📖 What is Craniosynostosis?

A baby's skull is made of several bony plates joined by flexible seams called sutures. These stay open in the first years of life so the brain can grow quickly. In craniosynostosis, one or more sutures close too early. The skull cannot grow in that direction, so it grows abnormally in other directions, giving an unusual head shape. It affects about 1 in 2,000–2,500 babies. When many sutures are involved, pressure inside the skull can rise and affect the brain, eyes and development.

Craniosynostosis surgery opens the fused suture and reshapes the skull. In babies under about 3–6 months, endoscopic strip craniectomy through two small cuts can be done, followed by a moulding helmet. In older babies, cranial vault remodelling or fronto-orbital advancement is done through a zig-zag cut across the top of the head, in which the skull bones are removed, reshaped and fixed back with dissolvable plates. The goal is a normal head shape and enough space for the brain.

🖼️ Understand It in Pictures

💡 In simple words
A baby’s skull is made of plates joined by soft seams so the head can grow. In craniosynostosis, one or more seams close too early, giving an unusual head shape and sometimes pressure. Surgery opens the seam and reshapes the skull so your child’s brain has room to grow.

🩺 Types of Craniosynostosis Treated

📏
Sagittal synostosis (scaphocephaly)
Most common type; long, narrow head from front to back.
🔺
Metopic synostosis (trigonocephaly)
Triangular forehead with a ridge in the middle and close-set eyes.
↘️
Unicoronal synostosis
Flattened forehead on one side and raised eye socket (anterior plagiocephaly).
⬜
Bicoronal synostosis (brachycephaly)
Short, wide head with flat, high forehead; often with syndromes.
↙️
Lambdoid synostosis
Rare; flat area at the back of the head on one side.
🧬
Syndromic / multi-suture
Apert, Crouzon, Pfeiffer or Saethre-Chotzen syndromes needing staged surgery.

⚠️ Signs That May Need Evaluation

  • ➤Unusual head shape from birth or early months
  • ➤Hard ridge along a skull seam
  • ➤Soft spot (fontanelle) closed early or absent
  • ➤Head not growing normally
  • ➤Uneven forehead or eyes
  • ➤Bulging eyes (in syndromic cases)
  • ➤Irritability, vomiting or headaches (raised pressure)
  • ➤Developmental delay
💡 Most flat heads in babies are NOT craniosynostosis. Positional (deformational) plagiocephaly from lying on one side is very common and improves with repositioning, tummy time and sometimes a helmet – no surgery is needed. A specialist examination and a low-dose CT scan confirm true craniosynostosis.

🔬 Tests Before Treatment

TestWhy it is done
Clinical examination by craniofacial teamHead shape, suture ridges, measurements and photos.
Low-dose 3D CT scan of the skullConfirms which sutures are fused and helps plan surgery.
Eye examination (fundoscopy)Checks for signs of raised pressure on the optic nerve.
MRI brain (selected cases)Looks for brain or venous anomalies, especially in syndromic cases.
Genetic testing (selected cases)For multi-suture or syndromic craniosynostosis.
Blood tests, blood group and cross-matchBabies often need a blood transfusion during open surgery.

🏥 The Procedure — Step by Step

Craniosynostosis Surgery procedure steps
1
Team consultation
Paediatric neurosurgeon, craniofacial plastic surgeon and anaesthetist review the baby and CT scan.
2
Pre-operative preparation
Blood tests, blood arranged, and the paediatric anaesthesia check.
3
General anaesthesia
The baby is put to sleep with careful monitoring by a paediatric anaesthetist.
4
Incision
Two small cuts (endoscopic) or a zig-zag cut from ear to ear hidden in the hair (open).
5
Release of the fused suture
The fused suture is removed to free the skull.
6
Reshaping the skull
In open surgery, skull bones are removed, reshaped and repositioned.
7
Fixation
Bones are fixed with dissolvable plates and screws.
8
Closure
The scalp is closed with dissolvable stitches; a head bandage is applied.
9
Paediatric ICU care
The baby is closely watched for 1–2 days; blood count and swelling are monitored.
10
Ward care and discharge
Feeding, swelling settles; discharge after 7–8 days with follow-up and helmet if needed.

