🦷 VASCULAR & ORTHOPAEDIC SURGERY PACKAGE — INDIA

Amputation Surgery in India (Below-Knee, Above-Knee & Foot)

Amputation is the planned surgical removal of part of a limb that is dead, badly infected, crushed beyond repair or affected by a tumour. In India it is done by vascular, orthopaedic and plastic surgeons working together at accredited hospitals, with wound care, physiotherapy and prosthesis planning. Before any amputation, our partner teams first check whether the limb can be saved with blood-flow treatment or infection control.

🦷 What is Amputation Surgery?

An amputation removes a toe, foot, leg, finger, hand or arm when the tissue cannot survive or is a danger to the patient’s life. The most common reasons are poor blood flow (peripheral arterial disease, often with diabetes), severe infection or gangrene, major injuries and bone or soft-tissue cancers. In the United States, vascular disease (including diabetes) accounts for about 54% of people living with limb loss, trauma about 45% and cancer under 2% (Ziegler-Graham et al., Archives of Physical Medicine and Rehabilitation, 2008).

The surgeon removes the diseased part at the lowest level that will heal. Keeping the knee is very valuable: people with a below-knee amputation usually walk with a prosthesis more easily than people with an above-knee amputation. The bone end is smoothed, nerves are cut cleanly so they sit away from pressure points, and muscle and skin are closed to form a well-padded residual limb (stump) that can later fit an artificial limb.

Amputation is often a life-saving operation, not a failure of treatment. If the leg can be saved, our partner hospitals also offer Peripheral Balloon Angioplasty, Femoro-Popliteal Bypass and, for bone tumours, Limb Salvage Surgery.

🖼️ Understand It in Pictures

💡 In simple words
When part of a leg or arm has no blood supply, is badly infected or is crushed, it can poison the whole body. The surgeon removes only the part that cannot be saved and shapes the end of the limb into a smooth, padded stump. After healing, an artificial limb and physiotherapy help most people walk and live independently again.

🔍 Why Amputation May Be Needed

🩸
Peripheral arterial disease
Blocked leg arteries that cannot be opened by angioplasty or bypass, with dead tissue or rest pain.
🧀
Diabetic foot infection / gangrene
Deep infection, bone infection (osteomyelitis) or wet gangrene spreading up the foot.
💥
Severe injury
Crushed or mangled limbs from road, work or blast injuries that cannot be reconstructed.
🎗
Bone and soft-tissue cancer
When limb-salvage surgery cannot remove the tumour safely.
❄️
Frostbite, burns, electric injury
Tissue that has died after cold, heat or electrical injury.
🦺
Deformity or non-healing limb
A painful, useless limb after failed operations, where a prosthesis would work better.

⚠️ Warning Signs and When Amputation Can Be Avoided

  • ➤Black or blue toes or skin (gangrene)
  • ➤Foot ulcer that is not healing after weeks of treatment
  • ➤Severe pain in the foot at rest, worse at night
  • ➤Spreading redness, swelling, pus or foul smell
  • ➤Fever and high blood sugar with a foot wound
  • ➤Cold, pale foot with weak or absent pulses
  • ➤Bone visible in a wound
  • ➤Crush injury with no feeling or blood flow below it
💡 Not every case needs amputation. Many diabetic foot wounds heal with infection control, wound care, off-loading and restoring blood flow (angioplasty or bypass). Ask for a limb-salvage opinion before agreeing to a major amputation. Satyug sends your reports to both vascular and orthopaedic surgeons so you receive both opinions. If amputation is needed, a smaller amputation done early often saves a bigger one later.

🔬 Tests Before the Procedure

Test / checkWhy it matters
Doppler ultrasound / ankle-brachial index (ABI)Measures blood flow; shows whether a lower level will heal.
CT or MR angiographyMaps blocked arteries and checks whether angioplasty or bypass is possible.
X-ray and MRI of the foot or limbDetects bone infection (osteomyelitis), gas or fractures.
Blood tests: CBC, HbA1c, kidney function, CRP, clottingInfection level, diabetes control and fitness for surgery.
Wound swab / tissue cultureChooses the right antibiotics.
ECG, echo and anaesthesia checkMost patients have heart and kidney risks that must be checked.

🏥 The Procedure — Step by Step

Amputation surgery steps
1
Multispecialty review
Vascular surgeon, orthopaedic surgeon, diabetologist and physician review your scans and wounds.
2
Infection control
Antibiotics and, if needed, an urgent clean-up (debridement) or drainage before the final operation.
3
Level decision
The lowest level with good blood flow is chosen so the wound heals and walking is easiest.
4
Anaesthesia
Spinal, epidural or general anaesthesia; a nerve catheter can give pain relief for several days.
5
Removal
The diseased part is removed and blood vessels are tied off.
6
Bone and nerve care
Bone edges are smoothed; nerves are cut cleanly and allowed to retract to reduce later nerve pain.
7
Stump shaping
Muscles are stitched over the bone (myodesis / myoplasty) to pad and stabilise the stump.
8
Closure
Skin is closed, sometimes with a drain; heavily infected wounds may be left open and closed a few days later.
9
Early mobilisation
You sit out of bed in 1–2 days and begin physiotherapy, exercises and stump shaping with bandages.
10
Prosthesis planning
Once the wound heals and swelling settles, a prosthetist measures you for a temporary then a permanent limb.

