📖 What is Kyphosis and Kyphosis Correction Surgery?

The spine normally has gentle curves. In the upper back (thoracic spine), a forward curve of about 20–45 degrees is normal. Kyphosis means this forward curve is too large, giving a rounded or hunched back. Mild kyphosis often causes no trouble, but a severe or worsening curve can cause back pain, tiredness, difficulty standing straight, breathing problems and, in some cases, pressure on the spinal cord causing weakness of the legs.

Kyphosis correction surgery straightens and stabilises the spine. Through an incision in the back, the surgeon places screws in the vertebrae, removes wedges of bone if needed (an osteotomy such as Ponte, pedicle subtraction osteotomy or vertebral column resection), and connects the screws with rods to pull the spine into a better position. Bone graft is added so the vertebrae heal into one solid piece (spinal fusion). Spinal cord monitoring is used throughout to keep the nerves safe.

🖼️ Understand It in Pictures

💡 In simple words
In kyphosis, the upper back curves forward too much, giving a rounded or hunched back. Causes include wedge-shaped or collapsed spine bones. If the curve is severe, painful or worsening, surgery uses screws and rods to straighten the spine and fuse the bones.

🩺 Conditions Treated

🧒
Scheuermann's kyphosis
Wedge-shaped vertebrae in teenagers causing a rigid curve, usually over 70–75 degrees when surgery is needed.
👶
Congenital kyphosis
Abnormal vertebrae from birth that can worsen quickly and threaten the spinal cord.
🦠
Post-tubercular kyphosis
Angular hunchback after spinal TB (Pott's spine), common in Africa and Asia.
💥
Post-traumatic kyphosis
Deformity after old spine fractures that healed in a bent position.
👵
Degenerative / osteoporotic kyphosis
Curve from disc wear or collapsed vertebrae in older adults, often with pain.
🔧
Ankylosing spondylitis & flatback
Fixed forward bend from inflammatory arthritis or after earlier spine surgery.

⚠️ Symptoms That May Need Treatment

  • ➤Visible hump or rounded upper back
  • ➤Back pain that limits daily life
  • ➤Difficulty standing upright or looking ahead
  • ➤Curve getting worse on X-rays
  • ➤Tiredness in back muscles
  • ➤Breathlessness from a severe curve
  • ➤Numbness or weakness in the legs
  • ➤Problems with bladder or bowel control (urgent)
💡 Most kyphosis does not need surgery. Postural kyphosis is corrected with exercise and posture training. Mild to moderate Scheuermann's kyphosis in growing children is treated with physiotherapy and bracing. Pain is first managed with medicines, physiotherapy and core strengthening. Surgery is considered for large or worsening curves, severe pain, or nerve compression.

🔬 Tests Before Treatment

TestWhy it is done
Full-length standing spine X-raysMeasures the kyphosis angle (Cobb angle) and overall body balance.
Bending / bolster X-raysShows how flexible the curve is, which decides the type of osteotomy.
MRI of the whole spineChecks the spinal cord, discs and nerve compression.
CT scan with 3D reconstructionShows abnormal vertebrae and helps plan screw placement.
Lung function testsChecks breathing capacity in severe curves.
Blood tests, DEXA scan (adults)Fitness for surgery and bone strength for screw fixation.
Cardiac evaluationECG and echo before a long operation.

🏥 The Procedure — Step by Step

Kyphosis Correction Surgery procedure steps
1
Consultation and planning
The spine surgeon studies your X-rays, CT and MRI and plans the levels and osteotomy.
2
Admission
You are admitted a day before for tests, blood arrangement and consent.
3
General anaesthesia
You are fully asleep; neuromonitoring electrodes are attached.
4
Exposure
A straight incision is made along the middle of the back.
5
Screw placement
Pedicle screws are placed into the vertebrae, often with navigation or X-ray guidance.
6
Osteotomy
Wedges of bone are removed (Ponte, PSO or VCR) to allow the spine to bend back.
7
Correction
Rods are fixed to the screws and the spine is gently straightened while the spinal cord is monitored.
8
Bone grafting
Bone graft is laid along the spine to create a solid fusion.
9
Closure
The wound is closed in layers, usually with a drain.
10
Recovery and discharge
ICU or HDU for 1 day, walking from day 2, discharge in 5–6 days.

