CyberKnife Radiosurgery in India

CyberKnife is a robotic radiosurgery system that delivers hundreds of precisely focused radiation beams to a tumour from many different angles, destroying or controlling it without a single incision. Despite the name, there is no knife and no cut - treatment is painless, needs no general anaesthesia and is usually completed in one to five outpatient sessions. Because the robot tracks the tumour and corrects for movement in real time, CyberKnife can treat tumours in the brain, spine, lung, liver, pancreas, kidney and prostate with sub-millimetre accuracy while sparing healthy tissue. Several accredited hospitals in Delhi/NCR operate CyberKnife units, and Satyug Healthcare coordinates your opinion, visa, travel and treatment schedule.

Cost of CyberKnife radiosurgery in India starts from USD 4500 at leading JCI / NABH accredited hospitals. No admission - 1 to 5 outpatient sessions; total stay in India 1-2 weeks.

About the Procedure

  • CyberKnife combines a compact linear accelerator mounted on a robotic arm with continuous X-ray image guidance, allowing beams to be aimed at the target from more than a thousand possible directions.
  • Unlike frame-based radiosurgery, CyberKnife is frameless: patients wear a soft thermoplastic mask for brain treatments or simply lie comfortably on the couch for body treatments.
  • Its real-time tracking systems follow the tumour during treatment, including lung and liver tumours that move with breathing, so the radiation dose follows the target.
  • Treatment can be given in a single session (stereotactic radiosurgery, SRS) or in 2-5 sessions (hypofractionated SRS or SBRT), which allows larger tumours or those close to sensitive structures to be treated safely.
  • Common uses include brain metastases, meningiomas, acoustic neuromas (vestibular schwannomas), pituitary tumours, arteriovenous malformations (AVMs), trigeminal neuralgia, spinal tumours, early-stage lung cancer, liver tumours, pancreatic cancer and localised prostate cancer.
  • CyberKnife is often used for patients who are not fit for surgery, for tumours in difficult locations, or for re-treatment of areas that have already received conventional radiotherapy.

Purpose of the Procedure

  • To destroy or permanently stop the growth of tumours without open surgery.
  • To treat tumours that are inoperable or located close to critical structures such as the brainstem, optic nerves or spinal cord.
  • To offer a non-invasive alternative to surgery for patients who are elderly or have medical conditions that make anaesthesia risky.
  • To control limited metastases (oligometastases) in the brain, lung, liver, spine or bones.
  • To relieve pain from spinal metastases or trigeminal neuralgia.
  • To complete treatment in a few days rather than the several weeks needed for conventional radiotherapy, which suits international patients with limited time.

Diagnosis and Tests Before the Procedure

  • Consultation with a radiation oncologist and, where relevant, a neurosurgeon or the organ-specific specialist.
  • Review of all previous scans, biopsy reports, surgery and radiotherapy records to confirm that CyberKnife is suitable.
  • Contrast MRI (for brain and spine lesions) with thin slices for precise target definition.
  • Planning CT scan in the treatment position, fused with MRI or PET-CT images for accurate contouring.
  • Biopsy or histopathology review where the nature of the lesion is not already confirmed.
  • Fiducial marker placement (tiny gold seeds) in some liver, pancreas or prostate cases to help the robot track the tumour.
  • Blood tests such as kidney function (for contrast imaging) and general health assessment.
  • Tumour board discussion to decide the number of sessions, dose and how CyberKnife fits with surgery, chemotherapy or other treatments.

Step-by-Step Procedure

  1. At the planning visit, a custom thermoplastic mask (for head and neck) or body cushion is made so that you lie in exactly the same position every session.
  2. A planning CT scan, and MRI or PET-CT if needed, is performed and fused on the planning computer.
  3. The radiation oncologist outlines the tumour and nearby normal organs, and the medical physicist designs a plan with hundreds of beams; this usually takes 2-5 days.
  4. On treatment day you lie on the couch wearing comfortable clothes; no anaesthesia, incision or hospital admission is required.
  5. The robotic arm moves silently around you, delivering radiation beams while X-ray cameras continuously check and correct for any movement.
  6. Each session typically lasts 30-90 minutes; you can listen to music and the team watches and talks to you throughout.
  7. Depending on the plan, you receive 1 to 5 sessions, usually on consecutive or alternate days.
  8. After each session you walk out and return to your hotel; steroids or anti-nausea tablets may be given to prevent short-term side effects.
  9. A final review is held with the radiation oncologist, who gives you a treatment summary and a follow-up imaging schedule.

Risks and Possible Complications

  • Tiredness for a few days to weeks after treatment.
  • Temporary headache, nausea or dizziness after brain radiosurgery, usually controlled with medicines.
  • Swelling around the treated area (oedema), which may need a short course of steroids.
  • Radiation necrosis - delayed injury to healthy tissue near the target - which is uncommon and may appear months later.
  • Skin redness or mild hair loss at the beam entry points in some cases.
  • Organ-specific effects depending on the site, such as cough after lung SBRT, temporary bowel or bladder irritation after prostate SBRT, or rarely a vertebral fracture after spinal radiosurgery.
  • Possible need for further treatment if the tumour does not respond fully, as with any cancer therapy.

Your specialist will explain the risks specific to your condition. Choosing an experienced, high-volume hospital keeps complication rates low.

Recovery and Aftercare

  • Most patients return to normal daily activities on the same day or the next day after each session.
  • There are no stitches or wounds, so there is no surgical recovery period.
  • Mild fatigue, headache or nausea usually settles within a few days to two weeks.
  • Patients are generally fit to fly 1-3 days after the final session, subject to the radiation oncologist's review.
  • Follow-up MRI or CT is usually done 6-12 weeks after treatment and then every 3-6 months, and the images can be reviewed remotely by your treating doctor.
  • Tumour shrinkage happens gradually over months; the main goal is often to stop growth permanently rather than make the tumour disappear immediately.
