Know More About Procedure & Surgery

Liver Resection (Hepatectomy) is the surgical removal of part of the liver. It is performed to treat liver cancers (primary or metastatic), benign tumours, liver cysts, hepatolithiasis and trauma. India is recognised as one of the world's top destinations for advanced liver surgery, with internationally trained hepatobiliary surgeons performing major resections at JCI/NABH accredited centres — at a cost 60–80% lower than the USA, UK or Europe.

What is Liver Resection Surgery

Liver resection is a major operation in which a portion of the liver is removed surgically. The liver has 8 functional segments and remarkable regenerative capacity — it can regenerate to its near-original size within 6–8 weeks after resection of up to 70% of its volume. This makes liver resection feasible even for large tumours, provided enough healthy liver tissue remains.

Resection can be performed using open surgery, laparoscopic (key-hole) techniques, or robotic-assisted surgery, depending on the size, location and nature of the lesion. Modern Indian centres routinely use intraoperative ultrasound, advanced vascular control techniques and energy devices (CUSA, Harmonic, LigaSure) to minimise blood loss and complications.

Types of Liver Resection

Wedge Resection

Removal of a small wedge-shaped piece of liver tissue — usually for small surface tumours, biopsies or benign lesions.

Segmentectomy

Removal of one of the eight anatomical liver segments — precise, parenchyma-sparing technique ideal for cirrhotic livers.

Lobectomy / Hemihepatectomy

Removal of an entire liver lobe — right hepatectomy (segments 5–8) or left hepatectomy (segments 2–4). Major surgery removing 30–60% of liver volume.

Extended Hepatectomy (Trisectionectomy)

Removal of more than half the liver — extended right or left hepatectomy for large or centrally located tumours. Reserved for fit patients with adequate future liver remnant.

Laparoscopic / Robotic Liver Resection

Minimally invasive approach — smaller incisions, less pain, faster recovery, shorter hospital stay. Suitable for selected peripheral and anterior segment lesions.

Indications for Liver Resection

  • Hepatocellular Carcinoma (HCC) — primary liver cancer
  • Colorectal Liver Metastases — secondary cancer spread from colon/rectum
  • Intrahepatic Cholangiocarcinoma — bile duct cancer within the liver
  • Gallbladder Cancer — extended cholecystectomy with liver resection
  • Benign Tumours — hepatic adenoma, large symptomatic haemangioma, focal nodular hyperplasia
  • Hydatid Cysts — parasitic liver cysts (echinococcosis)
  • Hepatolithiasis — intrahepatic gallstones with recurrent cholangitis
  • Liver Trauma — severe injury with non-viable liver tissue
  • Neuroendocrine Tumour Metastases — selected liver-only or liver-dominant disease

Pre-Operative Evaluation

A thorough workup is essential before liver resection to assess tumour resectability, future liver remnant volume and overall fitness.

Complete Liver Function Tests — bilirubin, albumin, INR, transaminases
Tumour Markers — AFP, CEA, CA 19-9
Viral Hepatitis Screening — HBV, HCV, HIV
Triple-Phase CT & MRI Liver — tumour mapping & vascular anatomy
CT Volumetry — calculation of future liver remnant
PET-CT Scan — for cancer staging & metastatic workup
Cardiac Evaluation — ECG, ECHO, stress test
Pulmonary Function Tests — fitness for major surgery
FibroScan — liver stiffness, fibrosis assessment

The Procedure & Recovery

Surgery Duration

Liver resection typically takes 3–6 hours depending on the extent of resection, tumour location and surgical approach. Performed under general anaesthesia with epidural pain management.

Hospital Stay

7–8 days total: 1–2 days in ICU for monitoring of liver function and vital signs, followed by 5–6 days in a semi-private ward. Drains are usually removed by day 5; sutures by day 7.

Post-Discharge Recovery

Most patients require 2–3 weeks of outpatient follow-up in India for wound check, suture removal and review of histopathology. Full recovery typically takes 4–8 weeks. Liver regenerates to near-original volume within 6–8 weeks.
