What is Glaucoma?

Glaucoma is a group of eye diseases that damage the optic nerve, the cable that carries visual information from the eye to the brain. In most cases the damage is linked to raised pressure inside the eye (intraocular pressure, or IOP), caused by fluid (aqueous humour) not draining properly. However, glaucoma can also occur with normal eye pressure. As nerve fibres die, blind spots develop, usually starting in the side (peripheral) vision and gradually moving towards the centre.

Main types of glaucoma

  • Primary open-angle glaucoma (POAG): the most common type worldwide. The drainage angle looks open, but fluid drains too slowly. It progresses gradually and painlessly.
  • Primary angle-closure glaucoma (PACG): the iris blocks the drainage angle. It can be chronic or appear suddenly as an acute attack, which is a medical emergency. Angle closure is relatively more common in people of Asian origin.
  • Normal-tension glaucoma: optic nerve damage occurs even though eye pressure is in the normal range.
  • Secondary glaucoma: caused by another condition or factor, such as steroid use, eye injury, inflammation (uveitis), advanced cataract, diabetes-related new blood vessels (neovascular glaucoma) or pseudoexfoliation.
  • Congenital and childhood glaucoma: present at birth or developing in early childhood, often signalled by large, cloudy eyes, watering and sensitivity to light.

Causes and Risk Factors

Anyone can develop glaucoma, but the risk is higher in people with:

  • Age over 40, with risk increasing further after 60.
  • African ancestry: people of African descent are more likely to develop open-angle glaucoma, often at a younger age and in a more severe form. Regular eye screening is especially important for patients from Africa.
  • Family history: having a parent or sibling with glaucoma significantly increases risk.
  • High eye pressure and a thin cornea.
  • Diabetes, high blood pressure and very low blood pressure, as well as migraine and sleep apnoea in some cases.
  • High myopia (short-sightedness), which increases the risk of open-angle glaucoma, and long-sightedness, which increases the risk of angle-closure glaucoma.
  • Long-term use of steroids, including steroid eye drops, inhalers, tablets and creams. In some regions, unsupervised use of steroid eye drops bought over the counter is an important cause of avoidable glaucoma.
  • Previous eye injury, surgery or inflammation.

Symptoms of Glaucoma

Open-angle glaucoma usually causes no symptoms in the early stages. As it progresses, people may notice:

  • Patchy blind spots in side vision, often in both eyes
  • Bumping into objects or difficulty with stairs and kerbs
  • Tunnel vision in advanced stages
  • Difficulty seeing in dim light or adjusting to darkness

Warning signs of acute angle-closure glaucoma (emergency)

Go to an eye emergency service immediately if you experience a sudden, severe eye pain, a red eye, blurred vision, coloured halos or rainbows around lights, headache, nausea or vomiting. An acute attack can cause permanent blindness within hours or days if not treated urgently. In babies, enlarged or cloudy eyes, constant watering and light sensitivity need urgent review by a paediatric eye specialist.

Diagnosis of Glaucoma

Glaucoma is diagnosed through a comprehensive eye examination. An ophthalmologist typically performs:

  • Tonometry to measure eye pressure (Goldmann applanation is the standard method).
  • Gonioscopy to examine the drainage angle and determine whether it is open or closed.
  • Dilated optic nerve examination to assess the size and shape of the optic disc and look for cupping.
  • Optical coherence tomography (OCT) to measure the thickness of the retinal nerve fibre layer and detect damage early.
  • Visual field test (perimetry) to map blind spots in peripheral vision.
  • Pachymetry to measure corneal thickness, which affects pressure readings and risk.
  • Anterior segment OCT or ultrasound biomicroscopy in angle-closure cases.

Treatment Options for Glaucoma

The goal of treatment is to lower eye pressure to a "target" level that prevents further optic nerve damage. Treatment depends on the type and stage of glaucoma.

Eye drops

Medicated eye drops are the most common first treatment. They include prostaglandin analogues, beta-blockers, carbonic anhydrase inhibitors, alpha agonists and newer agents such as rho-kinase inhibitors. Many patients need more than one drop. Drops must be used every day, exactly as prescribed, even when the eyes feel fine.

Laser treatment

  • Selective laser trabeculoplasty (SLT): a quick outpatient procedure that improves drainage in open-angle glaucoma. It is increasingly used as a first-line treatment and can reduce or eliminate the need for drops for some years.
  • Laser peripheral iridotomy (LPI): creates a tiny opening in the iris to relieve or prevent angle closure.
  • Cyclophotocoagulation (including micropulse laser): reduces fluid production and is used for advanced or difficult cases.

Surgery

  • Trabeculectomy: the traditional "filtration" surgery that creates a new drainage channel. It remains highly effective for lowering pressure.
  • Glaucoma drainage devices: tube implants such as the Ahmed or Aurolab valves, used in complex, secondary or previously failed cases.
  • Minimally invasive glaucoma surgery (MIGS): tiny stents and micro-incisional procedures, often combined with cataract surgery, offering faster recovery for mild to moderate glaucoma.
  • Lens extraction: in angle-closure glaucoma, removing the natural lens (cataract surgery) can open the angle and control pressure.
  • Paediatric glaucoma surgery: goniotomy, trabeculotomy and combined procedures for children.