⚖️ Types & Techniques

TechniqueWhat is doneBest for
Posterior spinal fusion with Ponte osteotomiesSmall wedge cuts at several levels plus screws and rodsFlexible curves such as Scheuermann's kyphosis
Pedicle subtraction osteotomy (PSO)One larger wedge removed through the back of a vertebra, about 30 degrees correctionRigid curves, ankylosing spondylitis, flatback
Vertebral column resection (VCR)An entire vertebra removed for maximum correctionSevere angular curves, congenital or post-TB kyphosis
Kyphoplasty / vertebroplastyBone cement injected into a collapsed vertebraPainful osteoporotic fractures, not large deformities
Hemivertebra excision (children)Removal of an abnormal half-vertebra with short fusionYoung children with congenital kyphosis
📊 Studies of posterior-only fusion for Scheuermann's kyphosis report curve correction of about 40–55% (for example 54% with posterior-only pedicle-screw fusion and 41% with anterior-posterior fusion; Lee, Lenke et al., Spine 2006) with high patient satisfaction. Large reviews of adult spinal deformity surgery report overall complication rates of about 20–40%, while new permanent neurological deficits are uncommon (about 1–2%) with neuromonitoring.

Results depend on the cause and stiffness of the curve, bone quality, age, smoking and the surgeon's experience with osteotomies. Younger patients with Scheuermann's kyphosis usually recover fastest, while older adults with osteoporosis may need longer recovery.

🩹 Risks & Possible Complications

Spinal deformity correction is a major operation; careful planning and neuromonitoring reduce the risks.

  • •Blood loss needing transfusion (common in osteotomy surgery)
  • •Wound infection (about 2–5%)
  • •Junctional kyphosis – new curve above or below the fusion (about 10–30%)
  • •Screw loosening, rod breakage or non-union needing revision (about 5–10%)
  • •Nerve or spinal cord injury causing weakness (about 1–2%, higher in VCR)
  • •Dural tear with fluid leak (about 2–5%)
  • •Blood clots, lung problems or chest infection

❓ Frequently Asked Questions

➤ What is the cost of kyphosis surgery in India?
Kyphosis correction surgery in India starts from about USD 12,500 with a 5–6 day hospital stay. Complex cases needing vertebral column resection cost about USD 16,000–20,000, compared with over USD 100,000 in the USA.
➤ How long does the surgery take?
Usually 4–7 hours, depending on the number of levels and the type of osteotomy.
➤ How long must I stay in India?
Plan for about 3–4 weeks: tests, surgery, a 5–6 day hospital stay, and 2 weeks of recovery and physiotherapy before flying home.
➤ How much will my back be straightened?
Most patients get about 40–55% correction of the curve, giving a much more upright posture. The aim is a balanced, stable spine, not a perfectly straight one.
➤ Is the surgery safe for the spinal cord?
Spinal cord monitoring is used throughout surgery so the team can act at once if nerve signals change. The risk of permanent nerve damage is about 1–2%.
➤ Is kyphosis surgery painful?
You are asleep during surgery. Afterwards pain is controlled with strong medicines, then tablets. Most pain settles within 4–6 weeks.
➤ When can I walk after surgery?
Most patients sit up and walk with help on day 1 or 2 after surgery.
➤ When can I return to school or work?
Students and desk workers usually return in 6–8 weeks. Heavy physical work may take 4–6 months.
➤ Will I need a brace?
Many patients do not need a brace after modern screw-rod fixation. Some surgeons advise a light brace for a few weeks, especially in weak bones.
➤ Can children and teenagers have this surgery?
Yes. Teenagers with Scheuermann's kyphosis and young children with congenital kyphosis are commonly treated. Timing is planned around growth.
➤ Can old TB kyphosis be corrected?
Yes. Post-tubercular kyphosis can be corrected, often with vertebral column resection, once TB is fully treated. These are complex cases done by experienced deformity surgeons.
➤ Can a family member come with me?
Yes, an attendant is recommended. We provide visa invitation letters for you and one or two attendants.
➤ What reports do I need for a free opinion?
Send standing full-spine X-rays, MRI of the spine and CT scan if available. Our spine surgeons reply within 48–72 hours.
➤ What follow-up is needed at home?
X-rays at 6 weeks, 3 months, 6 months and 1 year. Send them to us online and the surgeon will review your fusion and alignment.