⚖️ Types of Amputation Compared

TypeWhat is removedTypical hospital stayWalking afterwards
Toe / ray amputationOne or more toes with part of the foot bone2–4 daysUsually walk with special shoes or insoles
Transmetatarsal / SymeFront of the foot / foot at the ankle3–5 daysSpecial shoe or short prosthesis
Below-knee (transtibial)Leg below the knee; knee kept5–7 daysMost active patients walk well with a prosthesis
Through-knee / above-kneeLeg at or above the knee5–10 daysMore effort to walk; prosthetic knee needed
Upper-limb (finger, hand, arm)Part of the arm or hand2–5 daysHand therapy; body-powered or myoelectric prosthesis
📚 Evidence note: Surgeons aim for the lowest level that will heal because each higher level adds to the energy needed for walking and lowers the chance of walking with a prosthesis. Choosing the level uses blood-flow tests (ABI, toe pressures, TcPO2, angiography) and how the tissue looks at surgery (StatPearls: Lower Extremity Amputation; NHS: Amputation).

🏆 Outcomes After Amputation

MeasureWhat to expect
Main aimRemove dead or infected tissue and save life; relieve severe pain
Wound healingBest when blood flow at the chosen level is adequate and diabetes is controlled
WalkingMost active below-knee amputees walk with a prosthesis; above-knee walking needs more energy and training
Phantom limb painCommon in the first months; usually improves with time, medicines and therapy
Return to workDepends on the job and level; many return after rehabilitation
💡 Results depend heavily on blood flow, diabetes control, not smoking and rehabilitation. Patients with severe heart, kidney or vascular disease have higher risks; your surgeon will discuss your personal risk before surgery.

🩹 Risks & Possible Complications

  • •Wound infection or slow healing - more common with diabetes, smoking and poor circulation
  • •Need for a further operation or a higher amputation if the wound does not heal
  • •Bleeding or haematoma
  • •Blood clots in the legs or lungs (DVT / pulmonary embolism) - prevented with blood thinners and early movement
  • •Phantom limb pain and stump (residual limb) pain or neuroma
  • •Joint stiffness (contracture) of the knee or hip if exercises are not done
  • •Heart attack, stroke or kidney problems in high-risk patients
  • •Skin breakdown from a poorly fitting prosthesis

💰 Cost of Amputation Surgery in India

Amputation surgery in India costs an estimated USD 3,200–6,000 for a single-limb operation at an accredited hospital, depending on the level, infection, diabetes control, room type and length of stay. Complex cases with both feet, severe infection or major medical problems cost more.

❓ Frequently Asked Questions

➤ How much does amputation surgery cost in India?
An estimated USD 3,200–6,000 for a single-limb (below-knee or above-knee) amputation at an accredited hospital, with 5–7 days in hospital. Complex cases cost more; one partner hospital quoted about USD 8,000 for a bilateral diabetic foot amputation. The final price comes from the hospital after reviewing reports.
➤ Can my leg be saved instead of amputated?
Sometimes. If blood flow can be restored by angioplasty or bypass and the infection controlled, a limb can often be saved. We send your reports to vascular and orthopaedic surgeons for a limb-salvage opinion first.
➤ How long will I stay in hospital?
Usually 5–7 days for a below-knee or above-knee amputation, longer with severe infection or medical problems.
➤ How long should I stay in India?
About 3–4 weeks, so the wound can be checked, stitches removed and a rehabilitation and prosthesis plan made before you fly.
➤ When can I get an artificial leg?
A temporary prosthesis is usually possible once the wound has healed and swelling settles, often around 6–8 weeks; the permanent limb comes later when the stump shape is stable.
➤ What is phantom limb pain?
Pain or other feelings that seem to come from the removed part. It is common in the first months and usually improves with time, medicines, mirror therapy and physiotherapy.
➤ Is below-knee better than above-knee amputation?
When it can heal, yes. Keeping the knee makes walking with a prosthesis easier and needs less energy. The surgeon chooses the lowest level that will heal.
➤ Is amputation done under general anaesthesia?
Spinal, epidural or general anaesthesia can be used, often with a nerve block. The anaesthetist chooses based on your heart, lungs and kidneys.
➤ I have diabetes and kidney disease. Can I still have surgery?
Yes, but the risks are higher. The team controls blood sugar, checks heart and kidney function and plans dialysis if needed before surgery.
➤ What happens if the wound does not heal?
Further cleaning, wound therapy or, sometimes, a revision at a higher level may be needed. Not smoking and good sugar control lower this risk.
➤ Will I be able to walk again?
Many people walk again with a prosthesis after rehabilitation, especially after below-knee amputation. Age, fitness, heart and lung health and motivation all matter.
➤ Is the prosthesis included in the package?
No. Prosthesis cost depends on the level and type (socket, foot, knee) and is quoted separately by the prosthetist.
➤ Do you provide an interpreter?
Yes. Free interpreters for Arabic, French and Russian (plus English); our coordinators speak English, Arabic, French and Portuguese.
