About the Procedure
- The filter is inserted by an interventional radiologist through a vein in the neck or groin under local anaesthesia.
- A venogram is done first to measure the IVC and locate the kidney veins.
- The collapsed filter is released below the kidney veins, where it opens and anchors to the vein wall.
- Most filters used today are retrievable and should be removed once the risk has passed, usually within weeks to months.
Purpose of the Procedure
- Prevent pulmonary embolism in patients with DVT who cannot receive anticoagulation
- Protect patients who develop new clots despite adequate blood thinners
- Short-term protection before major surgery, trauma or cancer treatment in high-risk patients
Diagnosis and Tests Before the Procedure
- Venous Doppler of the legs confirming DVT
- CT pulmonary angiography if lung clots are suspected
- Clotting profile, CBC and kidney function
- Review of bleeding risk and reason blood thinners cannot be used
Step-by-Step Procedure
- Consent and pre-procedure checks
- Local anaesthesia at the neck (jugular) or groin (femoral) vein
- Venogram (cavogram) to measure the IVC
- Filter deployed below the renal veins under X-ray
- Final image to confirm position; sheath removed
- Observation for a few hours to overnight
Risks and Possible Complications
- Bruising or bleeding at the puncture site
- Filter tilt, movement or fracture (rare)
- Clot formation at the filter or IVC blockage over time
- Perforation of the IVC wall by filter legs (usually without symptoms)
- Difficult retrieval if left in for a long time
Your specialist will explain the risks specific to your condition. Choosing an experienced, high-volume hospital keeps complication rates very low.
Recovery and Aftercare
- Most patients go home the same day or next day
- Normal activity in 1-2 days
- Blood thinners are restarted as soon as safe
- Plan for filter retrieval at the appropriate time
