📖 What is Coronary Angiography?

The heart is a muscle that gets its blood supply from three main coronary arteries. Fatty deposits (plaque) can build up inside these arteries and make them narrow. This is called coronary artery disease. It can cause chest pain (angina), breathlessness and heart attacks. Risk is higher with diabetes, high blood pressure, high cholesterol, smoking and a family history of heart disease.

Coronary angiography (also called a coronary angiogram or cardiac catheterisation) is the gold-standard test to see these arteries. A thin, soft tube (catheter) is passed from the wrist or groin to the heart under local anaesthetic. A special dye (contrast) is injected and X-ray pictures show any blockage. The cardiologist then advises if you need only medicines, a stent (angioplasty) or bypass surgery (CABG). If a stent is needed, it can often be done in the same sitting.

🩺 Conditions Diagnosed

🫀
Coronary artery disease
Finds narrowing or blockage in the heart arteries and measures how severe it is.
💔
After a heart attack
Shows which artery is blocked so it can be opened quickly with a stent.
😮‍💨
Unexplained chest pain
Checks if chest pain or breathlessness is coming from the heart arteries.
📉
Abnormal stress test
Confirms a positive TMT, stress echo or CT angiography result.
🩺
Before heart valve surgery
Checks the arteries before valve replacement so both can be planned together.
❤️‍🩹
Heart failure / weak heart
Looks for blocked arteries as a cause of a weak heart muscle (cardiomyopathy).

⚠️ Symptoms / Signs That May Need Angiography

  • ➤Chest pain or tightness on walking or climbing stairs
  • ➤Pain spreading to the arm, jaw, neck or back
  • ➤Breathlessness with mild effort
  • ➤Sudden severe chest pain (possible heart attack)
  • ➤Positive treadmill (TMT) or stress echo test
  • ➤Reduced heart pumping (low EF) on echo
  • ➤Abnormal ECG changes
  • ➤Heart rhythm problems or fainting with heart disease
💡 Not everyone with chest pain needs an angiogram. If symptoms are mild and the risk is low, doctors may first advise an ECG, echo, treadmill test or a non-invasive CT coronary angiography. Angiography is chosen when these tests are abnormal or when treatment (stent or bypass) may be needed.

🔬 Tests Before Treatment

TestWhy it is done
ECG and EchocardiographyChecks heart rhythm and how well the heart pumps
Kidney function (creatinine)The contrast dye passes through the kidneys; checks they can handle it safely
Complete blood count and clotting (PT/INR)Checks for anaemia and bleeding risk
Blood sugar / HbA1cDiabetes control; some diabetes medicines are paused before the test
Viral markers (HIV, HBsAg, HCV)Standard safety screening before any invasive procedure
TMT or stress test (if not already done)Shows if the heart muscle is short of blood during exercise

🏥 The Procedure — Step by Step

Coronary Angiography procedure steps
1
Admission to day-care
You arrive fasting for 4–6 hours. Reports are checked and consent is signed.
2
Pre-procedure checks
Blood pressure, ECG and blood tests are reviewed. A small IV line is placed in the arm.
3
Move to cath lab
You lie on the X-ray table, awake. Mild sedation can be given if you are anxious.
4
Local anaesthetic
The wrist (radial artery) or groin (femoral artery) is numbed with an injection.
5
Catheter placement
A thin catheter is guided through the artery to the heart under live X-ray. You feel no pain inside.
6
Dye injection and X-ray pictures
Contrast is injected into each coronary artery and video X-rays are recorded from several angles.
7
Catheter removal
The catheter is removed. A tight band (wrist) or closure device/pressure (groin) stops bleeding.
8
Observation
You rest for 4–6 hours while the puncture site and vital signs are monitored. You drink fluids to flush the dye.
9
Results and plan
The cardiologist shows you the pictures and explains: medicines, stent or bypass.
10
Discharge
Most patients go home the same day with written instructions.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Radial (wrist) angiographyCatheter passed through the wrist arteryMost patients; fewer bleeding problems, walk soon after
Femoral (groin) angiographyCatheter passed through the groin arteryWhen wrist arteries are small or for complex cases
CT coronary angiographyCT scan with dye through a vein, no catheterLow-to-medium risk patients, screening
Angiography with FFR / IVUSPressure wire or ultrasound inside the arteryBorderline blockages, to decide if a stent is really needed
Angiography + angioplasty (ad hoc)Stent placed in the same sittingSignificant blockage found during the test (extra cost)
📊 Large studies and the RIVAL and MATRIX trials showed that wrist (radial) access lowers major bleeding compared with groin access, and serious complications of diagnostic angiography occur in well under 1% of patients.

