) Cancer Surgery in India

Endometrial cancer starts in the lining of the womb (uterus) and is the most common cancer of the womb. Most women are diagnosed early because of abnormal bleeding after menopause, and surgery – a hysterectomy with removal of both tubes and ovaries and checking of the lymph nodes – cures most of them. In India this is usually done by keyhole or robotic surgery by gynaecologic oncology teams.

📖 What is Endometrial Cancer Surgery?

The endometrium is the inner lining of the womb. Endometrial cancer grows from this lining and usually affects women after menopause. The most common type, endometrioid adenocarcinoma, is often linked to long-term exposure to oestrogen – for example with obesity, diabetes, polycystic ovaries, or tamoxifen use. Women with Lynch syndrome (an inherited condition) have a much higher lifetime risk. Rarer, more aggressive types include serous and clear-cell carcinoma and carcinosarcoma.

The main treatment is surgery: a total hysterectomy (removal of the womb and cervix) with bilateral salpingo-oophorectomy (both fallopian tubes and ovaries), plus staging of the lymph nodes – today usually by sentinel lymph node mapping. The final pathology report, including the tumour’s molecular type, decides whether any further treatment such as radiotherapy, chemotherapy or immunotherapy is needed. Cancer of the cervix is a different disease, covered on our cervical cancer treatment page.

🖼️ Understand It in Pictures

💡 In simple words
A cancer is growing in the inner lining of the womb. It usually shows itself early as bleeding after menopause. Through small keyhole cuts, the surgeon removes the womb, cervix, tubes and ovaries and checks the nearby lymph nodes. Most women with early cancer need no further treatment after this operation.

What decides treatment after surgery?

Treatment after endometrial cancer surgery by risk group

🩺 Conditions Treated

🔴
Early endometrial cancer
Stage I–II endometrioid cancer – surgery is usually the only treatment needed.
📊
High-risk types
Serous, clear-cell and carcinosarcoma: surgery plus omentectomy and wider staging, followed by chemotherapy.
🎯
Advanced endometrial cancer
Stage III–IV disease: cytoreductive surgery where possible, combined with chemotherapy and immunotherapy.
⚠️
Atypical endometrial hyperplasia
A pre-cancer with a high risk of hidden cancer; hysterectomy is usually advised.
🧬
Lynch syndrome
Risk-reducing hysterectomy and removal of tubes and ovaries after childbearing.
🩺
Uterine sarcoma
Rare cancers of the womb muscle, quoted case by case.

⚠️ Symptoms That May Need Evaluation

  • ➤Any vaginal bleeding or spotting after menopause
  • ➤Heavier, longer or irregular periods around menopause
  • ➤Bleeding between periods
  • ➤Watery or blood-stained vaginal discharge
  • ➤Pain or pressure in the lower tummy or pelvis
  • ➤Pain during intercourse
  • ➤A thickened womb lining on ultrasound after menopause
  • ➤Weight loss or a pelvic mass (later stages)
💡 Not all bleeding after menopause is cancer – it is often due to thinning of the vaginal tissues, polyps or benign hyperplasia. But it must always be checked with a transvaginal ultrasound and an endometrial biopsy (often done in the clinic, or with hysteroscopy). Surgery is planned only after the diagnosis is confirmed.

🔬 Tests Before Treatment

TestWhy it is done
Endometrial biopsy or hysteroscopy with biopsyConfirms cancer, its type and grade.
Pathology review incl. molecular tests (MMR/MSI, p53, POLE)Molecular type helps predict risk and guide treatment after surgery.
MRI of the pelvisShows how deep the cancer has grown into the womb wall and whether the cervix is involved.
CT scan of chest, abdomen and pelvis (or PET-CT)Checks lymph nodes and spread, especially for high-risk types.
Blood tests incl. CA-125, kidney and liver function, blood countFitness for surgery; CA-125 may be raised if the disease has spread.
ECG, echo and chest X-rayHeart and lung check before anaesthesia.

🏥 The Procedure — Step by Step

Endometrial cancer surgery procedure steps
1
Evaluation in India
Gynae-oncology consultation, pathology review, MRI and blood tests.
2
Tumour board
Gynaecologic oncologist, radiologist, pathologist and oncologists agree on the plan.
3
Admission and anaesthesia
Admission on the day before or day of surgery; general anaesthesia.
4
Keyhole access
A camera and 3–5 small ports are placed (laparoscopic or robotic); open surgery is used for very large uteri or advanced disease.
5
Inspection and washings
The abdomen and pelvis are carefully inspected and fluid washings taken.
6
Sentinel lymph node mapping
A dye (usually ICG) is injected into the cervix; the first draining lymph nodes light up and are removed.
7
Hysterectomy and BSO
The womb, cervix, both tubes and both ovaries are freed and removed through the vagina.
8
Extra staging when needed
Omentectomy and wider lymph node removal for serous or advanced cancers; removal of visible disease in advanced cases.
9
Closure
The top of the vagina is stitched; the small cuts are closed.
10
Recovery and results
Catheter out the next day, discharge in about 1–4 days; final pathology in about 7–10 days decides further treatment.

