Proton Beam Therapy in India
Proton beam therapy is an advanced form of radiation therapy that uses positively charged particles (protons) instead of X-rays to destroy cancer cells. Because protons release most of their energy at a precise depth and then stop, they spare much more of the healthy tissue behind the tumour. This makes proton therapy especially valuable for children, for tumours close to the brain, spinal cord, eyes or heart, and for patients who need re-irradiation. India now has dedicated pencil-beam proton facilities, and international patients can receive this treatment at a fraction of the cost charged in the USA or Europe. Satyug Healthcare coordinates the medical review, treatment planning appointment, visa, accommodation and daily transport for the entire course.
Cost of proton beam therapy in India starts from USD 25000 at leading JCI / NABH accredited hospitals. Mostly outpatient, 5 sessions per week; total stay in India 4-8 weeks depending on the number of fractions.

About the Procedure
- Proton therapy delivers radiation using a beam of protons produced by a cyclotron, a large particle accelerator housed in a specially shielded building.
- The key physical advantage is the Bragg peak: protons deposit most of their dose at the tumour and almost none beyond it, unlike X-ray beams that continue through the body.
- Modern centres in India use pencil beam scanning (PBS) and intensity-modulated proton therapy (IMPT), which paint the dose layer by layer to match the exact shape of the tumour.
- Common indications include paediatric brain tumours, medulloblastoma, skull base and spinal chordomas and chondrosarcomas, head and neck cancers, eye tumours, liver cancer, prostate cancer, some lung and oesophageal cancers and selected recurrent tumours.
- Treatment is given in small daily doses called fractions, usually five days a week; each session takes 20-45 minutes, of which the beam itself is on for only a few minutes.
- Proton therapy may be used alone or combined with surgery and chemotherapy, as decided by a multidisciplinary tumour board.
Purpose of the Procedure
- To destroy or control cancer cells while giving a lower radiation dose to surrounding healthy organs.
- To reduce the risk of long-term side effects such as growth problems, hormone deficiencies, hearing loss and learning difficulties in children.
- To lower the lifetime risk of a second radiation-induced cancer, which is particularly important for young patients.
- To allow higher, more effective doses to tumours that sit next to critical structures such as the optic nerves, brainstem or spinal cord.
- To offer re-irradiation for recurrent tumours where conventional radiation would exceed safe tissue limits.
- To preserve quality of life by reducing side effects such as dry mouth, swallowing difficulty and fatigue in head and neck cancers.
Diagnosis and Tests Before the Procedure
- Review of all pathology, biopsy reports and previous treatment summaries by the proton therapy team before travel to confirm suitability.
- Recent MRI and/or CT scans of the tumour area; PET-CT may be requested to stage the disease accurately.
- Histopathology slide and block review in India if the original diagnosis needs confirmation.
- Blood tests including complete blood count, kidney and liver function.
- Consultation with the radiation oncologist and, where needed, the surgical and medical oncology teams at a tumour board.
- Anaesthesia assessment for young children who will need sedation to stay still during daily sessions.
- Dental, eye, hearing or endocrine baseline checks depending on the area to be treated.
Step-by-Step Procedure
- Arrival and consultation: the radiation oncologist examines the patient, reviews scans and explains the treatment plan and expected side effects.
- Immobilisation: a custom mask, mould or vacuum cushion is made so the patient lies in exactly the same position every day.
- Simulation scan: a planning CT, often fused with MRI or PET images, is taken in the treatment position.
- Contouring: doctors outline the tumour and every nearby organ at risk on the planning images.
- Treatment planning: medical physicists design the pencil-beam plan and run quality-assurance checks; this usually takes 5-10 working days.
- Daily treatment: before each session imaging confirms the position, then the gantry rotates and the beam is delivered; the patient feels nothing during treatment.
- Weekly reviews: the oncologist checks side effects, weight and blood counts, and a repeat scan may be done mid-course to adapt the plan.
- Completion: a final review is held, a treatment summary is issued and a follow-up imaging schedule is agreed with the home oncologist.
Risks and Possible Complications
- Fatigue, which often builds up during the course and settles within a few weeks of completion.
- Skin redness, dryness or hair loss in the treated area.
- Site-specific effects such as mouth soreness, taste change and swallowing discomfort in head and neck treatment, or bladder and bowel irritation in pelvic treatment.
- Nausea, headache or temporary swelling when the brain is treated, which is managed with medicines.
- Rare late effects such as tissue scarring, hormone changes or damage to nearby nerves, although the risk is generally lower than with conventional radiation.
- Anaesthesia-related risks in children who need daily sedation.
- Not every cancer gains a meaningful advantage from protons; for some cases modern photon techniques give similar results at lower cost.
Your specialist will explain the risks specific to your condition. Choosing an experienced, high-volume hospital keeps complication rates low.
Recovery and Aftercare
- Most patients continue normal daily activities throughout treatment and stay in nearby accommodation.
- Acute side effects usually peak in the last week of treatment and improve over 2-4 weeks afterwards.
- Patients are usually fit to fly within a few days of the final session, once the oncologist has reviewed them.
- A first follow-up MRI or CT is normally done 6-12 weeks after treatment and can be performed in the home country.
- Children are followed up long term for growth, hormones, hearing and school performance.
- Good nutrition, hydration, gentle skin care and avoiding sun exposure on the treated area aid recovery.
