📖 What are Wilms Tumour, Neuroblastoma and Retinoblastoma?

Childhood cancers are different from adult cancers. They grow from very young cells, often respond well to chemotherapy, and need treatment by paediatric oncologists, paediatric surgeons and child-friendly hospital teams. Wilms tumour (nephroblastoma) is a kidney cancer, usually found before age 5 as a painless swelling in the belly. Neuroblastoma grows from immature nerve cells, most often in the adrenal gland above the kidney; it ranges from low-risk tumours needing only surgery to high-risk disease. Retinoblastoma is a cancer of the retina (the back of the eye), usually before age 3; the first sign is often a white glow in the pupil in photos or a squint.

Treatment is planned by a tumour board using international SIOP (European) or COG (American) protocols. Wilms tumour is treated with chemotherapy and removal of the affected kidney (nephrectomy), with radiotherapy in some stages. Neuroblastoma treatment depends on risk group – surgery, chemotherapy, and for high-risk disease stem cell transplant, radiotherapy and immunotherapy. Retinoblastoma is treated with chemotherapy (into a vein or directly into the eye artery) plus focal therapy – laser, cryotherapy (freezing) or plaque brachytherapy – under anaesthesia (EUA), aiming to save the child's life and, where possible, the eye and vision.

💡 In simple words
Children can get cancers such as Wilms tumour, a kidney cancer usually found in children under 5. Treatment combines surgery to remove the affected kidney, chemotherapy, and sometimes radiotherapy. Many children are cured.

🩺 Cancers Treated

👁️
Retinoblastoma (one or both eyes)
Chemotherapy with laser, cryotherapy or plaque therapy to save the eye; enucleation for advanced eyes.
🎗️
Wilms tumour (nephroblastoma)
Chemotherapy with nephrectomy; radiotherapy for higher stages.
🧒
Neuroblastoma – low & intermediate risk
Surgery alone or with a few cycles of chemotherapy.
⚠️
High-risk neuroblastoma
Intensive chemotherapy, surgery, autologous stem cell transplant, radiotherapy and maintenance.
👀
Bilateral Wilms or retinoblastoma
Special protocols to save kidney tissue or vision in both organs.
🔁
Relapsed tumours
Second-line chemotherapy, surgery or radiotherapy after review by the tumour board.

⚠️ Warning Signs in Children

  • ➤White glow in the pupil in photos (leukocoria)
  • ➤New squint or poor vision in one eye
  • ➤Painless lump or swelling in the belly
  • ➤Blood in the urine or high blood pressure
  • ➤Bluish lumps under the skin or bruising around the eyes (neuroblastoma)
  • ➤Bone pain, limping or unexplained fever
  • ➤Weight loss, poor appetite or tiredness
  • ➤Red, painful or swollen eye
💡 Every child needs a confirmed diagnosis before treatment. Some very small neuroblastomas in babies shrink on their own and only need close observation. Small retinoblastomas may need only focal laser or cryotherapy without chemotherapy. The tumour board decides the lightest treatment that gives the best chance of cure.

🔬 Tests Before Treatment

TestWhy it is done
Ultrasound and CT/MRI of abdomen and chestShows the tumour size, spread and the other kidney
MRI of the orbits and brain (retinoblastoma)Checks spread to the optic nerve and brain
Examination under anaesthesia (EUA) with RetCamMaps eye tumours and grades the eye (ICRB group)
Urine catecholamines (VMA/HVA) and MIBG scanDiagnose and stage neuroblastoma
Biopsy / histopathology and MYCN testingConfirms tumour type and risk group (not usually done for retinoblastoma)
Bone marrow testsChecks spread in neuroblastoma
Blood counts, kidney/liver function, hearing and echoBaseline before chemotherapy

🏥 The Procedure — Step by Step

Paediatric Cancer Treatment procedure steps

⚖️ Treatment Options by Cancer

Cancer / TreatmentWhat is doneBest for
Retinoblastoma – chemo + focal therapyIV or intra-arterial chemo plus laser/cryo/plaque under EUAIntraocular tumours where the eye can be saved
Retinoblastoma – enucleationRemoval of the eye with an orbital implant; artificial eye laterAdvanced eyes (ICRB group D/E) with no useful vision
Wilms tumour – chemo + nephrectomyPre-op chemo (SIOP) or upfront surgery (COG), then post-op chemoMost children with Wilms tumour
Neuroblastoma – surgery ± chemoTumour removal with 2–8 cycles of chemotherapyLow and intermediate risk
High-risk neuroblastoma – multimodalInduction chemo, surgery, stem cell transplant, radiotherapy, maintenanceHigh-risk disease (MYCN-amplified or widespread)
📊 With modern SIOP/COG protocols, about 90% of children with Wilms tumour and over 90–95% of children with intraocular retinoblastoma are cured in high-income countries; low- and intermediate-risk neuroblastoma survival exceeds 90%, while high-risk neuroblastoma survival is about 50–60%.

🏆 Success Rate

OutcomeResult
Wilms tumour – overall survivalAbout 90%
Intraocular retinoblastoma – survivalOver 90–95%
Retinoblastoma – eye salvage (groups A–C)About 80–90% of eyes
Neuroblastoma low/intermediate risk – survivalOver 90%
High-risk neuroblastoma – survivalAbout 50–60%

Results depend most on the stage at diagnosis, tumour biology, completing the full protocol without delay, and access to supportive care. Children who start treatment early and finish it on time do best.

❓ Frequently Asked Questions

➤ What does paediatric cancer treatment cost in India?
Retinoblastoma chemotherapy with focal therapy starts from about USD 4,000, Wilms tumour treatment from about USD 6,500 and intermediate-risk neuroblastoma from about USD 9,000. High-risk neuroblastoma with stem cell transplant costs much more.
➤ How long will we stay in India?
Usually 2–6 months, depending on the protocol. Some families return home between cycles if a local hospital can give supportive care.
➤ Will my child be admitted all the time?
No. Most chemotherapy is given as day care. Hospital stay is mainly for surgery (5–10 days) or if there are complications.
➤ Can my child's eye be saved?
In many cases, yes. Smaller retinoblastomas (groups A–C) are often controlled with chemotherapy and laser or cryotherapy. Very advanced eyes may need removal to save the child's life.
➤ Is retinoblastoma inherited?
About 40% of cases are hereditary, especially when both eyes are affected. Genetic testing and regular eye checks for brothers and sisters are advised.
➤ Will my child live normally with one kidney?
Yes. After Wilms tumour surgery, the remaining kidney grows and works well. Blood pressure and kidney function are checked during follow-up.
➤ What are the chances of cure?
About 90% of children with Wilms tumour and over 90–95% with retinoblastoma inside the eye are cured. Neuroblastoma depends on risk: over 90% for low risk and about 50–60% for high risk.
➤ Which protocols do Indian hospitals follow?
Leading centres follow international SIOP and COG protocols, discussed in a multidisciplinary tumour board for each child.
➤ What are the side effects of chemotherapy?
Low blood counts, infections, nausea, hair loss and tiredness. Most are temporary. Some drugs can affect hearing, so hearing is checked.
