📖 What is a Spinal Cord Stimulator?

Some pain continues long after an injury or surgery has healed because the nerves themselves keep sending pain signals. This is called chronic neuropathic (nerve) pain. Common examples are persistent leg and back pain after spine surgery (failed back surgery syndrome), complex regional pain syndrome (CRPS) and painful diabetic neuropathy. Strong medicines often help only a little and cause side effects.

A spinal cord stimulator (SCS) is a small device, like a heart pacemaker, with thin wires (leads) placed in the space over the spinal cord (epidural space) and a battery (pulse generator) placed under the skin of the buttock or tummy. It sends mild electrical pulses that block or change pain signals before they reach the brain. Every patient first has a trial of about 5–7 days with temporary leads; only if pain falls by at least 50% is the permanent system implanted. This package uses a non-rechargeable battery.

🖼️ Understand It in Pictures

💡 In simple words
Some people have long-term nerve pain in the back or legs that continues even after surgery or other treatment. A spinal cord stimulator is a small implant that sends gentle electrical pulses to the spinal cord to block pain signals. A trial is done first to check it helps.

🩺 Conditions Treated

🦴
Failed back surgery syndrome
Ongoing leg and back pain after one or more spine operations – the most common reason for SCS.
🔥
Complex regional pain syndrome (CRPS)
Severe burning pain, swelling and skin changes in an arm or leg after injury.
🦶
Painful diabetic neuropathy
Burning or electric pain in the feet not controlled by medicines.
⚡
Chronic radiculopathy (sciatica)
Long-lasting nerve-root leg pain where further surgery is not advised.
🫀
Refractory angina and limb ischaemia
Selected heart and circulation pain in specialist centres.
🧠
Other nerve pain
Selected post-herpetic, peripheral nerve injury and phantom limb pain.

⚠️ Who May Benefit From SCS?

  • ➤Leg or back pain for more than 6 months
  • ➤Pain continuing after spine surgery
  • ➤Burning, shooting or electric-shock pain
  • ➤Numbness or pins and needles with pain
  • ➤Pain not controlled by medicines and injections
  • ➤Side effects from strong painkillers
  • ➤Not a candidate for more spine surgery
  • ➤Poor sleep and daily function due to pain
💡 SCS is not a first-line treatment. Physiotherapy, nerve-pain medicines, injections, radiofrequency treatment and psychological pain programmes are tried first, and surgery is considered if there is a fixable cause such as a new disc herniation. SCS is not suitable for people with untreated depression, active infection, bleeding disorders, or when the trial does not give at least 50% relief.

🔬 Tests Before Treatment

TestWhy it is done
MRI of the spineChecks the spinal canal and rules out a problem that needs surgery instead.
X-rays of the spineShows previous implants and the space for leads.
Pain specialist assessmentConfirms the type of pain and that other treatments have been tried.
Psychological assessmentRecommended by guidelines to check mood and expectations, which affect results.
Blood tests, clotting and sugarEnsures safe needle placement and good healing.
ECG and anaesthesia checkConfirms fitness for sedation and surgery.

🏥 The Procedure — Step by Step

Spinal Cord Stimulator Implant procedure steps
1
Assessment
The pain specialist and spine surgeon review your history, scans and psychological assessment.
2
Trial lead placement
Under local anaesthesia and X-ray guidance, temporary leads are placed in the epidural space (1 day in hospital).
3
Programming
Leads are connected to an external stimulator and adjusted so the pulses cover your painful area.
4
Trial period
You go about normal life for about 5–7 days and keep a pain and activity diary.
5
Trial review
If pain falls by 50% or more and function improves, the permanent system is planned; trial leads are removed.
6
Permanent lead implant
Permanent leads are placed (by needle or a small laminotomy) and anchored.
7
Battery implant
A non-rechargeable pulse generator is placed under the skin of the buttock or tummy and connected.
8
Testing
The system is tested during surgery to confirm good coverage.
9
Hospital stay
Wound care, antibiotics and device programming over 2–3 days.
10
Discharge and training
You learn to use the hand-held remote and get activity rules before going home.

