📖 What is Sympathetic Trunk Reconstruction?
The sympathetic trunk (sympathetic chain) is a pair of nerve chains running along the spine inside the chest. It controls sweating, blood vessel size and some heart and gut functions. For very sweaty hands, face blushing or armpit sweating, some patients have an operation called endoscopic thoracic sympathectomy (ETS), which cuts or clips this chain. Many patients later develop compensatory sweating – heavy sweating of the back, chest, belly or legs. In a small number it is severe and disabling.
Sympathetic trunk reconstruction (STR) is a rare, highly specialised microsurgery that tries to restore the cut nerve chain. Through keyhole (thoracoscopic or robot-assisted) surgery, the surgeon finds the cut ends and bridges the gap with a nerve graft, most often the sural nerve taken from the back of the leg, or an intercostal nerve from between the ribs. If ETS was done with clips, simply removing the clips may be tried first. The nerve regrows slowly along the graft over many months, so improvement is gradual and not guaranteed.
