ICSI Treatment in India
ICSI (Intracytoplasmic Sperm Injection) is an advanced form of IVF in which a single healthy sperm is injected directly into each mature egg in the laboratory. It was developed to help couples with male-factor infertility, such as a very low sperm count or poor sperm movement, but it is now also used after failed fertilisation in earlier IVF cycles and with frozen or surgically retrieved sperm. About the Procedure
- ICSI follows the same basic steps as IVF (ovarian stimulation, egg collection, fertilisation, embryo culture and embryo transfer), but fertilisation is done by injecting one sperm into each egg with a fine glass needle under a high-power microscope.
- It is the treatment of choice for male-factor infertility, including low sperm count, poor motility, abnormal sperm shape and high DNA fragmentation.
- Men with no sperm in the semen (azoospermia) can often still father a child through ICSI using sperm retrieved surgically from the testis or epididymis by procedures such as PESA, TESA or micro-TESE.
- ICSI is also used when previous conventional IVF gave poor or no fertilisation, when only a few eggs are available, and with frozen eggs or frozen sperm.
- Embryos can be grown to the blastocyst stage (day 5) and either transferred fresh or frozen for a later frozen embryo transfer, which in some patients improves the chance of implantation.
- Optional add-ons such as laser-assisted hatching, embryo glue or pre-implantation genetic testing (PGT) are offered only when the specialist feels they are needed.

Step-by-Step Procedure
- Planning visit: the couple meets the fertility specialist, tests are completed and a personalised stimulation protocol is chosen.
- Ovarian stimulation: the woman takes daily hormone injections for about 9-12 days to help several eggs mature.
- Monitoring: regular ultrasound scans and blood tests track follicle growth and adjust medicine doses.
- Trigger injection: once follicles are ready, a final injection is given to complete egg maturation, about 34-36 hours before collection.
- Egg retrieval: under short sedation, eggs are collected through the vagina with a fine needle guided by ultrasound; it takes about 20-30 minutes and the woman goes home the same day.
- Sperm preparation: the male partner gives a fresh semen sample on the same day, or sperm are retrieved surgically or thawed from storage.
- ICSI: the embryologist injects a single selected sperm into each mature egg; fertilisation is checked the next day.
- Embryo culture and transfer: embryos are grown for 3-5 days and the best one or two are placed into the uterus with a thin catheter; surplus good embryos can be frozen.
- Pregnancy test: a blood beta-hCG test is done about 12-14 days after embryo transfer.
Risks and Possible Complications
- Ovarian hyperstimulation syndrome (OHSS), causing bloating, abdominal pain and fluid build-up; modern protocols and freeze-all strategies have made severe OHSS uncommon.
- Mild cramping, spotting or discomfort after egg retrieval, and rarely bleeding or infection.
- Some eggs may be damaged during injection or may not fertilise, and not every embryo will develop well.
- Multiple pregnancy if more than one embryo is transferred, which carries higher risks for mother and babies; single embryo transfer is often advised.
- Ectopic pregnancy or miscarriage, as can happen with any pregnancy.
- A possible small increase in some genetic or developmental conditions, mostly linked to the underlying male infertility rather than the technique itself.
- Emotional stress, as success is not guaranteed and more than one cycle may be needed.