Ovary-sparing surgeryOvaries keptSelected young women with low-grade early cancer
Fertility-sparing hormone therapyProgestin (e.g. hormonal IUS) with close biopsiesSelected young women with grade 1 cancer limited to the lining
📚 Evidence note: In the GOG LAP2 trial, laparoscopy had fewer moderate-to-severe complications than open surgery (14% vs 21%) and far fewer women stayed more than 2 days in hospital (52% vs 94%), with the same 5-year survival (89.8% in both groups). The LACE trial found equal 4.5-year disease-free survival after laparoscopic and open hysterectomy (81.3% vs 81.6%). In the FIRES study, sentinel lymph node mapping detected node spread with 97.2% sensitivity and a 99.6% negative predictive value.

🌱 Recovery After Treatment

🚶
First days
Walking the same evening or next day; mild pain and some vaginal spotting are normal.
🏠
Weeks 1–2
Rest at home or hotel; light walking every day.
💼
Weeks 2–4
Most women return to light work after keyhole surgery; 6 weeks or more after open surgery.
🚫
Weeks 6–8
No heavy lifting, intercourse or swimming until the top of the vagina has healed.
🌡️
Menopause symptoms
If the ovaries are removed before menopause, hot flushes may start; your doctor can advise on management.
🔬
Follow-up
Check-ups every 3–6 months for the first 2–3 years, then every 6–12 months; report any new bleeding.

🏆 Success Rate

OutcomeResult
5-year relative survival, cancer confined to the womb (ACS/SEER)About 95%
5-year relative survival, spread to nearby tissues or nodes (ACS/SEER)About 70%
5-year survival after laparoscopic or open surgery (GOG LAP2)89.8% in both groups
Vaginal recurrence at 5 years, vaginal brachytherapy vs external radiotherapy (PORTEC-2)1.8% vs 1.6%
💡 About two-thirds of endometrial cancers are found while still confined to the womb, which is why the outlook is usually very good. Results depend on stage, grade, cell type and molecular type. Even advanced and recurrent cancers now respond better to chemotherapy combined with immunotherapy, especially mismatch-repair-deficient (dMMR) tumours.

🩹 Risks & Possible Complications

  • •Bleeding needing blood transfusion – uncommon
  • •Urine or wound infection – occasional
  • •Conversion from keyhole to open surgery – uncommon (more often with obesity or adhesions)
  • •Injury to the bladder, ureter or bowel – rare (about 1 in 100 or fewer)
  • •Separation of the vaginal vault stitches – rare; avoid intercourse until healed
  • •Leg swelling (lymphoedema) – much less common with sentinel node mapping than full lymph node removal
  • •Early menopause symptoms if ovaries are removed before menopause
  • •Blood clots in the legs or lungs – prevented with early walking and blood thinners

❓ Frequently Asked Questions

➤ How much does endometrial cancer surgery cost in India?
At BLK-Max, New Delhi, surgery with hysterectomy, removal of tubes and ovaries and lymph nodes is quoted at about USD 6,700 (open, economy) and USD 8,350–10,000 (robotic, economy). Twin and single rooms cost more. Evaluation of about USD 2,000 is extra.
➤ Is keyhole surgery as safe as open surgery for womb cancer?
Yes. Large trials (GOG LAP2 and LACE) showed the same survival with fewer complications and a shorter hospital stay after keyhole surgery.
➤ Do the ovaries have to be removed?
Usually yes, because the cancer can spread to them and they make oestrogen. In selected young women with low-grade early cancer, the ovaries may be kept.
➤ What is sentinel lymph node mapping?
A dye is injected into the cervix and the first lymph nodes it reaches are removed and tested. It is accurate and causes much less leg swelling than removing all the nodes.
➤ How long is the hospital stay?
About 1–4 days after keyhole surgery; longer after open surgery.
➤ How long must I stay in India?
About 2–3 weeks, including tests, surgery, recovery and the final pathology review.
➤ Will I need radiotherapy or chemotherapy?
Many women with early, low-grade cancer need no further treatment. Others may need vaginal brachytherapy, and higher-risk or advanced cancers may need chemotherapy, immunotherapy or external radiotherapy.
➤ Can I still have children?
Hysterectomy ends fertility. Selected young women with grade 1 cancer limited to the lining may be offered hormone treatment instead, with close monitoring.
➤ Is bleeding after menopause always cancer?
No, but it must always be checked with an ultrasound and a biopsy because endometrial cancer is a common cause.
➤ What are molecular tests and why do they matter?
Tests such as MMR, p53 and POLE classify the tumour into groups with different risks. They help decide whether further treatment is needed, and dMMR results may also suggest testing for Lynch syndrome.
➤ When can I return to normal activities?
Most women resume light activities in 2–4 weeks after keyhole surgery; heavy lifting and intercourse should wait about 6–8 weeks.
➤ What follow-up is needed?
Check-ups every 3–6 months for the first 2–3 years and then less often. Report any new bleeding, pain or swelling.
➤ Do you have female doctors and coordinators?
Yes. Female gynaecologic oncologists and female coordinators can be arranged on request.
➤ Do you provide an interpreter?
Yes. Free interpreters for Arabic, French, Russian and other languages, plus a coordinator for all hospital visits.