⚖️ Surgical Techniques

TechniqueWhat is doneBest for
Endoscopic strip craniectomy + helmetFused suture removed through 2 small cuts; helmet for 6–12 monthsBabies under 3–6 months, single suture
Spring-assisted cranioplastySuture removed and springs placed to widen the skullSagittal synostosis at 3–6 months
Cranial vault remodellingOpen surgery to reshape the top and back of the skullBabies over 6 months, sagittal or lambdoid
Fronto-orbital advancementForehead and brow reshaped and moved forwardMetopic and coronal synostosis, 9–12 months
Posterior vault distractionGradual expansion of the back of the skullSyndromic or multi-suture cases
📊 Published series from major craniofacial centres report good to excellent head shape in about 85–95% of single-suture cases, with re-operation rates of about 5–10%; endoscopic surgery in young infants reduces blood transfusion and hospital stay compared with open remodelling.

🏆 Success Rate

OutcomeResult
Good head shape (single suture)About 85–95%
Re-operation neededAbout 5–10%
Blood transfusion – open surgeryCommon (60–90%)
Blood transfusion – endoscopicAbout 10–25%
Serious complication rateUnder 1–2%

Results are best with early diagnosis, the right technique for the baby's age and suture type, and a dedicated paediatric craniofacial team. Syndromic and multi-suture cases often need more than one operation as the child grows.

🩹 Risks & Possible Complications

Craniosynostosis surgery is major surgery in a small child, but serious problems are uncommon in expert centres.

  • •Need for blood transfusion (common in open surgery)
  • •Swelling of face and eyes for a few days (very common)
  • •Infection (about 1–2%)
  • •Leak of spinal fluid through a tear in the brain covering (dural tear, about 1–5%)
  • •Irregularities or bony gaps in the skull
  • •Need for repeat surgery (about 5–10%)
  • •Very rarely, brain injury, seizures or death (under 0.5%)

❓ Frequently Asked Questions

➤ What is the cost of craniosynostosis surgery in India?
It starts from about USD 8,000 with a 7–8 day private room stay. Complex or syndromic cases and helmet therapy cost more.
➤ What is the best age for surgery?
Endoscopic surgery is best before 3–6 months. Open skull remodelling is usually done between 6 and 12 months.
➤ How long will we stay in India?
About 3 weeks: tests, surgery, 7–8 days in hospital and a check-up before flying.
➤ Is the surgery safe?
In experienced centres serious complications are uncommon (under 1–2%). A full paediatric team and blood bank are always available.
➤ Will my baby need a blood transfusion?
Often yes in open surgery, because babies have little blood volume. Transfusion is much less common with endoscopic surgery.
➤ Will there be a big scar?
Open surgery uses a zig-zag scar across the top of the head, hidden by hair as it grows. Endoscopic surgery leaves two small scars.
➤ Is the baby in pain after surgery?
Pain is controlled with regular medicines. Swelling around the eyes is common for 2–3 days, then improves quickly.
➤ Does it affect brain development?
Surgery gives the brain room to grow. Most children with single-suture craniosynostosis develop normally. Regular development checks are advised.
➤ Will the head shape be normal?
About 85–95% of single-suture cases have a good to excellent shape. Some children need minor further surgery.
➤ Is a helmet needed?
A moulding helmet is needed for about 6–12 months after endoscopic surgery; usually not after open surgery.
➤ What follow-up is needed?
Check-ups at 1, 3, 6 and 12 months and then yearly, including head measurement and eye checks. Our team arranges video reviews.
➤ Can both parents travel?
Yes, both parents can travel on medical attendant visas. Satyug provides the visa invitation letter.
➤ What should we send for a free opinion?
Photos of the head from all sides, any CT scan or X-ray, the child's age, weight and medical history.
➤ Will an interpreter be available?
Yes, Satyug coordinators speak English, Arabic, French and Portuguese, and interpreters support Arabic, French and Portuguese (plus English).
