📖 What is Stereotactic Radiosurgery (SRS / SRT)?

Some brain tumours and blood-vessel problems are deep, small or close to important areas, making open surgery risky. Stereotactic radiosurgery (SRS) is not surgery in the usual sense – nothing is cut. Dozens or hundreds of thin, precisely shaped radiation beams enter the head from many directions and meet exactly at the target. Each beam alone is weak, but where they meet the dose is very high, while the surrounding brain receives very little.

When the full dose is given in one session it is called SRS. When it is split into 3–5 sessions it is called stereotactic radiotherapy (SRT) or hypofractionated SRS, used for larger tumours or those close to the optic nerves or brainstem. It is given on Gamma Knife, CyberKnife or modern linear accelerators (e.g. TrueBeam, Novalis, Edge). You stay awake, feel no pain and usually go back to your hotel the same day.

🖼️ Understand It in Pictures

💡 In simple words
Stereotactic radiosurgery (SRS) is not surgery: it uses many precise radiation beams that meet at a small target in the brain, giving a high dose with no cut. SRS is one session; SRT splits it into a few sessions.

🩺 Conditions Treated

🎗️
Brain metastases
1 to about 10–15 small spread tumours from lung, breast, kidney or skin cancer.
🧠
Meningioma
Small to medium or leftover tumours after surgery.
👂
Acoustic neuroma (vestibular schwannoma)
Hearing-nerve tumours, often controlling growth while protecting facial nerve.
🔵
Pituitary adenoma
Residual or recurrent pituitary tumours.
🩸
Arteriovenous malformation (AVM)
Abnormal tangle of blood vessels; closes gradually over 2–3 years.
⚡
Trigeminal neuralgia
Severe face pain not controlled by medicines.

⚠️ When Your Doctor May Advise SRS / SRT

  • ➤Small brain tumour seen on MRI
  • ➤Brain metastases from another cancer
  • ➤Leftover tumour after brain surgery
  • ➤Growing acoustic neuroma or hearing loss
  • ➤Patient not fit for open brain surgery
  • ➤Tumour in a deep or high-risk location
  • ➤AVM not suitable for surgery
  • ➤Severe facial pain (trigeminal neuralgia)
💡 SRS is not suitable for every brain lesion. Large tumours (usually over 3–4 cm) causing pressure may need surgery first, very small stable benign tumours may only need MRI monitoring, and many brain metastases or diffuse tumours may need whole-brain or standard radiotherapy. Your free opinion will confirm the best option.

🔬 Tests Before Treatment

TestWhy it is done
MRI brain with contrast (thin slices)Defines the exact size and shape of the target
CT planning scanUsed with MRI for accurate dose calculation
Angiogram (for AVM)Maps the abnormal vessels
Biopsy / histopathology (if available)Confirms tumour type, especially for metastases
Hearing test (acoustic neuroma)Baseline hearing before treatment
Hormone and eye tests (pituitary)Baseline hormones and visual fields
Blood tests (kidney function)Safety for contrast dye

🏥 The Procedure — Step by Step

Stereotactic Radiosurgery procedure steps
1
Consultation
Neurosurgeon and radiation oncologist review MRI and history, and choose SRS or SRT.
2
Immobilisation
A custom thermoplastic mask (frameless) or a light head frame (Gamma Knife) is fitted.
3
Planning scans
Thin-slice MRI and CT scans are taken in the treatment position.
4
Target outlining
Doctors outline the tumour and sensitive structures (optic nerves, brainstem).
5
Physics planning
Medical physicists design the beams to give a high dose to the target and very little to the brain.
6
Plan check
The plan is checked and approved by the team.
7
Treatment session
You lie still and awake for 20–60 minutes; images confirm position. It is painless.
8
Further sessions (SRT)
For SRT, the same process is repeated for 3–5 days.
9
Observation and go home
Short observation, then back to the hotel the same day.
10
Follow-up plan
Steroids if needed and an MRI after 2–3 months.

⚖️ Radiosurgery Systems & Schedules

TechniqueWhat is doneBest for
Gamma Knife SRS~190 cobalt beams focused in one sessionSmall brain metastases, AVM, trigeminal neuralgia
CyberKnife SRS / SRTRobotic LINAC, frameless, 1–5 sessionsTumours near optic nerves, larger lesions
LINAC-based SRS (TrueBeam / Novalis / Edge)Shaped arcs with image guidanceMultiple metastases, meningioma
Hypofractionated SRT (3–5 sessions)Dose split over several daysTumours 2.5–4 cm or near critical structures
Fractionated stereotactic RT (25–30)Many small dosesLarger benign tumours near optic pathway
📊 In the NCCTG N0574 trial (JAMA 2016), patients with 1–3 brain metastases treated with SRS alone had less cognitive decline at 3 months (about 64% vs 92%) than those who also received whole-brain radiotherapy, with no difference in overall survival.

❓ Frequently Asked Questions

➤ What is the cost of SRS / SRT in India?
It starts from about USD 4,500 for 1 SRS session or 3–5 SRT sessions, including planning scans. Gamma Knife, CyberKnife or many targets may cost USD 5,000–8,000.
➤ Do I need to be admitted?
No. SRS/SRT is done as an outpatient or day-care treatment. You stay in a hotel or guest house near the hospital.
➤ How long should I stay in India?
About 7–12 days, including consultation, planning, treatment and a final review.
➤ Is it painful?
No. Radiation is painless. A frame (if used) is fitted with local anaesthesia. Most people use a comfortable mask instead.
➤ Is there any cut or stitches?
No. Nothing is cut. That is why recovery is quick and most people return to normal life within days.
➤ How many sessions will I need?
SRS is 1 session. SRT for larger tumours or tumours near important nerves usually needs 3–5 daily sessions.
➤ When will I know if it worked?
An MRI is done after 2–3 months. Benign tumours often stop growing rather than disappear, which is considered successful.
➤ Which conditions can be treated?
Brain metastases, meningioma, acoustic neuroma, pituitary tumours, AVMs, trigeminal neuralgia and some other small tumours.
➤ What are the side effects?
Mild tiredness, headache or nausea for a few days are common. Brain swelling or radiation damage happen in a small number of patients and are usually treated with steroids.
➤ Can it treat many brain metastases?
Yes. Modern systems can treat several, often up to 10–15 small metastases, depending on total volume and your overall condition.
➤ Is it safe for elderly patients?
Yes. Because there is no surgery or general anaesthesia, it is well suited to older patients and those with other illnesses.
➤ Can an attendant come with me?
Yes. One or two attendants can travel on a medical attendant visa. We send the invitation letter.
