Know More About Procedure & Surgery
Balloon Mitral Valvotomy (BMV / PTMC) in India
A catheter procedure for rheumatic mitral stenosis that opens the narrowed valve with a balloon passed through a groin vein, done by experienced interventional cardiologists at JCI & NABH accredited hospitals in India – India has one of the largest BMV experiences in the world.
📖 What is Balloon Mitral Valvotomy?
The mitral valve sits between the upper left chamber (left atrium) and the main pumping chamber (left ventricle) of the heart. After rheumatic fever in childhood – still common in Africa, South Asia and the Middle East – the valve leaflets can thicken and stick together over the years. The valve opening becomes narrow, called mitral stenosis. Blood backs up into the lungs, causing breathlessness, tiredness, palpitations (often atrial fibrillation) and sometimes coughing up blood or stroke.
Balloon mitral valvotomy (BMV), also called percutaneous transvenous mitral commissurotomy (PTMC) or balloon mitral valvuloplasty, opens the valve without open-heart surgery. A thin tube is passed from a vein in the groin to the heart. The cardiologist crosses from the right to the left upper chamber through a tiny puncture in the wall between them and inflates a special Inoue balloon across the valve. This splits the fused edges of the leaflets, usually doubling the valve opening within minutes.

🩺 Who Is BMV Suitable For?
⚠️ Symptoms That May Need BMV
- ➤Breathlessness on walking or climbing stairs
- ➤Breathlessness when lying flat or at night
- ➤Tiredness and low energy
- ➤Palpitations or irregular heartbeat
- ➤Coughing up blood-stained sputum
- ➤Swelling of the feet
- ➤Chest discomfort
- ➤Stroke or mini-stroke from a blood clot
🔬 Tests Before Treatment
| Test | Why it is done |
|---|---|
| Transthoracic echocardiography (2D echo) | Measures valve area, pressure gradient, lung pressure and the Wilkins score. |
| Transoesophageal echo (TEE) | Rules out a clot in the left atrium and checks valve leakage – essential before BMV. |
| ECG | Checks for atrial fibrillation and heart rhythm. |
| Chest X-ray | Shows heart size and lung congestion. |
| Blood tests incl. INR and kidney function | Checks clotting (especially on warfarin), kidneys and blood count. |
| Pregnancy test (women of child-bearing age) | Allows X-ray shielding and planning if pregnant. |
🏥 The Procedure — Step by Step

⚖️ BMV Compared With Other Treatments
| Treatment | What is done | Best for |
|---|---|---|
| Balloon mitral valvotomy (BMV/PTMC) | Balloon opens the valve through a groin vein | Pliable valves, no clot, mild or no leakage |
| Closed mitral commissurotomy | Valve opened by finger or dilator through a side chest cut | Rarely used now; some low-resource settings |
| Open mitral commissurotomy | Valve repaired under direct vision on bypass | Clot in the left atrium or valve not suitable for balloon |
| Mitral valve replacement (MVR) | Valve replaced with a mechanical or tissue valve | Calcified valves or significant leakage |
| Medicines | Diuretics, beta-blockers, blood thinners, penicillin | Mild disease and control of symptoms |

🌱 Recovery After Treatment
🏆 Success Rate
| Outcome | Result |
|---|---|
| Immediate success (good opening without severe leak) | About 85–95% |
| Typical valve area after BMV | About doubles (e.g. 1.0 → 2.0 cm²) |
| Free from re-intervention at 10 years | About 60–80% with favourable valves |
| Severe mitral regurgitation needing surgery | About 2–5% |
| Hospital stay in this package | 2–3 days |
Results are best in younger patients with pliable, non-calcified valves (low Wilkins score), normal heart rhythm and no significant leakage. Heavily calcified valves have higher rates of re-narrowing and may do better with valve replacement.
🩹 Risks & Possible Complications
BMV is safe in experienced hands, but it is a heart procedure with some important risks.
- •Groin bruising or bleeding – common, usually minor
- •Severe mitral valve leakage (regurgitation) – about 2–5%, may need valve surgery
- •Small leftover hole between the upper chambers – common, usually closes or is harmless
- •Fluid around the heart (cardiac tamponade) – about 0.5–1%, needs drainage
- •Stroke or embolism – about 0.5–1%
- •Heart rhythm problems – uncommon, usually temporary
- •Risk of death – under about 0.5%
