🩺 LIVER CANCER PACKAGE — INDIA

Liver Cancer Treatment (TACE, TARE & Ablation) in India

TACE – transarterial chemoembolisation – treats liver cancer from inside the blood vessels. A thin tube passed from the groin delivers chemotherapy straight into the tumour and then blocks its blood supply, with no open surgery. It is the standard treatment for intermediate-stage hepatocellular carcinoma (HCC) and is also used to control cancer while waiting for a liver transplant.

📖 What is Liver Cancer Treatment with TACE?

The most common primary liver cancer is hepatocellular carcinoma (HCC). It usually develops in a liver already damaged by cirrhosis – most often from long-term hepatitis B or C infection, alcohol, or fatty liver disease. Because the liver has a large reserve, HCC often causes no symptoms until it is large, which is why people with cirrhosis or hepatitis B are advised to have an ultrasound every 6 months.

Treatment depends on the size and number of tumours, whether blood vessels are involved, and how well the liver works (Child-Pugh score). Small tumours in a healthy-enough liver can be cured by liver resection, ablation or liver transplant. When there are several tumours or surgery is not possible, TACE is used: liver tumours get most of their blood from the hepatic artery, so blocking that artery after giving chemotherapy starves the tumour while the normal liver survives on its portal vein blood supply. TARE (radioembolisation, Y-90) works the same way but uses tiny radioactive beads.

🖼️ Understand It in Pictures

💡 In simple words
A cancer is growing in the liver. A doctor passes a thin tube through a small hole in the groin, up to the liver. Medicine is put straight into the tumour, then its blood supply is blocked so it shrinks. There is no big cut, and most people go home in about 2 days.

Liver cancer HCC treatment options by stage: resection, ablation, transplant, TACE, TARE, immunotherapy

🩺 Conditions Treated

🔴
Intermediate-stage HCC
Several or large tumours confined to the liver with good liver function (BCLC stage B) – the main use of TACE.
⏳
Bridge to liver transplant
TACE or ablation keeps the cancer under control while waiting for a transplant.
📉
Downstaging
Shrinking tumours so that a patient may become eligible for transplant or surgery.
🎯
Small HCC not fit for surgery
Ablation (radiofrequency or microwave) for tumours up to about 3 cm, sometimes combined with TACE.
🧬
Liver metastases
Selected tumours that have spread to the liver (e.g. neuroendocrine or bowel cancer) may be treated with TACE or TARE.
🔄
Recurrent liver cancer
New tumours after resection or ablation.

⚠️ Symptoms That May Need Evaluation

  • ➤Pain or a lump in the upper right side of the tummy
  • ➤Unexplained weight loss and loss of appetite
  • ➤Feeling full after small meals
  • ➤Yellow eyes or skin (jaundice)
  • ➤Swelling of the tummy (ascites)
  • ➤Worsening of known cirrhosis
  • ➤A rising AFP blood test
  • ➤A new liver lesion on a surveillance ultrasound
💡 TACE is not suitable for everyone. It is usually avoided when the liver is failing (Child-Pugh C), when the main portal vein is blocked by tumour, or when the cancer has spread outside the liver – in these cases immunotherapy, targeted drugs or supportive care are better. A liver tumour board decides the right option for each patient.

🔬 Tests Before Treatment

TestWhy it is done
Triple-phase contrast CT or MRI of the liverTypical HCC pattern (arterial enhancement and wash-out) confirms the diagnosis in cirrhosis, often without biopsy.
AFP (alpha-fetoprotein) blood testTumour marker used for diagnosis and follow-up.
Liver function tests, albumin, bilirubin, INR, plateletsChild-Pugh and ALBI scores show if the liver can tolerate treatment.
Hepatitis B and C tests (viral load)Antiviral treatment protects the liver during and after TACE.
Kidney function testsContrast dye is used during the procedure.
CT chest or PET-CTChecks for spread outside the liver.
Upper GI endoscopy (if cirrhosis)Looks for varices that may bleed.