🏆 Success Rate

OutcomeResult
Accurate diagnosis of artery blockageGold-standard test, close to 100% visualisation
Major complications (stroke, heart attack, death)Less than 1 in 100 (about 0.1–0.2% for death)
Same-day dischargeMost patients
Wrist (radial) access successAbout 95% of patients

Safety depends on your age, kidney function, diabetes, how unwell the heart is and the experience of the cath lab team. Stable patients having a planned test have the lowest risk.

🩹 Risks & Possible Complications

Coronary angiography is a very safe test. Possible problems include:

  • •Bruising or small bleeding at the wrist or groin (common, usually minor)
  • •Allergic reaction to the dye – usually mild itching or rash; severe reactions are rare
  • •Temporary kidney strain from the dye (higher risk with kidney disease or diabetes)
  • •Irregular heartbeat during the test, usually brief
  • •Blocked wrist artery (radial occlusion) in about 1–5%, usually without symptoms
  • •Stroke, heart attack or serious artery damage – rare (under 1 in 100)

💰 Cost of Coronary Angiography in India

The cost of coronary angiography in India starts from about USD 300 for a day-care procedure. This covers the cath lab, cardiologist fee, contrast dye, catheters, routine pre-test investigations and observation in the day-care ward.

❓ Frequently Asked Questions

➤ How much does angiography cost in India?
Coronary angiography in India starts from about USD 300 as a day-care procedure. The price includes the cath lab, cardiologist fee, dye and routine tests. A stent, if needed, costs extra.
➤ Is angiography painful?
No, it is usually not painful. You get a local anaesthetic at the wrist or groin, so you only feel a small prick. You may feel a warm flush for a few seconds when the dye is injected.
➤ How long does the test take?
The test itself takes about 20–40 minutes. With preparation and observation, you spend about 4–6 hours in hospital.
➤ Will I need to stay in hospital overnight?
Most patients go home the same day. An overnight stay is advised only if a stent is placed, the groin route is used in a high-risk patient, or there is a complication.
➤ How long should I stay in India?
Plan for about 3–5 days for consultation, tests, the angiogram and the result review. If you need a stent, add 2–3 days; for bypass surgery, plan 2–3 weeks.
➤ What happens if a blockage is found?
The cardiologist will explain the result. Mild blockages are treated with medicines. Significant blockages may be treated with a stent in the same sitting (with your consent) or with bypass surgery.
➤ Is the wrist or groin route better?
Most Indian centres use the wrist (radial) route. It has less bleeding and you can sit up and walk soon after. The groin route is used when needed.
➤ What are the risks?
Serious problems such as stroke or heart attack happen in less than 1 in 100 patients. Minor bruising at the puncture site is common. The dye can strain weak kidneys, so kidney tests are done first.
➤ I have kidney disease or diabetes. Can I have angiography?
Yes, with precautions. Doctors give IV fluids, use less dye and may stop metformin for 48 hours. Your kidney function is checked after the test.
➤ When can I fly home?
Usually 1–2 days after a simple angiogram, once the doctor says you are fit to fly. After a stent, most patients fly after 3–5 days.
➤ Should I stop my medicines before the test?
Do not stop anything yourself. Blood thinners like warfarin and some diabetes medicines may need to be paused. Our coordinator will share the cardiologist's instructions before you travel.
➤ Can a family member come with me?
Yes. One or two attendants can get a medical attendant visa. We send the visa invitation letter for the patient and attendants.
➤ Is an interpreter available?
Yes. Satyug Healthcare provides interpreters for Arabic, French, Russian and other languages, plus a coordinator throughout your stay.
➤ Will I get my angiogram pictures?
Yes. You get the full report and the angiogram video on a CD or download link to show your doctor at home.