⚖️ Types of SCS Systems

SystemWhat is doneBest for
Non-rechargeable SCS (this package)Battery lasts several years, then replaced; no daily chargingLow-energy settings, patients who prefer no charging
Rechargeable SCSBattery charged through the skin, lasts about 9–10+ yearsHigher energy needs, younger patients
High-frequency (10 kHz) SCSParaesthesia-free stimulation, strong for back painBack-predominant pain, diabetic neuropathy
Burst / closed-loop systemsNewer waveforms or automatic adjustmentPatients not helped by conventional settings
DRG stimulationLeads placed on specific nerve-root gangliaFocal pain such as CRPS of the foot or groin
📊 In the PROCESS randomised trial (Kumar et al., Pain 2007), 48% of patients with failed back surgery syndrome treated with SCS achieved at least 50% leg pain relief at 6 months versus 9% with medical management alone; the SENZA-RCT (Kapural et al., 2015) reported about 80% responders with 10 kHz SCS.

🩹
Week 1–2
Wound soreness over the back and battery site; keep wounds dry; short walks encouraged.
🚫
Week 1–6
Avoid bending, twisting, lifting over 2–3 kg and raising arms above the head so the leads do not move.
💻
Week 2–4
Return to light activity and desk work.
⚡
Week 2–8
Settings are fine-tuned online or by the company representative to get the best relief.
🏃
Week 6–12
Gradual return to normal activity once leads have scarred in place.
🔋
Several years
Non-rechargeable battery replaced by a short procedure when it runs out.

🏆 Success Rate

OutcomeResult
Patients with ≥50% pain relief (good candidates)About 50–80%
Patients who pass the trialAbout 60–80%
Reduction in pain medicinesCommon – many reduce or stop opioids
Need for revision surgery over yearsAbout 10–30%
Hospital stay in this packageTrial 1 day + implant 2–3 days

Results are best in carefully selected patients with nerve-type leg pain, a clearly successful trial, good emotional health and realistic goals. Results from conventional settings may fade over years and can often be improved by reprogramming.

🩹 Risks & Possible Complications

SCS is a reversible, low-risk treatment; most problems relate to the hardware.

  • •Lead movement (migration) reducing pain coverage – about 5–15%, may need repositioning
  • •Pain at the battery site – about 5–10%
  • •Infection – about 2–5%, may require device removal
  • •Hardware problems such as lead fracture – about 5–10% over years
  • •Spinal fluid leak with headache – about 1%
  • •Loss of benefit over time – some patients
  • •Bleeding or spinal cord injury – rare, under 1%

❓ Frequently Asked Questions

➤ How much does a spinal cord stimulator cost in India?
A spinal cord stimulator in India starts from about USD 13,500 for a trial plus a non-rechargeable system. Rechargeable systems cost about USD 16,000 and high-frequency/MRI-compatible systems about USD 19,000.
➤ Why is a trial needed?
The trial lets you test SCS in daily life for about 5–7 days. The permanent system is implanted only if your pain falls by at least 50%, so you do not pay for a device that does not help.
➤ What happens if the trial does not work?
The temporary leads are removed and you are charged only for the trial. The team will discuss other treatment options.
➤ How long do I need to stay in India?
About 2–3 weeks: tests, a 1-day hospital stay for the trial, about a week of trial at the hotel, 2–3 days in hospital for the implant and a week for wound checks and programming.
➤ How successful is SCS?
About 50–80% of carefully selected patients get at least 50% pain relief, and many reduce their pain medicines.
➤ Does it hurt?
The trial and implant are done under local anaesthesia with sedation or general anaesthesia. Wound soreness after the implant lasts 1–2 weeks.
➤ What does stimulation feel like?
Conventional SCS gives a gentle tingling over the painful area. High-frequency systems usually give relief without any tingling.
➤ How long does the battery last?
A non-rechargeable battery usually lasts several years, depending on the settings; it is then replaced with a short procedure. Rechargeable batteries last about 9–10 years or more.
➤ Can I have an MRI with an SCS?
Many modern systems are MRI-conditional, which means MRI is possible under certain conditions. If you may need frequent MRIs, the MRI-compatible package is recommended.