🏥 The Procedure — Step by Step

TACE procedure steps for liver cancer
1
Liver tumour board
HPB surgeon, hepatologist, interventional radiologist and oncologist agree that TACE (or another option) is best.
2
Admission
Admission on the day or the day before; blood tests, IV fluids and fasting for about 6 hours.
3
Sedation and local anaesthesia
No general anaesthetic is usually needed; the groin is numbed and you are given medicine to relax.
4
Arterial access
A thin catheter is inserted into the groin artery (sometimes the wrist) and guided to the liver.
5
Angiography
Contrast dye shows the arteries feeding the tumour; a micro-catheter is advanced as close as possible.
6
Chemotherapy delivery
Chemotherapy (such as doxorubicin) is given mixed with lipiodol oil (conventional TACE) or loaded on drug-eluting beads (DEB-TACE).
7
Embolisation
Tiny particles block the tumour’s artery so the drug stays in the tumour and its blood supply is cut off.
8
Catheter removal
The catheter is removed and the groin is pressed or sealed with a closure device; the procedure takes about 1–2 hours.
9
Monitoring
Lie flat for 4–6 hours; pain, nausea and fever medicines are given as needed.
10
Discharge and follow-up scan
Home in about 2 days; a CT or MRI after 4–6 weeks shows the response and whether another session is needed.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Conventional TACE (cTACE)Chemotherapy mixed with lipiodol, then embolisationIntermediate HCC; most widely used
DEB-TACEDrug-eluting beads release chemotherapy slowlySimilar uses; may cause less systemic side effects
Bland embolisation (TAE)Blocking the artery without chemotherapySelected patients, bleeding tumours
TARE / SIRT (Y-90)Radioactive microspheres into the tumour artery; needs a planning angiogram firstLarge tumours, portal vein branch invasion in selected cases
Radiofrequency / microwave ablationNeedle through the skin heats and destroys the tumourTumours up to about 3 cm (sometimes up to 5 cm with TACE)
Liver resection or transplant (separate packages)Surgical removal or replacement of the liverEarly HCC; Milan criteria for transplant
📚 Evidence note: In a randomised trial (Llovet et al., Lancet 2002), chemoembolisation improved survival compared with conservative treatment: 82% vs 63% at 1 year and 63% vs 27% at 2 years. For advanced HCC, the IMbrave150 trial showed that atezolizumab plus bevacizumab improved survival compared with sorafenib. According to the Leeds Teaching Hospitals NHS patient leaflet, about half of patients get some degree of post-embolisation syndrome after TACE.

Typical hospital stay per session2–3 days
💡 TACE usually controls the cancer rather than curing it; many patients need 2 or more sessions. The best results are seen in patients with good liver function and tumours confined to the liver. Small tumours treated by ablation, resection or transplant have the best chance of cure.

🩹 Risks & Possible Complications

  • •Post-embolisation syndrome (fever, pain, nausea, tiredness) – common, about half of patients
  • •Bruising or bleeding at the groin puncture – common bruise; bleeding usually controlled by pressure
  • •Worsening of liver function, especially in advanced cirrhosis – uncommon
  • •Liver abscess or bile duct injury – uncommon
  • •Gallbladder irritation (chemical cholecystitis) causing pain – rare
  • •Chemotherapy side effects such as hair thinning or low blood counts – rare
  • •Kidney strain from contrast dye – rare; fluids are given to protect the kidneys
  • •Liver failure or serious bleeding – rare

❓ Frequently Asked Questions

➤ How much does TACE cost in India?
At BLK-Max, New Delhi, TACE costs about USD 2,750–3,650 per session in an economy room (USD 3,050–4,050 twin, USD 3,350–4,450 single), with about 2 days in hospital. Evaluation of USD 1,000–2,000 is extra.
➤ What is the difference between TACE and TARE?
TACE delivers chemotherapy and then blocks the tumour artery. TARE (Y-90) delivers tiny radioactive beads instead. TARE costs much more (about USD 21,700–26,300 at BLK-Max) and needs a planning procedure first.
➤ Is TACE painful?
It is done under local anaesthesia with sedation. Afterwards, upper tummy pain, fever and nausea are common for a few days and are controlled with medicines.
➤ How many TACE sessions will I need?
Many patients need 2 or more sessions. A scan 4–6 weeks after each session shows whether more treatment is needed.
➤ Can TACE cure liver cancer?
TACE usually controls the cancer and prolongs life rather than curing it. Cure is most likely with resection, ablation or transplant for early tumours; TACE may help some patients become eligible for these.
➤ How long must I stay in India?
About 1–2 weeks for evaluation and one TACE session. Further sessions may need another visit after 4–8 weeks.
➤ Who is not suitable for TACE?
Patients with failing liver function (Child-Pugh C), main portal vein blocked by tumour, spread outside the liver, or poor general health. Other treatments are advised for them.
➤ Is a liver biopsy needed?
Often not. In a cirrhotic liver, a typical pattern on triple-phase CT or MRI can confirm HCC. A biopsy is done if the scans are not typical.
➤ Can I have a liver transplant instead?
If the tumours are within transplant criteria and a suitable family donor is available, living donor liver transplant may be possible. BLK-Max quoted about USD 25,000 for the transplant package.
➤ What medicines are used for advanced liver cancer?
Immunotherapy combinations such as atezolizumab with bevacizumab, or targeted tablets such as lenvatinib or sorafenib, depending on liver function.
➤ Do I need to treat my hepatitis B or C?
Yes. Antiviral treatment protects the liver, lowers the risk of new tumours and is important before and after TACE.
➤ When can I fly home after TACE?
Usually about 5–7 days after the procedure, once fever and pain have settled and blood tests are stable.
➤ What follow-up is needed?
CT or MRI and AFP tests every 3 months at first, with treatment of any new tumours.
➤ Do you provide an interpreter?
Yes. Free interpreters for Arabic, French, Russian and other languages, plus a coordinator for all hospital visits.
