📖 What is Mitral Valve Replacement?

The heart has four valves that act like one-way doors. The mitral valve sits between the left upper chamber (left atrium) and the main pumping chamber (left ventricle). If it becomes narrow (mitral stenosis) or leaky (mitral regurgitation), blood backs up into the lungs. This causes breathlessness, tiredness, swelling of the legs and an irregular heartbeat (atrial fibrillation). Rheumatic heart disease, which follows untreated throat infections in childhood, is the most common cause in Africa, the Middle East and Asia.

In mitral valve replacement (MVR) the surgeon opens the chest, connects you to a heart-lung machine, removes the damaged valve and stitches in a new artificial valve. A mechanical valve (made of carbon) lasts a lifetime but needs lifelong blood thinner (warfarin) with regular INR blood tests. A tissue (bioprosthetic) valve needs no lifelong warfarin but may wear out after 10–15 years. When possible, surgeons repair the valve instead of replacing it.

🖼️ Understand It in Pictures

💡 In simple words
The mitral valve is the door between the left top and bottom chambers of the heart. It can become narrow or leaky, often after rheumatic fever. In open-heart surgery, the damaged valve is replaced with a new valve.

🩺 Conditions Treated

🫀
Rheumatic mitral stenosis
Thick, scarred, narrow valve that cannot be ballooned or repaired.
💧
Severe mitral regurgitation
Leaking valve not suitable for repair.
🦠
Infective endocarditis
Infection destroying the valve leaflets.
🔁
Failed previous repair or balloon
Valve that narrowed or leaked again after earlier treatment.
🧱
Heavy calcification
Hard, calcified valve in older patients.
❤️‍🩹
Mixed mitral valve disease
Both narrowing and leakage, often with tricuspid or aortic valve disease.

⚠️ Symptoms That May Need Valve Surgery

  • ➤Breathlessness on walking or lying flat
  • ➤Waking at night short of breath
  • ➤Tiredness and weakness
  • ➤Fast or irregular heartbeat (palpitations)
  • ➤Swelling of feet and ankles
  • ➤Coughing, sometimes with blood
  • ➤Chest discomfort
  • ➤Stroke or mini-stroke from clots
💡 Not every mitral valve problem needs replacement. Mild disease is treated with medicines and check-ups. Pliable narrow valves may be opened by balloon (BMV), and many leaky valves can be repaired, which is usually better. Your free opinion will advise the best option from your echo.

🔬 Tests Before Treatment

TestWhy it is done
Echocardiogram (2D echo / TEE)Measures valve narrowing, leakage and heart pumping
ECGShows rhythm, such as atrial fibrillation
Coronary angiography (age over 40)Checks for blocked arteries that may need bypass at the same time
Chest X-rayShows heart size and lung congestion
Blood tests (kidney, liver, clotting, INR)Fitness for surgery
Dental check-upTreats infections to protect the new valve
Pulmonary function testLung fitness before surgery

🏥 The Procedure — Step by Step

Mitral Valve Replacement procedure steps
1
Admission and tests
Admission the day before surgery for final tests, blood arrangements and anaesthesia review.
2
General anaesthesia
You are fully asleep; lines and a TEE probe monitor the heart.
3
Opening the chest
A cut in the middle of the chest (sternotomy); some centres offer a small side cut.
4
Heart-lung machine
The machine takes over pumping while the heart is stopped safely.
5
Valve removal
The damaged valve is removed, keeping supporting chords when possible.
6
New valve implanted
The mechanical or tissue valve is stitched in place; other valves fixed if needed.
7
Heart restarted
Air is removed, the heart is restarted and the valve checked by TEE.
8
ICU care
About 6 days in ICU for monitoring, breathing support removal, drains and medicines.
9
Ward recovery
2 days in a single room; walking, physiotherapy and warfarin dose adjustment.
10
Discharge
Discharge with medicines, INR plan and wound-care advice.

⚖️ Valve Options & Techniques

OptionWhat is doneBest for
MVR – mechanical valveCarbon valve, lasts a lifetime, lifelong warfarinYounger patients (under ~60) who can do INR tests
MVR – tissue valvePig or cow tissue valve, no lifelong warfarinOver ~65, women planning pregnancy, poor INR access
Mitral valve repairOwn valve repaired with ring and stitchesDegenerative leaky valves – preferred when possible
Minimally invasive MVRSmall right-side chest cutSelected patients, faster recovery (extra cost)
Balloon mitral valvotomyBalloon opens narrow valve via the groinPliable rheumatic stenosis without heavy calcium
📊 In large registry data (STS database), operative mortality for isolated elective mitral valve replacement is about 2–5%, and 10-year survival after MVR is about 60–75% depending on age and heart function.

🚗
Weeks 6–8
Driving and desk work usually allowed.
💪
Months 2–3
Cardiac rehab; most people feel much less breathless.

🏆 Success Rate

OutcomeResult
Operative survival (elective MVR)About 95–98%
Symptom improvement (NYHA class)Over 85–90% of patients
Mechanical valve durabilityUsually lifelong (over 20–25 years)
Tissue valve freedom from re-operation at 10 yearsAbout 70–90% (lower in the young)
10-year survival after MVRAbout 60–75%

Results depend on heart pumping strength before surgery, lung pressure, other valve disease, age, kidney function and – for mechanical valves – keeping the INR in the target range for life.

🩹 Risks & Possible Complications

MVR is major heart surgery, but outcomes in experienced centres are very good.

  • •Atrial fibrillation (irregular beat) – about 20–40% after surgery, often already present
  • •Bleeding needing re-exploration – about 2–5%
  • •Infection of wound or lungs – about 2–5%
  • •Stroke – about 1–3%
  • •Kidney problems – about 2–5%, mostly temporary
  • •Heart block needing pacemaker – about 1–3%
  • •Valve clot or bleeding from warfarin over the long term – about 1–2% per year

❓ Frequently Asked Questions

➤ What is the cost of mitral valve replacement in India?
It starts from about USD 9,500 with a mechanical valve, including 6 days in ICU and 2 days in a single room. A tissue valve costs about USD 10,000–15,000. A written estimate is given after report review.
➤ Which valve is better – mechanical or tissue?
Mechanical valves last a lifetime but need daily warfarin and regular INR blood tests. Tissue valves avoid lifelong warfarin but may wear out in 10–15 years, faster in young people. Your surgeon will advise based on your age, lifestyle and access to INR testing.
➤ How long will I stay in hospital?
About 8 days in total – roughly 6 days in ICU and 2 days in a ward room.
➤ How long should I stay in India?
Plan for about 3–4 weeks. This covers tests, surgery, hospital stay and enough time to stabilise your warfarin dose before flying home.
➤ What is the success rate?
For planned surgery, about 95–98% of patients come through the operation safely, and most feel much less breathless within weeks.
➤ Can my valve be repaired instead?
Sometimes. Leaky valves due to wear are often repairable. Rheumatic valves that are thick and calcified usually need replacement. The surgeon decides during evaluation and sometimes during surgery.
